Did you tried clean flash COS12.1 or Color OS.
This usually happens due to incompatible Modem with the firmware, bootloader combination.
I also experienced similar issue sometime back using Sultan's build, which is my primary OS right now.
check this post out and the thread.

Hope this helps!

Well I did the full stock rom to cos 11 updating it until the final incremental cos 13 but no success , never tried color Os I will look! any Rom in specific ?!

Try flashing latest CyanogenOS using fastboot:
https://cyngn.com/support
And if needed modems separately.
All the best!!

I had the same issue a few months ago with CM13 builds. My TWRP recovery was the problem. Try updaing it to the newest version.

i had a similar problem after OTA update to CM13.1. So upset I just waited a few days to calm down then went on here and found how someone said sim issues sometimes just need a new sim card. so i went down to my carrier metropcs and they put in a sim adapter for one thing my sim was nano and needed to be made to fit opo micro slot. then they did a refresh and it was good as if it never happened. cost me 15 bucks but worth it compared to what i thought i was up against.

After restoring from lost IMEI problems with cellular network

After I tried to flash Oxygen OS some weeks back and loosing IMEI and WiFi with that, I was able to recover my IMEI and WiFi is working, too. However I have huge problems with the cellular network: I don't have any data transfer when I am not on HSDPA+, and the phone doesn't switch to GSM anymore.
Here's what I did:
1) Installed ColorOS using the help of the No IMEI/WIFI/BASEBAND? thread and got WiFi back
2) Flashed Kitkat (33R) with fastboot and got cellular network back (somehow)
3) OPO started to automatically update using OTA in several steps up to latest Lollipop with 230 modem - no more cellular network
The status bar shows the "SIM card missing" icon, switches to the empty triangle then to the filled triangle and immediately back to the "SIM card missing" icon in a constant loop.
4) Flashed 44S modem with TWRP 2.8.6.0 -> status as above: only data connection in HSDPA+ mode, no data connection in HSDPA and no connection at all otherwise
I am using the German E-Plus network.
I was trying all kinds of modems and tips. I tried the above procedure many times. This post is my last try ...
Any help is highly appreciated.

Just flash "fastboot images" of either latest CMOS12 or CM11S or OxygenOS
The guide is available as a sticky thread in general section
If issue unsolved, flash different modem zip available in general section(use search option or index thread)

inexplicable and random network losses

My Moto G XT1039 (Peregrine) was running perfectly on CM 12.1 since July 2015. But two weeks ago it lost network access. This happened without any update or change on the phone. I tried the basic settings : reboot, toggle airplane mode, re-insert sim card... without any success.
Then I followed this thread to take it back to stock : still no network. After that I installed CM13 with this thread : network is back !
But after two days it lost network again. This time I swapped my sim card with another one : ok, problem solved.
For a few days only ! I then fastboot flashed this radio firmware : great, network is back !
And gone again... And now after flashing stock again, CM13 again, radio firmware again, it doesn't connect at all. Except from that everything works, including bluetooth, wifi and gps. The sim card is new and working in other phones. The exact error is : "Error while searching for networks" when I go to Settings | Mobile networks | Network operators.
I'm completely lost and I can't figure what is the root cause of all this. What do you suggest ? Is there a low level log file somewhere on the phone, in which I could find an error like "unable to start radio because XXX" ?
Thanks !
I installed CM13 once again using TWRP. And it's been running fine for the last 3 weeks ! I will never understand what the problem was...
It only loose network for a few seconds from time to time, but this may be normal.
The same problem occured again.
But now I'm sure it's related to the phone and hundreds of people have the same problem. There is a 96 pages topic on official motorola / lenovo support forum about it.
I can't link it here but it's the first result if you type "Post KitKat upgrade BUGs (Loss of signal etc)" in google.

SIM not recognised after reboot

Hi All,
My Oneplus is currently on CM13.0 (6.0.1). Whenever I restart my phone it does not recognise the SIM. The only way for the SIM to be recognised again is to restart the phone around 6-10 times. This happened back when I was on CM12 so I flashed CM13 hoping that would resolve the issue. It did not.
Any assistance would be appreciated.
EDIT: TWRP version is 2.8.0.1
EDIT2: The issue was the SIM card. Replaced it and it's working fine now.

Try to check if your baseband is still showing something, instead of unknown, and check for IMEI too.
If baseband is unknown and IMIE is showing nothing, you have to flash Oxygen OS to get back your modem.
I had exactly the same problem.

The latest, I have 2 bacons, the China version seems to fine, but the US version had missing baseband(sim card cant be detected) and IMEI after clean flashed to Cyanogen OS 13.1.
I'm not sure if mine is same as your case, i basically factory reset COS13 via latest twrp, side loaded the Oxygen OS and the baseband was back.
Or may be you should try to check or clean your sim card?:good:
Brother are you trying with jio sim in india? If yes then forget about it and if no then do a fresh installation of cm 11 and then upgrade to next cm updates by OTA. If that doesnt helps then it might be problem with the network receptor. Cant help you with that.
Sim issue was common in OPO when it was launced so u can find some solution video on yoitube as well

Weird Connection Issue - Two AT&T Connections

I'm having a super weird issue with my OPO. No matter what I do (install different OS, flash new modem, complete wipe including formatting drives to a different format type) I keep getting two AT&T connections on my phone. *Can't post image due to post restrictions*
I am using a WIND sim from Canada with roaming in the US. I used to be able to connect to my choice of either AT&T or Tmobile. Now when I try and connect to one of those carriers, it flashes registered on network but then does not change what network it is registered on. I've tried disconnecting from the network and then trying to connect. Nothing! The only network it will let me connect to is that weird extra AT&T network, and all call through it have no caller ID, just says call from AT&T.
Any ideas of what this could be would really help me out. I'm all out of ideas
Update - After uninstalling and re-installing everything I could think of, I am now running on CM11. If I update through OTA to CM13 the problem returns.
Update 2 - In case anyone else encounters this issue, after a whole lot of work, including softbricking my phone, I managed to get it working. You must install TWRP Materialized and install the 44S baseband. No other recovery will work, must be Materialized. After I installed the 44S baseband I updated to Omnirom 6.0 and everything is working great!
