I noticed a couple of things upon restart. Do you guys know what it is and why it happens?
1) when im restarting my phone, i realize a quick message appears before it boots. That says "No memory card. phone photo, image, audio, video, album and moments features disabled" (i have one), is this normal? Some times it say no valid card too (I think its referring to SIM). Is this normal? Why does it do that?
2) Also one time when I rebooted, I noticed the widgets on my homescreen was a loaded weird. The spacing and text was not showing up properly, the size of widget looked compressed (weather and news app). What can cause this?

Here's a puzzle that's got me baffled!
Last night took some photos on my phone, plugged it into the laptop to copy them over as I've done nearly everynight for a year.
Went into the \\Storage\DCIM\100Media directory and couldnt see any files at all! Weird I thought.
So went into the Photo Album of the phone to see if they were there.
The photo album showed about 10-20 photos from AGES ago, all other photos had a circle with a line through it (like a stop sign).
Looked again in the directory, couldn't see any files, so reset the phone.
Went into the Album again, only this time it showed the default photos that comes with the phone, again nothing was showing in the 100Media directory.
Paniced at the thought of losing a load of photos but still continued looking.
Something caught my eye, the fact that even though it looked like all my photos had gone, the "number available" that appears in photo mode (the number being the one that shows how many photos you've got left to take) was still showing about 1500.
So took a few random snaps.
Sure enough, the number went down, but they were no where to be found.
Checked the size of Mb free on the storage card, made a note, then set the phone to record nothing for about 10mins.
Sure enough, the size available on the storage card had gone down but still no video was appearing in the 100Media directory.
I've reset the phone several times, any picture I take results in the "number available" going down, a few more videos of varying lengths does indeed show the space available as going down on the storage card but can not find anything anywhere!
Have manually been through every directory on the storage device, can't see anything anywhere.
Anyone had this before or suss out what the problem is and obviouslly where the hell my media is being saved?
Have checked the main device, but of course not there and wouldn't explain where the space available keeps going down.
I'm stumped!!
Install Total Commander and try to search files by date.
That's best I can suggest "out of the hat".
I've searched for this here and Google and can't find anything similiar. Two days ago I tried to use the camera to take a pic and the phone locked up. The rotating "working" ring icon just went on forever. Reset the phone, and tried again, same result. I changed the image size to the smallest and it would take a pic, and the preview looked fine, but the image itself when viewed was a scrambled mess of green and purple. Kept messing around with it and it and discovered that ONLY pictures are affected. Contact pics are fine, burst mode fine, panorama fine. Even video is no problem. The preview in album is even okay. Took a couple of samples off the phone and viewed them on my PC and they were either the same corrupt image, or wouldn't open and I got an error saying "Invalid JPEG file structure: two SOF markers". I'd figure I screwed something up, but I have changed absolutely nothing about the phone in weeks. Thanks for any help you can provide, I greatly appreciate it!
try another memory card, or back up all the files on your current card and format it.
chances are your memory card is corrupted.
I've tried saving the images to the phone memory and get the same results. Also why would it only affect the full picture jpegs, and not the contact pic jpeg? I'm stumped. I've got another memory card at home, I'll try that and see what happens. Thanks!
If youre getting the same issues on your phone memory, you might need to update/reflash your phone.

Of course try the simplest fix first (different memory card) and then try these other solutions.
Are you using Advance Config? If so, or even if not, check under the camera settings and see if "encode portrait" is enabled. That seems to lock up our devices similar to how you describe it.
I tried the spare card last night and no change. I then tried SDreamer's suggestion and instant fix. Thank you sir! I really really appreciate all y'alls help.
First of all, thanks to chui101 and all the other people here who put in the effort to get CyanogenMod to run on this phone. However, I'm having a bit of a problem with my phone slowing down severely and becoming unresponsive, usually at the lock screen. The specific issue is that the lock screen will be on screen, but not respond to button presses, or respond very slowly, and then it will do the "old CRT television turning off" effect much much slower than it's supposed to (think close to a minute), go blank for a while, the lock screen comes back up, another very slow screen turn off effect, and the whole process keeps repeating until I get tired of it and pull the battery to reboot the phone. This issue happens to me with both the 2012-11-21 and 2012-11-25 nightlies. To me, the behavior sort of seems like an out of memory issue, maybe some program is leaking memory or something.
Things that cause the freezing issue:
Power button presses too close together
2 cases of a call coming in while the phone is locked and playing music in Apollo
Things I've tried thus far:
Switch launcher from Trebuchet to NovaLauncher
Set Background Process Limit from "Standard Limit" to "At most 3 processes"
Set "Force GPU rendering"
Remove SD card
Out of those, pulling the SD card is the only one that seemed to help. The SD card I had installed was a 16GB SanDisk Class 2 card with ~3.7 GB of music along with less than 10MB of small text files, ring tones in .wav format, pictures, etc.
I put the 2GB card that originally came in the phone back in, this card currently has nothing but the text files and ringtones and a few pictures.
Is anyone else getting hit with this problem? What can I do to help? I don't know much about android ROM development, but I can handle collecting a LogCat from a case where the problem I'm describing is happening and know a reasonable amount about general Linux stuff, just not Android internals.

With the smaller SD card without all the music files installed, I'm getting less of the freezing problem, although I am not certain whether this is due to the music files not being there, or me not using the phone to play music. I guess the next part of this experiment is to go back to the card with the music files on it but not play any of them. I did grab a LogCat from one of the cases where the phone is 'freezing' and see a bunch of "Application Not Responding" messages, and the CPU usage stats that follow these messages indicate that most of the CPU time is being used by 'iowait,' for instance "100% TOTAL: 0.3% user + 6.9% kernel + 92% iowait". I've read some threads relating to other phones indicating that this could be some process poking around somewhere in storage. I'm thinking that running top from adb shell next time the phone ends up in this state might give some clue as to what's eating the CPU.
Also, I've hit the issue with email message bodies showing up blank that others on this phone, and on CM9 on the non-keyboard LG Mytouch (E739) have experienced.
Hope I'm not spamming with yet another status update, but I tried disabling live wallpapers (going instead to no wallpaper/black screen) last night, and I haven't had any of the freezing problems today. I'll try adding the music files back and see if the problems return then.

I put the 16GB card with ~4GB of music back in the phone, used it all day at work for listening to music/checking email/some web browsing without any crashes. (Still with live wallpaper disabled) So, I'm starting to think live wallpapers are causing my problem.

My M2 recently threw a wobbler and decided that it didn't like the SD card which wa sinstalled, so reported that there wasa problem with the card, and then promptly decided to format it.
The problem that I now have is that in the Alphabetical list of apps there are two icons/short cuts with a red circular sd card image in the top right hand corner of the icons and the icons simply show a rectangular blue image in perspective. These are now the first two in the list.

Pressing either returns a message saying, 'Application not found' and I cannot figure out how to remove them having tried everything I can think of, including reboots and resets etc.
Apaert from a factory reset is there any way to get rid of these useless icons ???

I also am having this problem. Except my phone recognises the sd card, but I can't transfer items to it, I have r/w permissions, the apps I have on there I can't use, but I can listen to my music and view pictures that I have on there, I have stock lollipop rom and rooted
I went to go play some games on my phone today. One of the games, when I launched it, gave an error that said "App isn't installed". I noticed that the game's home screen icon had been replaced by a grayscale version of itself, possibly something that Nova Launcher was doing. Then I noticed half my other games had the same problem - all greyscale icons, all "not installed". I have also noticed every couple of weeks or so that a "hole" will appear in my home screen icons - there used to be an icon there, but there isn't anymore, and I install so many apps that I can't remember what it was. I also notice that my "Free space" widget will randomly jump up by a GB or two. "Huh, I thought I only had 3GB remaining, now I have 4GB? What the heck?"
Remnants of the apps still remain in the obb/data folders, but the apps themselves are disappearing, and I don't know why. I do not, and have never, installed any apps on an external SD card, I wouldn't even know how. What on earth could cause my system to actually take an installed app, delete almost all its data, without ever asking me or telling me? What kind of system logger could I install to figure out why this is happening?
Example: Ski Safari AT, second row 5 across, has been deleted and had its icon turn grey. When I tap it, it says "App isn't available":

May be there is a virus or someone is making you fool by messing your mobile up.
Please make a factory reset but before that try to figure out what is going on?
Do you know how to make log file?
Google it.
Upload that log file here and devs will help you.
