Embarrased to say that I screwed up my phone pretty badly.
Series of events so far:
- Flashed TWRP by accident, thereby wiping everything
- Now unable to boot to the OS because I wiped the OS inadvertently
- Managed to access TWRP via fastboot
- Factory wiped and attempted to flash an oxygenOS zip through TWRP - didn't work
- Attempted to flash a new rom from fastboot using this tool - didn't work
- After using the tool above, the phone bootloops on the bootloader warning
- I am still able to access TWRP
EDIT: Solved. The only way to fix the phone was to use the unbrick tool for OP7.

Recently I updated my Nexus 5 from Lollipop to Marshmallow 6.0.1 (MMB29Q) - latest build.
everything stock works fine but, once I flash a customer recovery, in my case TWRP it stops working...I get either a startup loop or TWRP gets erased by stock recovery.
I tried various steps ...
flashed TWRP 2.8.7.1 - rebooted to boot loader was able to get into TWRP but, once I reboot system, everything works but TWRP is gone/erased and replaced by stock recovery?
Flash TWRP 2.8.7.1, then flash latest TWRP 3.0, reboot to boot loader and can get into TWRP but once i reboot into system, I am stuck on the initial boot loop.
I have to start from scratch of flashing system and boot etc to get past the loop and it still replaces TWRP with stock recovery.
I can always get into Fastboot but when I go to recovery it seems its being replaced by stock recovery.
anyone experiencing this issue? thanks for your help.
I guess I am doing something stupid but, not sure what...and thanks for your help
If you give more detail on the steps you are taking it will be easier for someone to help. Also, have you tried doing a factory reset when it bootloops?

yesterday, I downloaded latest build from android site...extracted all files....booted into bootloader and flashed all files..
rebooted - Marshmallow works perfectly...offcourse it not rooted yet or its locedback due to this flash
Scenario:1
So downloaded TWRP 2.8.7.1 and flashed it as recovery, no errors nothing...i rebooted to bootloader and went to recovery to confirm TWRP is there
I rebooted the system - TWRP gives option to root which I said yes so its rooted. I reboot system, this goes into bootloop - I wait for say 5-10 min in this loop mode and I see that adb can recognize the device but nothing else....its stuck in loop
when I force shut down by holding power button for few sec - and go back bootloader/recovery - TWRP is still there
***note*** I did factory wipe in TWRP but that did not fix above condition.
Scenario2:
exact same steps but after I flashed twrp2.8.7.1, I rebooted bootloader and flashed twrp 3.0 as well confirmed twrp 3.0 is flashed and rebooted system - phone start normally no issues - when I restart to bootloader - no TWRP exists, its replaced by stock recovery
Scenario 2, I tried couple of other ways and still the same thing happens - twrp 3.0 get erased.
not sure what I am doing wrong but TWRP doesn't stick
since this is my active device - i flash system.img and boot.img to get the phone to working condition

That did the trick.....thank you very much. I was using older version of supersu thinking it was latest.....I downloaded V2.65 and installed it and that fixed all my troubles..

I have unlocked the boot loader on oneplus one long back. I tried to upgrade my rom to Resurrection Remix 7.1.1
I successfully upgraded the ROM and also flashed SuperSu in it. I wanted to flash the Opengaps Aroma 7.1 and that is where I ran into issue because of my stupidity.
I found from GitHub that there were some issues with twrp versions to run the aroma installer thus I tried multiple TWRP versions via (Google play - official TWRP recovery app) by downloading them and flashing it to recovery.
I have taken the full nandroid backup but unfortunately didn't keep it on external devices. So it was in the phone itself.
Now the issue -
When I tried flashing TWRP 3.0.2.0 from existing 2.8.4.1 I tried to run the Aroma but it didn't work. Then I tried to flash the recovery with TWRP 2.8.6.1 but Aroma failed again.
However, on the GitHub there was a info stating that known working TWRP recovery on Oneplus one is of TWRP 2.8.4.1
I tried to flash that into recovery and flash the Aroma, but it didn't boot into recovery with that image. Later I restarted the phone, and again flashed the latest recovery version 3.0.2.0
From here I ran into issue. When tried again to flash the Aroma, it didn't work. So I booted recovery again and in that I selected the recovery version 2.8.4.0 for both recovery and boot partition.
After that, phone is neither booting the system nor into recovery. I don't want to loose my data (which is in phone backup) and neither want to try Aroma again. (Even if all the 900mb data needs to be deleted.).
Can the XDA members confirm can I reinstall recovery working image (2.8.4.1) once again through the adb flash recovery <image>. And boot into recovery mode so that I can delete the boot partition once again and reinstall the ROM and data ?
I tried doing that once but one of the laptops which I had access didn't recognize the adb devices. May be the driver was not working for Win10 OS. However, on my personal laptop earlier I was able to run adb commands through CMD prompt. Currently, I don't have personal laptop and may get it by next week.
Can anybody please share me the links of working android driver so that I can install them on current laptop and then try adb commands from SDK to connect my phone.
Any suggestion from members would be of great help.
Note: Please don't suggest the methods to factory reset or lock bootloader to factory settings. As I don't want to loose all my contacts and data.
If there is a way to get just recovery working on the phone, it would be of great help. So that I can wipe other partitions and rebuild it again.

No surprise i screwed something up.
If I run the msm tool boots up fine. If I flash twrp 3.2.2-0 I get corrupt msg when booting.
Tried the fastboot room, it runs through fine but when rebooting I get the blue light at the top and have to use the msm tool.
I used msm oreo yesterday then updated to pie (which I don't want) yesterday then msm back to stock Oreo to see if it would fix the issue, no same thing.
This started when I tried to flash lineage Oreo and blue spark twrp couple weeks ago which failed.
So just want to be able to be back on Oreo and able to flash twrp again.
Twrp is good until I try and boot up so I think I'll try and flash stock on both slots while in twrp then twrp again, maybe that'll fix the problem.
Have you tried using the downgrade package?

boot into the OOS (Pie I would assume)as normal, then go to the OTA screen within setup. Select the local upgrade and point to the downgrade package that is mentioned above.
This will wipe the full phone (user data as well), please ensure you take a backup if there is something important on the phone.
Once you are on Oreo - settings ->check if the bootloader is unlocked -> copy the latest TWRP zip to the phone and keep the img on the pc-> reboot to fastboot -> run the fastboot command to flash twrp "fastboot boot twrp-x.x.img" -> you will reboot into TWRP -> from within TWRP flash the TWRP zip and that should allow you to have TWRP installed.

The first time i flashed LineageOS 16 microg edition it booted but since i tried to restore stock ROM backup with TWRP it all went to hell.
Device immediately bricked, only booting to fastboot.
Had to use msm tool to unbrick. ( the first of so many times.... )
Then updated from settings to latest OOS 9.0.9.
unnlocked bootloader
and flashed again.
NO OS INSTALLED.
phone boots only in fastboot.
can't boot any recovery from fastboot. gives error.
if reboot to recovery blue led and black screen is all that i see.
only fix is msm.
From that point onwards i tried EVERY guide I could find (and recognize as valid of interest) here on xda, youtube, google.
Multiple times per guide.

Tried also official lineage, SyberiaOS. Different previous version of the two (since I read in the lineage official thread others have this issue)
Basically at the point after flashing any of these roms results immediately in TWRP warning "no os is installed" at reboot attempt with or without re-flashing twrp after.
Then if rebooted just hangs on blue led. can't boot anything.
Meanwhile flashing OTAs + twrp results in bootable system.
Other things that i tried:
before unlocking BL update to latest OTA_34 -> 9.0.9, reboot, install again.

What some guides miss out is that you have to install everything twice. Once on slot A and once on slot B and after each install, verify what slot you are on before reboot and then check what slot you are on after reboot. I ran into issues flashing Havoc OS onto my phone too and figured it out in the end after so many times that I was in fact installing stuff twice on the same slot because it never switched over. I'll run you through the order I did things so hopefully you can follow.
-Use MSM Tool to revert everything back to stock and once the phone is booted, update to 9.0.9 either by downloading the update through the phone or by local upgrade if you have the file to hand. (remember to only have the OOS file present as any other OS in zip form crashes the local updater). You don't need to do this if you are already rocking stock Oxygen OS.
-Once the phone has restarted after the upgrade, let the post update procedure finish before continuing.
-Enter developer mode and enable OEM unlocking, Advanced Restart and USB Debugging. From here, reboot to bootloader.
-Plug your phone into the PC now and assuming you have fastboot setup ready on your PC, type 'fastboot oem unlock' and use the volume keys to select the option to unlock and press the power button to select.
-After the phone has completed it's wipe cycle, re-enable Advanced Restart and USB Debugging. The OEM unlock option should be greyed out as it's already been enabled. Make sure you don't add a pin code or password just yet as you will enable encryption which will make the next steps harder than it should be.
-Boot back to bootloader and type 'fastboot boot twrp.img' (or whatever you named it. Make sure that the image file is in the same folder you initiated cmd or powershell)
-Once booted into TWRP, flash blu_spark TWRP as that supports encryption apparently and is more stable than any other. To make sure that the script executed on both slots, hit reboot and check what slot you are on and tap the button for the other slot. Now reboot back to recovery, check you are on the other slot and flash it again.
-Now flash your 9.0.9 OOS zip followed by TWRP once on slot A and once on slot B while making sure you are on the other slot when doing it a second time. Remember that you have to re-flash TWRP after any flashable file that alters the boot partition before you reboot or you will undo all your work. Just make it habit that you re-flash it every time and you should be good.
-Once OOS and TWRP have been flashed on BOTH slots, reboot back to recovery and do the whole process again with your custom ROM (Remembering to flash TWRP before you reboot and checking which slot you are one before and after each flash).
-Once that has completed, reboot back to recovery and flash Magisk on each slot and GApps or MicroG if your ROM supports it. You don't need to re-flash TWRP after this but I do anyway to keep the habit lol.
-Entirely up to you, but before you reboot to your fresh new OS, you can wipe internal storage ONLY if you want all traces of your previous OS gone, but this will also remove personal files etc. I generally just head into TWRP file manager and delete the Android folder and keep the rest.
If you run into issues like crashes on your new OS, reboot to recovery and do a factory reset, but you will also need to re-flash Magisk again. GApps installs itself to system, so you won't need to re-flash that. MicroG does not and will need setting up again. One thing that took me a while to realise is that if you try rebooting while your USB is plugged into your PC, you will sometimes enter download mode, so only keep it plugged in when you need it for MSM Tool or to transfer files across.
I'm pretty sure I put everything down the way I did it and didn't miss anything, but it is late and I am half asleep. Will check back in the morning and double check with a fresh brain.

i tried all the mentioned steps, nothing worked for me. Even i tried all the guides, here on xda and on youtube i am not able to boot my 1+6 it is getting booted to fastboot mode, after installing custom rom. But if i flash only OOS then it boots back properly.
But in case of a custom rom it boots back to fastboot mode, and then in the end msm tool is the last resort to lock bootloader and make it stock.

If you are stuck on fastboot try
fastboot set_active other
then try rebooting recovery if fails try
fastboot boot pc-stored-twrp
I found changing the active slot via fastboot may unbrick it without msm. It all depends on what steps you actually take during installation.
For installing LOS try not flashing twrp after installation. Los comes with its own recovery. Anyway if it says no OS installed you HAVE to fix recovery bootloop.

I wanted to update my oneplus 8 from 11.0.4.4 to newest version. OTA was not working idk why. I had magisk in place and I patched magisk to second partition or something like that [I dont remember how it was called] - I'm sure this is the reason of the problems. I turned off devices and wanted to boot into recovery, but suddenly it boot into Qualcomm crash dump mode with message: Attempted to kill init! exitcode: 0x00007f00 do_exit - that's all. I can't boot the system, I can;t boot into recover, I flashed patched boot, I flashed recovery - none of these works. Fastboot is present and recognized by PC.
First question can I do some backup through fastboot? To be honest I was not prepared for such problems. APKs are not even such a problem I care more about photos, documents and other data.
Can I flash some rom and restore phone back to life without wiping my personal data?
Please can you help me? I tried with some tutorials, but no success...
Maybe an idea would be to load TWRP recovery and then do backup, but how to boot into recovery when even after flashign recovery or booting via fastboot it enters in qualcoom crash dump mode. Something must be corrupted, but is it possible to fix some partition just in order to flash recovery to do backup of the files?
Okay I managed to get back to normal - since like I mentioned i patched the magisk into second slot, then probably this corrupted something. What has helped me is this: fastboot --set-active=b
It was not working via powershell I had to launch CMD and then it worked I switched slot to "b" and phone booted normally.
Hi, I'm facing the same problem but probably not for the same cause.
I'm right now on PE12 (with PE recovery) and wanna install TWRP recovery to then move on another A12 based ROM (Evox).
Basically my goal is to replace PE recovery with TWRP. So when trying to install twrp after downloading the ZIP file onto the phone, when I fastboot the img file, it sends me to the crash Dump mode.
How can I fix this? please note I have no issue booting to the main OS.
