You have to untick the option in CM11S' Developer Options regarding recovery - if you don't it won't stick

Yeap, I think the solution is a factory reset on the recovery with reflashing the CM11S fastboot image.

Big THUMB up!! I had the same AudioFx stopped issue and followed your post and now it's good again. THANKS!
I am very confused. I pushed the newest OTA to my phone today and it appeared that It over wrote my customer recovery with the CM recovery. When I got home, I used my laptop to push the philz.img to my phone through fastboot. It installed and recovery booted and I was able to re-flash superSU.zip to re-root.
Now, every time I go into my recovery, it boots into the CM recovery vs Philz and for some reason I cannot get into my bootloader. When I reboot-> bootloader from my reboot screen, it brings me directly to fastboot vs the bootloader. Can someone please help me out. Why do I no longer have a bootloader and how can I get my recovery to stay?
When you see the gray CyanogenMod icon with the gray words "Fastboot Mode" that is bootloader mode ( http://i.imgur.com/VUjyuKs.jpg )
If you want your recovery to stick go into settings -> developer options -> update cm recovery (uncheck it) and then flash your recovery in fastboot mode or by using flashify.
Activate developer options by settings -> about phone -> tap build # 10 times -> back -> developer options if developer options is not enabled.

The bootloader option is suppose to bring you to fastboot, that's the purpose of the bootloader reboot option.
As for recovery being over-written, in developer settings just disable "Update CM recovery" and reflash your recovery. It should no longer be overwritten when any new OTA's come out.
Thanks so much! I re-flashed philz in fastboot once I checked the setting in dev setting and it finally stuck!
As for bootloader, I guess I was just pictured the stock android on where is shows a bunch of phone info, and then says fastboot, recovery and reboot underneath

Yeah I prefer how the Nexus 5 styles the bootloader. It looks way better and its nice that you can use it to navigate to reboot to various parts of the phone (phone/recovery/bootloader/etc)

Hi guys!
I have installed twrp recently but i decided to install stock recovery back. After installing stock recovery still twrp appears.(tried it 3x times)
Any idea?
Thank you for your answers.
Are you using fastboot to flash the recovery? If so what gets displayed when it fails?
As BakedOnSomeSour said, stock recovery must be flashed through fastboot using the same command you used to flash TWRP, and so "fastboot flash recovery <recoveryname>.img". That's the only way to flash a recovery (unless that you use dload and reflash full firmware).

Allow USB debug and OEM unlock by developer settings menu and retry... Plus, tell me what your fastboot says about FRP and Phone

If FRP is locked, read my post above. OEM unlock must be enabled by developer options.
Anyway, procure yourself a valid copy of your actual firmware's recovery and retry a flash by fastboot. Then, try to boot directly to recovery
Thank you for your asnwers. Finally i fixed my problem.
