Which version of CWM is installed? You can mount USB storage in the recovery and simply copy a rom to your sdcrad as it will be handled as usb mass storage. In case you can't mount storage --> we'll get it working another way
WHY you use adb to copy files? Mass storage is even more easy + safe

Problem with ClockworkMod Recovery v3.0.2.4 (USB Not Recognnized)

I wiped and cleared everything (cache, SD Card...) from the settings in order to install the new Ice Cream rom which I got working but then wanted to clean everything up. So obviously after doing the formats, it also wiped the zip (icecream). I can access clock work recovery, but it doesn't recognize my USB at all, same machine and cable is fine. I am trying to do it as a USB Mass Storage device but still it cannot recognize it. Any help would me much appreciated.

I'm not sure if you mean SD storage and not USB but you should have never formatted your SD storage.
In clockworkmod recovery there is an option to mount the SD storage. Make sure its mounted and then try activating the USB storage again. This should give you the ability to add a ROM zip file on the SD storage so you can flash it.
Thanks for the reply. Basically I am in clockwork recovery mode, I mount the SDCARD, then I go and select the option "mount USB storage". Sometimes I find that it finds Android 1.0 and I see the I: come up but then goes away quite quickly. But most of the time it comes up as unrecognized.
Now I have 2 cables, both which charge the phone fine. I have used one on a BlackBerry and remember it does the job of connecting as a data connection fine. Also have tried my XP laptop, same result.
My feeling is that it may possibly be the actual USB port on the phone that is the problem but I am unsure in this. It never failed to connect when the phone was working properly (in an actual OS).
You should update your CWM... 5.0.2.0 is the latest version.
But anyway, you don't need to mount SD card then mount USB storage. Only need to do the latest. Once you do this, unplug and replug the phone, the drive should pop up.

When I go into clock work recovery it says USB device Unrecognized. Problem is I don't have anything on the SD I have the ROM on my computer. Whether I go to the mount USB storage menu or not. Unsure what to do?

I imagine you don't have a working adb setup on your computer to update CWM, right?
Boot into CWM with phone plugged in. Go to mount USB storage. If your "SD" isn't recognized, still on that screen, unplug and replug phone. It should get detected then.
Tried this several times keep getting Unknown Device. I installed the SDK with Google Drivers. Unsure if that is the ADB set up.

Yes that's the adb setup.
You might need to manually install the drivers for your phone, although this shouldn't impact on the SD mounting capabilities. Just to make sure, I would get a working adb setup and use fastboot to update your Clockworkmod Recovery to 5.0.2.0.
You can follow this to know how to use fastboot
http://wiki.cyanogenmod.com/wiki/Fastboot#Flash_Custom_Recovery.img
If you have trouble getting your phone to be detected and the drivers installed, I suggest you use PDAnet drivers instead (if you're under Windows). Some phones for some reason need .inf driver hacking to get detected.

I will give this a go many thanks.

Just to let you know, I am trying to access USB Storage from clockwork recovery since I don't have any rom on here at all.
This may sound weird but I held the USB in the phone port in an angle, luckily the removable drive came up. I got the the ROM on the SD it has installed and loading. I'll see if its an issue with the USB on the phone later on but as long as it got back up and running (Ice Cream) for now.
Anyway thanks guys much appreciated. I'll be a lot more careful next time before I go around wiping everything especially the SD LOL.

I gathered this much from your posts.
Edit: Glad you got it working. Make sure you get CWM updated whenever you can if you haven't done so, this could lead to problems down the road to flash different zips.

Stuck with ICS in CWM and no USB.

Upon trying to update my TF101 device to Jelly Bean using this guide (wiki.cyanogenmod.org/index.php?title=Install_CM_for_tf101), at the 6th step of Installing CyanogenMod from recovery (i.e., Select install zip from sdcard) the tab says it's unable to mount the SD card.
The exact error is: E: unable to mount /sdcard/
I have CWM v6.0.1.3 installed.
Now, I'm stuck with a TF101 that isn't detectable via USB (or APX for that matter) running ClockWorkMod without root. What do I do? I'd like to upgrade to JellyBean but if that's an issue, I do not mind sticking with ICS.
Bear in mind that I'm completely new at this and I might need a step-by-step guide.
P.S. - I can't post links. Sorry.
If you boot to ICS and enable USB Debugging in Settings - Developer Options, is your device visible from the PC?
If yes, then I would try using PERI to root and install CWM.
If not, then I would try installing the Hybrid Touch 6.0.1.2 of this recovery: http://forum.xda-developers.com/showthread.php?t=1855686 Download the file in ICS, place it in the /sdcard/, flash from Recovery and see if it will let you. If you can see your microSD from recovery put it there. Then reboot and you should have a working recovery.
I have seen quite a few people try to use that wiki 6.0.1.3 and have no luck with mounting the /sdcard/

No such luck. Still not able to mount the /sdcard/ and it doesn't seem to recognise the microSD as far as I can see.
Might be a bad USB cable. I have seen a few users (myself included) where my cable would charge, and be recognized by the PC but it would not do APX or ADB. New cable fixed it for me, and most others.

i just want to install a room but i cant.
I Install the recover cwm-6.0.4.5.img go to mount and storage but i cant because i got this message : usb Mass storage device leaving this menú unmounts your sd card from your pc.
i have the last driver on my pc and also htc sync. im try with openrecovery-twrp-2.6.3.0-evita without result.
I dont have backup only the room 4.2 to install.
hope u help me. i just want to my sd reconice my pc.
sorry for my bad english .
The message you get in CWM recovery (usb mass storage device, leaving this menu unmounts your sd card from your PC) looks right to me. That means it's currently mounted and should show up as a removable drive on the PC. Is it not showing up?
What happens when you enter the mount menu in TWRP and select mount usb storage?
Are you using the HTC usb cable? Are you using a usb 2.0 port? Have you tried different cables/ports? Have you tried with a different PC?

I don't think a different recovery will help, I don't think that's the problem. Does your device show up at all when connected to the PC? Did it ever connect properly? If it connected properly in the past what did you do to the phone before it stopped connecting?

Thank u for the help bro, im do this from this case:
Go to the folder you installed the SDK tools to If your on Windows 7 x64 and installed it using the default paths it will be located here C:\Program Files (x86)\Android\android-sdk\platform-tools when at that folder move the recovery image file in to that folder then hold shift and right click in the folder select open command window here.
Now get your phone back into the bootloader select fastboot and connect it to your pc when your phones bootloader says fastboot usb its connected and ready to fix.
type fastboot flash recovery openrecovery-twrp-2.1.8.1-evita.img
that will update your recovery to TWRP which is much better than CWM. From there you can select reboot into recovery from the bootloader TWRP will load up you can Wipe /system /cache then mount your phone to your PC and upload any ROM you want. I recommend AOKP or CleanROM and flash away.
my problema was that my cpu not recongine my usb. now i can format and install room
thank u

You cannot mount storage on Nexus devices. You were right with pushing a rom with adb, but it seems your drivers aren't installed correctly. Fix the driver problem and you'll be able to push with adb.

[Q] Wiped everything - Unable to access internal storage from Computer

Hello
I have a big problem. I wiped my phone(HTC One m7), because since some updates, there was big noise problem during phone calls.
Now I have the problem, that I'm unable to access the internal storage for putting on the zip file containing a new version of Cyanogen mod.
I would be glad, if someone could help me. I see the (storage) device on the "computer" but it looks like a hidden file. If I try to access it by a command prompt, it says "The device is not ready".

Thats just what i like to know TWRP is still fine? You could flash a actual recovery which supports OTG to get a romfile to your recovery.
Easier was to mount the storage to windows. Does your phone handle the storage as "MTP" or "Mass storage" ? Have you been able to mount sdcard to your computer using TWRP when a working android version was installed? Or did you need boot up android to enter the phone's storage?
Edit:
forgot "g®eets"
Edit II:
You could also extract the needed partition "system" as system.img from a OTG update and flash it using fastboot. Running a RUU should work , too
Edit III:
You said nothing about you ereased the internal SD, so where are the pervious installation files and your backups gone?

TRPW seems to be ok.
If I press "mount USB-OTG", it says "unable to mount...".
Before I pulled the new zip files from the PC to the phone was in the "booted android" mode. I never tried opening the storage from my computer, when I was in TWRP, so I'm not sure, if it is a driver problem or something other. But I backuped most things this morning from my computer, so technically the driver shouldn't be broken in my understanding.

My fingers are already burning from typing for hours to fix phones lol. If you tick "fastboot" and connect your phone to the computer it will show up "fastboot usb"
Then you can flash .img files using the windows commandline by typing "fastboot flash recovery recovery.img"
If you never did before or just be not sure about
May be install "Teamviewer" As i will only need some minutes to do.
If you do PLS DO NOT post the id+pass here in the topic BUT send me a pm. Yes people already posted the acces codes here in public chat lol.
To explain how you (or me) can solve actual problems according to your phone:
Flash TWRP 2.7+ revovery to the phone by using fastboot command
Within TWRP you have a "ADB Sideload" feature
Means once the new recovery is installed a Rom.zip can be "pushed" to your sd-card by using ADB command
Then you'll have a flashable rom on your sdcard automatically gets flashed
Just tried all steps on mine own phone. Worked without any issues. Even restored my backup again
