wow... this came totally uncalled for. after it had worked perfectly since i got the Galaxy Tab, and i just more or less randomly opened the camera app to see the settings, i get this error message on camera start-up:
"Warning - Camera failed"
blablabla... fixed it
it was in fact my tampering with the Email.apk. i removed the original email.apk and email.odex, however only replaced email.apk but no *.odex... that screwed my camera
that will teach me tampering with something i have no clue about ...

Ohh so I randomly found this post... This is the exact same issue I was having!! Thank youuu sooo much!!! I had no idea deleting the email.apk would cause this issue... Luckily I hadn't deleted it but placed it into "removed apps" folder. You're awesome!

my camera is also not working,, i have installed few apps from market though factory reset solve this problem but i need those apps and whenever i download them my camera shows warning "CAMERA FAILED".

well my problem was the same as the op i froze\deleted my email and some how its connected to the camera so i just unfroze the email and rebooted and the camera worked again perfect hope that helps any
Just a note.
Default camera will stop working on <200 LCD Density on 2.3.3 (I'm using 180) it's quite surprising since this problem never occur in Froyo before.. Even in the same density
mine fails sometimes when i change my LCD density, changing it to a higher number fixes it. i am at 190 right now.

Hey guys, i have sum strange issue with camera. whenever i open the camera it shows me inverted images, means i see everything upside down.
I thought its because of faulty hardware so i given it in service center. They have changed the hardware and given it back to me. Everything was working ok but as soon as I rooted it, the same problem happened again. Now even after flashing a full stock sbf doesn't solve the issue.
Surprisingly the first time the problem happened also after rooting.
This is really irritating now. What I m thinking now, it may be due to a software bug or the camera is getting damaged upon rooting. I still cant figure out the exact reasons. I am posting it in developer's section so that more developers can see it and solve my problem.
I have the option to give it in service center and get the camera hardware replaced again, but from then onwards I will not be able to root and couldn't enjoy the full benefit of Android.
Guyz please help me, I love my defy.
The stock SBF that I have is JRDNEM_U3_3.4.3-36-1.7

Rooting has nothing to do with camera(apk) apps as far as I know. Or perhaps you could try clear data/cache in manage application -> camera..

Things that I have tried till now.
1. Flashed a full SEA 2.2.2 sbf
2. Cleared cache/data from camera app.
3. Tried MIUI/CM7
Maybe some one can give me the camera libs from stock 2.2.2 (and tell me if replacing that on MIUI rom will not have any adverse effect.)
I have the SAME issue.. But with me, it happens since I bought the phone.
It happened on 2.1, 2.2 and now with CM7.
I gotta say, I must have tried EVERYTHING excluding sending the phone away. I hate, hate, hate this bug, and nobody seems to know how to fix it.

and its got to be a software bug coz its happening even after changing the camera hardware. And so many people are suffering with this problem. Sadly no developer in the forum is willing to help even after I sent a PM to many developers.

this is easy fix. Guys went wild with bit rate just a bit. In /etc folder edit your media_profiles.xml
find the line starting: <Encoder profile quality ="high">
and edit bitRate value. I believe By default it is set to 10000000 just change it to 5000000 or 3000000 and set freameRate to 30
you'll be good to go. Dont forget to restart after making changes

Anyways, thanks for atleast taking interest.
My Defy has come to the point where 50% of the time I start the camera, it's upside down.
40% the camera will start OK, but when I try to take a picture, it becomes all laggy and non responsive, then freezes, next time, it's inverted.
And the other 10%, it will work.
Anyone, help, please.
those facing issue can try this.
open build.prop in text editor with rootexplorer.
remove the line "ro.media.capture.flip=horizontalandvertical" and reboot
now everytime you will see upside down image.
Now open build.prop again and add the same line where it was i.e. "ro.media.capture.flip=horizontalandvertical" and reboot.
The problem should be solved.
I did the same and the problem happens for me only 1 in 100 times.
*do it at ur own risk, i have just tried it and it worked for me, not sure about you.
Hey man, just did what you posted, and so far so good!
I took a bunch of pictures, kept entering and leaving the camera app, things that would normally make the camera inverted, and everything looks normal.
I'm trying not to get too excited about this, since I really didn't test that much, but if it at least reduces the amount of the occurrence by half, I'll be a happy man.

I'm on franco kernel r33 and stock rom. Sometimes the camera doesn't work, it says it "can't connect to device" or something like that. I don't know what's going on and I have to reboot to make it work. Here's the log. Thanks in advance.

Well I'm not gonna look through you logcat right now(might circle back to that, even if it may not contain anything relevant) but I've found that I can make this error message appear by having the camera used by another app(e.g. one that works like a flashlight) and then opening the camera.
So do you use any third party app that make use of the camera? It could be snapchat or Skype for example. If they fail to release the camera when you press the home button, they may be able to cause this problem. If you have such a third party app, try quitting it properly by use of the back button or maybe even force close it.
edit: I'm actually not sure that killing the app will fix the problem, so you may have to try not using it for a while to see if it makes the problem not appear. My bet is that it is caused by skype not releasing the camera properly, since that seems to be the only app accessing it prior to the error.
It can of course also be a hw problem or a low level(kernel) problem. And after looking through the log it doesnt look like the same error i get when the camera is simply occupied. Does it work in a third party camera app after you get the error?

Snapchat has been borking the Camera on my phone and a Galaxy S3 with Beanstalk.
Snapchat or other 3rd party camera app probably.

Well that rules out problems with the camera app itself, so it is either because the camera is not being released by Skype or something similar, or it is a problem with the phone or os.
Try to avoid using Skype or any other third party app that uses the camera, after a reboot and see if the problem still appear. Maybe even try safe mode (also without running those apps).
If that doesn't fix it, you should do a factory reset.
It seems to be Instagram causing the issue. Is there any way to fix it?
So... I had no problems with my(second) idol 3, all was well and dandy. But recently the colours on the screen became more saturated, almost like an amoled display, followed by a significant decrease in performance. Nothing is changed, no root no nothing. Totally stock, except for the launcher. That would be Nova launcher and I used it since the day I got the phone with no problems. What the hell happened and has anyone else experienced this?
Random restart happened today and all my wifi passwords are lost.

i asked the gallery 'cause on my idol mostly (but not the only one) the gallery app that distorts color badly... have you tried a CM rom? in my case there is no color distortion on CM at all so the stock rom obviously has the wrong color balance settings

Yeah, I know about the gallery app too. Can't really install a CM rom due to warranty, it's only two monts old. I'm sure that the problem can be solved with a full wipe but that is not the answer I'm looking for.

I doubt that a wipe can solve errors like this. I did it a few times, restored firmware with Mobile Q, color distortions on stock ROM never disappeared. Best experience was with Color Balance app from Play Store. It's just too expensive for my budget.
I have tried wiping cash but to no avail. The performance has slightly improved during the day but has returned back to garbage now. This is so annoying. Is there a way to check the "settings" for this without root? Maybe something in build.prop or such?

without root you can't do any really useful. if you don't want to install cm, check this app (earlier i made a mistake with its name):

If it can stop color distortion, at least you'll know that the phone hardware itself is correct, just the stock rom sucks.

just read the new symptoms, at first i'd make a full reinstall with mobile upgrade q, if it doesn't solve anything, i'd go to a cm rom.
The problem seems to have fixed itself. I don't know what happened. All it's buttery smoothnes has returned after an accidental tap on the gallery app. The saturation has returned to normal too. I'll check out the app link for good measure. Thanks for all the help. How does one close a thread?
Guess I spoke too soon. The phone just did something of a restart. The whole boot animation happened but none of my open apps(gmail and chrome) have closed once it booted. Very strange.

that's normal, opened apps don't disappear from 'recent apps' after a reboot on lollipop.
however a reboot just from nowhere isn't normal.

I did not know that. Guess I will have to do the mobile upgrade q procedure. I'll give it a few days before I do that though.
Welp. I've given it some days and the problem has not returned. Also, a fun thing i didn't mention. This replacement idol 3 is not exactly the same as the previous one. For example the camera lenses had a green tint on the first one, but on this one they're blue. And the logo niw sits flush with the surface instead of being pushed more inside.
Here's a weird one. Just got my 8 yesterday and noticed today it won't play any type of video...talking YouTube, external, even videos taken with the camera! Any help is appreciated. I was to get sound on one video but black screen. Almost like it's missing video drivers or something.
EDIT: A hard reboot fixed the issue...like the old blackberry days!
This is actually a known bug apparently. Except some people have to reboot more than just once due to the issue coming back. Hopefully a small update will fix the issue.

Also experiencing this. Over time, videos won't play via any app until I manually reboot the phone. Kinda annoying.

Factory reset is the solution
I also have this issue. Factory reset only works for a few days than it comes back. I hope they patch quickly.

Yep this happens to me too!
YouTube playback goes black but audio still plays sometimes.
Browsing on reddit, any post with a gfycat that plays as an mp4 or related format doesn't load at all.
And as others say, rebooting fixes the bug.
Next time it happens I'll run a logcat or something as I'm rooted and I'll see if there are any errors and post logs. Issue happened before and after root so I'm sure root isn't the issue.
UPDATE: So it happened again and I captured a logcat while trying to play some videos. The last 4 lines are of interest. Logcat added as screenshot

So, I've been trying to change my lock screen image (as well as the app screen and such) for my HTC U11 but it will only let me use the default selections and not my own pictures...it was working a few weeks ago but it's really bugging me!
Every time I try to select my chosen images it says "Error: Could not load media" - there's nothing wrong with the image files and I've tried many different ones...
I tried going to the "edit current theme" and such but it just won't work! Even HTC customer support couldn't help me :c

Same here.. I deleted the Sense home data, restarted the phone - nothing worked.
Perhaps it is a problem on HTC's side..

My old HTC 10 also has this issue.
However, a friend of mine with HTC desire 530 has no problems haha. Really baffling.

Doesn't work.
Also when I select a pic from the G photos itself, to create a theme for an example - it says the same - could not load media. I deleted also the data for the GPhotos - still doesnt work. I run out of ideas.
Can your gallery see the photos OK? It might be a permission thing. Try deleting gphoto cache and data, reopen the app and see if it asks for permission. The screen saver is linked to the gphoto app so the problem must lie there.

Gallery sees and edits the photos ok. Deleted and reopened everything possible. Nopermissions asked. No luck.
From this point I'll admit I'm stabbing a fish in a barrel. Do you have encrypted storage?.
Out of curiosity, are you trying to change from personalize or display settings?
Try edit current theme under personalize.
The error occurs when I select edit current theme using the Themes app and then try to load a picture from the device

I have the absolute same issue, I uninstalled the whole sense home app (therefore my whole homescreen messed up and all my icons) and it reinstalled itself and I restarted my phone and it worked, however it just updated again and now does the same thing
