I don't know why it does it but I think a factory reset fixes it. There are quite a few threads about it.

ok, it seem like i have to do a factory reset. i think i will wait til i get my new nexus 6p.
btw, since i hae backup app on titanium backup and store inside the phone. Does anyone know factory reset will wipe the phone data?
because my nexus 5 showing 14gb instead of 32gb. I heard a factory reset will help the problem. I just dont know which factory reset (recovery or in setting) to do.
but if i use twrp wiping cache, data and dalvik (leaving system and internal storage alone).... will it wipe my lollipop os too? will it wipe my storage too?
what is the different between the factory reset under setting and the factory reset under twrp?

Every time I do it, wipes my imei and I have to flash the factory image...I'm actually looking for a twrp flashable cwm file if that makes any sense

Haven't seen a zipped cwm for the N5 yet. Why not just flash the .img in fastboot?

It took several minutes also for me. Really awkward. With my previouse phone, LG p880, it was a matter of few seconds.
Probably the best part of 10 minutes to wipe cache/dalvik for me today!
Its a know issue with the N5. I believe its hardware related. TWRP devs will figure it out.

Yes, it is. And there is a workaround for that issue: Go to the TWRP settings and activate "format rm - rf", so your wipe takes normal time as known.
You can find more about that in the TWRP Nexus5 main thread here .

For me personally, wiping the cache entirely deletes my internal storage so I would avoid it at all costs until there is an update..

Such thing never happened to me. Formatting cache takes like 3 minutes for me, but it doesn't do any awkward. Unless you reboot while formatting ofc.
I'm avoiding wiping cache in TWRP at the moment...
first time i did that it borked something and i couldnt even switch airplane mode off or open play store when i got wifi on etc...wasnt that nice

Which factory image do you use? I just ran into this issue and it's driving me nuts.
I wish I would have found this thread sooner. I just wiped my internal storage and IMEI doing this. I have a backup of my IMEI, but restoring it with TWRP doesn't seem to do anything.
Will restoring the factory image fix this, or do I need to do that and restore this backup?
SO if you change it to rm-rf it doesn't bork your cell connection? I know it'll wipe faster but still leary on cell connection. Or is it that this only happens if you make the mistake of rebooting instead of waiting???? I'm wanting to jump on SlimKat and coming from an Evo LTE the procedure to clean wipe I'm familiar with is wiping system/data/cache/davlik cache so I've been holding off worried about the cache part. I've been using 2.6.3.2

I was having some major issues with my nexus 5 today, so I decided to flash completely back to stock using the google factory images in fastboot. After I restarted my phone, I noticed that my phone is only displaying 16 gigs of memory available when my model is the 32 gig model. Is there any way to restore the phone back to being a 32 gig? Or a way to reformat it? Thanks!
You're going to have to go into the stock recovery and clear cache and do a factory reset

Wiping cache should fix it. Factory reset wipes cache anyhow.

Try following this guide posted in the Nexus 4 forums, posted by @El Daddy with helpful shortcut provided by @dellenrules in this post. Basically clear cache/factory reset using the stock recovery image, not a custom one. Of course, use the recovery image for the Nexus 5 instead of the Nexus 4.
Okay, I will try it and let you all know what happens! Thanks for the help! Luckily I flashed everything back to stock including recovery, so no extra steps hopefully!

I am probably going to have to return my N5 due to issues with the GPS not working all the time. My phone is not rooted. What then is the safest and easiest way to backup everything? And will a factory reset afterwards be enough to protect my data from being accessed while the phone is in for repair? It is an unroofed phone.

Helium backup (available in Play) is good for backing up user apps and its data, can be used on rooted and not rooted devices.
I would just copy your SD card using GDriveSync to Google Drive. It can sync which folders you choose.

If you do it through the Settings app, I think so, yes.
But don't quote me on it, haven't tried it personally.

Settings app uses recovery to factory reset. And stock recovery wipes everything in /data

Factory reset wipes all his files, apps, and data. You will have to reinstall all of them. Any photos he had on the phone are gone There is no way to recover them

If he had back up set, Google will restore some stuff when starting phone up and entering Google account.

Flash stock recovery, wipe all data / factory reset. Just happened to me too.

there have been quite a lot of threads facing the same issue. to resolve it do factory data reset (which wipes everything on the phone storage)

You need to factory reset your phone and it will solve this issue
