Hey welcome to xda and the ONEPlus forum. Just so you know this is a place where you should do as much troubleshooting prior to posting. Now questions:
1. have you checked the md5 to make sure it matches the zip file (Google it)
2. are you sure the file you downloaded is not corrupted
3. have you tried to open up the zip file on a pc to see if there are any issues
4. have you tried to download the file again and make another attempt
5. have you tried to download other files and flash it to pinpoint issue.
Android is great but dont play around with a phone that you will have to wait for another invite if you are unsure of what you are doing. I am saying this as someone trying to help you and not flame you....
Try the above any issues let me know

after wiping all, click on mount, uncheck system & then flash & you are good to go

google md5 checker and then go back to where you got the file and check what the md5 number is. Got to google this stuff my friend. but why dont you just download the file again. All the info you need is out there but you got to get your hands dirty and search and learn about them. Its only going to help you in the long run :good:

go to advanced wipe and simply check system, dalvik cache and cache. Nice clean wipe of system folder will always help. Remember you will have to flash gapps again as well(if necessary)
Full stock CM11S rom no need to worry about gapps

I had same issue I fixed it like this: after I pushed the cm11 zip from PC to phone using cmd, I didn't just go ahead and installed it. Instead i disconnect the phone did a reboot back into the twrp recovery, AND THEN installed the cm11 zip. That worked for me and no errors.

Are you trying to flash the factory image from the CM site? Of so, it won't work as it's not a flashable ZIP

is there some other issue with your phone that is making you need to flash this? I ask only because you should be able to get an OTA update to that build directly from "System updates" in Settings under About...

1º plug your device into computer with usb debugging turned on
2º download this zip file: https://dl.google.com/dl/android/aosp/nakasi-jop40c-factory-6aabb391.tgz
3º open the wugfresh toolkit and select "Flash Stock + Unroot"

md5 check: 3689d95c00c45ae4ed53011083420730
THIS WILL WIPE ALL YOUR DATA
You probably want to flesh out the directions a bit more. Otherwise you are going to get a ton of questions about root, recovery, etc.
Do you have to be on stock 4 .1 .2 in order to do this?
will this wipe
and does it work stock not rooted

No, you can ne on a custom ROM.
OP, try to ne more clear...this is the stock facrtory image, and will delete all your data...on both partitions...everything...so backup all your important files...
Doesn't matter which ROM you nave installed, with this you will have 4.2 like out of the box!
Will I lose root if I flash this update? If so, is there a way to root 4.2?

No kidding.
Why not just update via the flash able ota file? Even if you're not rooted you can push it without losing data

Here are the preferred manual instructions: http://forum.xda-developers.com/showthread.php?t=1781250
Like the toolkit version of this method mentioned in the OP, this WIPES ALL YOUR DATA!!!!
Before starting, copy your titanium backup folder and licence, as well as various other data you want to keep to your PC.
... and sorry for bumping this in the development forum.

Please help.. Softbricked nexus 5

Guys please help. A friend has soft bricked his nexus 5. He wiped everything and can't flash any rom using twrp as there is no rom file in the phone. We don't know how to move the cyanogenmod rom.zip on the phone. Adb push through fastboot doesn't seem to work, and when we use the adb sideload from twrp, the rom is moved on phone but not automatically installed. What are we doing wrong? Appreciate your help.
P.s. We have access to fastboot and recovery obviously, but phone has no rom to boot and gets stuck in Google screen.

I have had problems with adb push using TWRP from a long time (before my nexus as well). Try using PhilzTouch and use the adb commands to sideload or push the zip. I'm sure it will work.
PhilzTouch: http://forum.xda-developers.com/showthread.php?t=2565174
Besides that, you could even try using a OTG cable to transfer the ROM in recovery

You're using the latest TWRP? Try Philz Touch recovery (Download link in previous posts). No idea what's the supersu error you're getting.
Wipe everything... Factory reset/cache/dalvic and then flash the rom zip (Although it has nothing to do with the error)...

What he points you to is to return to stock and start over again unlocking and flashing. recovery. It is recommended, but the problem could be just twrp as well as the very Rom you are installing. Try another recovery, another Rom. If not luck, then return to stock

You need to head to CWM recovery and then flash the rom.zip files...
Make sure you do a complete wipe before installing a new rom...

Dude, uninstall Rom Manager asap and never use it again, this has always been a terrible app and now its real terrible. Flash a custom recovery with Flashify (Play) or in fastboot (fastboot flash recovery namerecovery.img). Rom Manger is not a solution but a problem.
Sorry I don't understand!!! I have been using CWM for a very long time. I am just asking now that I am able to flash ROMs?

Rom Manager is unstable and buggy mate, if you have a working recovery then you`re lucky. Make sure to backup your current rom before you start flashing and backup your EFS folder and store it on the PC to be sure.

ART is still experimental atm and some apps are not optimized for it yet. That can cause bootloops in some cases. If that should happen restore the backup.

Dude, @gee2012 is just trying to help, and is right about RM. I used it back in the day on my Nexus One running CM before there were any alternatives. Over time, RM got wonkier and wonkier, ending up an app that is best avoided. I recommend that you take the advice of a recognized contributer, and don't continue to be loyal to a has-been app that could bork your system.
PS. Welcome to the forum!

Sure it is. I was talking about Rom Manager, please don`t ever use it again. Just for your sake mate.
Everything said about Rom Manager is 100% correct; it's long been known to brick some phones, due to the recovery files it lists not being compatible with a lot of devices (and poorly defined).
Great advice, @PhilipTD and @gee2012.

I don't need closed framework. So uninstalling and then switching ART should work? I'm scared it will go into a bootloop!

If it works for you, it works. Don`t have to flash it twice. Look above @PhilipTD`s post for Philz Touch CWM, which is even better and has more options than CWM. Its a zip and you can flash it in CWM.

You have to remember this phone is primarily done by a company that has no issues with rooting CM. Also I am quite sure finding the update zip will not be that much of a hassle. Also not sure if you know this but OnePlus One has put out a AOSP rom of there own. How man phone companies do you know that tells you how to root and puts out a rom.... :good:
