Are you using any rom that has focus issues present?

Have you tried cleaning the lens? When ever my photos come out like that, i just give the lens a good wipe down and the pictures come out a lot better
Im using cleanROM which as far as I know has no issues with the focus.
I have cleaned both the outside and inside cover and still the same

So it sounds like you took the phone apart, in order to clean the inside of the cover? have you ever taken it apart before? Maybe you got a smudge on the tiny lens itself, and not just the cover on the casing?
Are you sure all settings are default? Default is ISO and White Balance on "Auto", and Image Adjustments, all (Exposure, Contrast, Saturation) should be zero.
Does lowering the exposure help any?
This looks like an exposure problem, or a smudge on the lens or cover, rather than a focus problem. The focus actually seems ok.
I have a problem with my camera. All the images have a smudged black spot at the lower left side.
The lense is crystal clear with no scratches or anything.
I really want help with this issue. Please help me. Is my phone broke?
You have something on your lens, I think you have to use warranty
If it's on the lens, can't I clean it myself?
I don't see anything and I've tried tapping the phone on my lap thinking it would jerk that "something" off..
Please help me!
The lens is inside the phone. You can't get to it without voiding your warranty.

It's not on the glass probably but on the camera lens itself, use your warranty because you can't fix it yourself
Over the past few days my pictures have started looking like they were all taken in a 1920s jazz club.
I took a photo on the 9th of my living room setup that came out perfectly clear. The next day I noticed that pictures were coming out a little fuzzy, having a hard time focusing, and there appeared to be a slight film on the lens cover. The picture I post below is from today and the haze seems to cover almost the entire lens cover.
I removed the film the day I purchased the phone and haven't made any changes to my daily routine that may have brought this about.
Does anyone have any ideas on how to fix this? Hair dryer on medium for a minute or so? Should I RMA? Anyone else having this problem?
Is there any kind of condensation under the lens?

I can't tell what it is but there is definitely something under the lens. However, I checked the water indicator in the sim tray and it seems to still be white.
Edit: I know you can't really tell anything from those photos. In the first one, even though it is out of focus, you can tell there is definitely something on the lens.
I can't tell much by the photos, but the camera and finger sensor ship with protective vinyl covers. The one on my finger sensor was particularly hard to remove because it was cut and aligned so perfectly, there was no lip to grab. I had to use tweezers to remove it.

Yeah, I distinctly remember the fingerprint protector being a pain to remove. That's why I know there isn't a protector on the lens...

Ah, I completely missed that in your OP. My S6 takes the best photos of any phone I've ever owned, I have zero complaints about that. Your photo is definitely hazy. I have never taken a photo like that. If you are still in your return period, you should exchange it.

There is definitely something wrong there. Return the device while you can.
Can't really tell from the pics, but you might have condensation in the inner elements of the compound lens.

Thanks, gents. Looks like I'll be filing a warranty replacement on Monday.
The only thing I can think is that when I went for a run with it in an armband the morning it started and somehow sweat got in there. I guess they weren't kidding when they removed the water proof rating from the s5.
I attached a couple pictures from before it started. This camera seriously is phenomenal.

Any ideas? Can it be cleaned or should it be replaced?
You did the obvious and removed the plastic cover on the lens when you got the phone right?
No.. i removed it just yesterday
Hi.
I don't know if the removal of the plastic fixed your problem ... but i have the same black spot on my rearfacing camera too without having any kind of plastic on the camera module ... knox is tripped ... no warranty for me so i want to ask you guys, if the problem is the camera module, or the camera lens glass since i would like to order the right spare part.
Thanks!
Has anyone figured this out yet? I have seen other sites suggest "dropping the phone" lol ummmmm no.
I have the outer lens off now, and I have cleaned the top and the sides of the lens very gently maybe one time not so gentle and still the spot remains. I see if I out pressure where the spot is it fades lightly.

Hey, well i had the same black spot on the camera and actually dropping the phone solved the problem ... it was an accident (it fell from about 3 ft) and i had a case on my s6 but it fixed the whole thing ... it may not be the brightest idea, but it will most likely solve your issue.... ( Do it on your own risk tho!)
I just replaced the back glass and noticed that the auto focus does not longer work. The camera just won't focus at all.
I tried to cover the 3 dots at the back but it didn't fix the problem. Still very blurry picture at any distance.
What went wrong?
Ps: does the camera move in order to focus? Because I think there is a tiny piece of glass between the camera and the plastic edge behind the replaced back glass. I tried to get it out before installing the glass but instead of removing it I somehow managed to dig it deeper. My guess is it could block the camera from moving.

If the glass was broken and autofocus isn't working, chances are there is glass inside the camera. Parts of the camera do have to move in order to autofocus correctly. I would recommend taking off the glass, turning the phone upside down(so the camera is facing the ground), and hitting the phone hard( but not breakig it obviously.) This may even work without taking the glass off, you'll just have to try it. This would hopefully knock some of those pieces of glass free.

BTW, this could also be caused if the glass is dirty on the side with the camera.

Do you have a link for the rear glass you purchased? I broke mine about 2 months ago and every replacement I've bought is not oem, it has adhesive covering most of the back instead of just on the edges, also there's a hole cut out for one of the sensors when it shouldn't be.
As for your issue, they "windows" for the camera or sensors might be dirty, or even a spec of dust throwing off the auto focus.

Lol I wouldn't recommend banging it. Would probably have better success using tweezers and compressed air to remove whatever is there.

Banging it would probably be the best option. If there's tiny shards of glass in your camera your not just going to get it all out with tweezers. Compressed air could work, but if you don't have compressed air banging it (aka lightly tapping the top of the screen to get the glass out) is probably your best bet.
Did OP ever resolve this issue? Dealing with the same thing now...
Same issue for me. Any idea how to make focus works again? I have dirty lens message from time to time... Is focus in lens or laser near the lens?
I'm having the same issue.
