I figured it out! =D I ran around searching and found an SBF for the 4G LTE. It is 4.0.4 and worked like a beauty. I have re-uploaded the SBF with RSDlite in the file as well. You can find that here.
To revert to stock just...
1. install RSDlite
2. Flash the file provided in the zip
3. Reboot into fastboot mode and re-unlock (I don't know if that is mandatory, did it anyway)
4. Reboot into fasboot mode and re-flash recovery (Again, I don't know if that is mandatory, did it anyway)
5. Reboot and it should boot Stock ICS just fine!

My Xoom has had an odd problem. When I got it new, I unlocked the bootloader and rooted.
However, any pushed updates from Moto such as HLK55 caused my Xoom to boot and show the unhappy !Android icon. I had to shut down cold, and reboot. In other words, it never updated properly.
Now that ICS is out, I have Kang 1/15 running but have some non-crash issues. I'd like to flash to try to update to the factory ICS push file but fear that my XOOM won't complete as well.
Any ideas?
So long as you flash back to complete stock using the Motorola files you should be able to upgrade your way up to ICS (via 3.1 and 3.2).
Get the files from http://developer.motorola.com/products/software/
And as on that page, get ADB running and make sure it detects your device then run these commands.
fastboot flash boot boot.img
fastboot flash system system.img
fastboot flash recovery recovery.img
fastboot flash userdata userdata.img
fastboot erase cache
fastboot oem lock
Make sure you get the right version for your device.
im not the most experience with rooting and roms but i have never had issues before with my glacier and pyramid, however this HtC one s is pissing me off...ive rooted custom recovery and all but while trying to flash ROM cm9 unofficial (which looks great) i have managed to F it up and am stuck on cm9 bootloop, tried everything dont know what im doing wrong no back ups either for what ever reason

Do factory reset/wipe data and wipe cache. Flash rom. Reboot into bootloader. Fastboot flash boot boot.img. reboot phone. Then reboot back into recovery to flash gapps. Should work "

If you haven't, extract the boot.img file from the .zip and place it where you have adb/fastboot (Android SDK). Then boot into bootloader, go into Fastboot, plug phone in for "Fastboot USB" then type the following in command prompt and reboot.
fastboot flash boot boot.img

ive tried all that boot.img from zip even another one with the download no luck just stuck on boot with that cm9 rom on loop thanks for your help though guys any other suggestions other then just shooting myself in the head??lol
Try flashing Paranoid/AOKP and then flashing back to CM9.

this is my first attempt of any ROM for this phone starting with cm9 alpha 13 does anyone recomend maybe installing a older alpha?? maybe alpha 10 cm9?? idk
Try flashing the latest alpha 14. Maybe you got a bad download of alpha 13.

I had the same problem. But this worked for me.
wipe all -> flash cm9 14 -> wipe all -> flash gapps -> wipe all -> fastboot flash boot boot.img ( i used the touchfix one ).
then i got in.. but took some time first load. But now i cant get wifi to work and cant see simcontacts. Drives me nuts.
Just reflashed all with no luck.
This is done on a Xoom 2 ME(mz608)
I had bricked my xoom and tryed to revive it.
During the process I hoped to have booted an alternative boot img, but without success.
Next I got the "ap fastboot flash mode s flash failure" after each reboot.
To remove this error I dit the following:
I downloaded the right sbf file(1.6.0M-272.12_MZ608_p2HW_BlurRegion02_CFC1FF_fastboot_signed_latam.xml.zip) for my xoom from http://sbf.droid-developers.org/xoom2/list.php
I extracted it.
Then through fastboot I flashed the preinstall image: fastboot flash preinstall preinstall
After this the error was gone.
However I still have a brick an would need a system partition.
CAUTION: Do not use the sbf to flash your xoom if you don't have the latin america version.

Trying to root using easy ace root tool. All seemed fine until I tried to flash rom. Now phone will boot to HTC screen and stop there. I can get into CWM and hboot, I have copied files to the SD from there but I can't get it to boot normally. I have tried putting a new PD98IMD but am not totally sure I have the right one. I miss the AAHK
You need to fastboot flash a boot.img.

I tried that with no change
Unlock your bootloader and flash a custom recovery (CWM or 4ext)
Download the rom zip file you are willing to flash to your PC.
Extract the the boot.img from inside the zip file (do not extract the whole rom)
Put the zip file in your sdcard.
Boot into recovery and do a full wipe (wipe data/factory reset, wipe cache partition, format system)
Flash rom (install zip from sdcard)
Reboot into bootloader again and plug the phone to the PC.
You need to be in fastboot usb (if you see HBOOT USB PLUG in the phone, just press power to toggle)
Open a cmd/terminal and navigate to where the boot.img file is located.
Type: fastboot flash boot boot.img
Reboot and now you should be enjoying your custom rom
And what does it say when you run fastboot flash boot boot.img?

Hi all,
I was fed up with waiting for a signed rom to update to Cyanogen 13 COS, so after quite a few attempts I found a solution!
Happy to share with you.
Step 1: back to stock recovery and stock CM 12.
I was on CM 12.1 YOG7DAS2K1.
Download the Fastboot image corresponding to you OPO from this very nice page: http://forum.xda-developers.com/oneplus-one/general/official-cm11s-roms-ota-updates-t2906746
In my case I downloaded: cm-12.1-YOG7DAS2K1-bacon-signed-fastboot.zip
Extract the zip file on you PC.
Then install ADB & Fastboot on you PC (see here: https://twrp.me/devices/oneplusone.html)
Then reboot your Oneplus One to Fastboot (Volume Up + On/off).
Then flash the following files from the cm-12.1-YOG7DAS2K1-bacon-signed-fastboot.zip file with the following commands:
fastboot flash recovery recovery.img
fastboot flash system system.img
fastboot flash cache cache.img (don't ask me why but it was required even if it just wipes the cache in the end)
DON'T FLASH BOOT, NOR DATA, of course...
Then reboot:
fastboot reboot
You should be back to your smartphone as before (if not sorry, something went wrong!!).
Step 2: Install CM 13!
Then, if you had the OTA pending (that was my case) just let Cyanogen apply it. This has worked like a charm for me, finally!!
If not, I am pretty sure, you can download it from the same page - http://forum.xda-developers.com/oneplus-one/general/official-cm11s-roms-ota-updates-t2906746 - and apply it manually via the stock recovery (volume down + on/off).
Step 3: TWRP
Then, if you want you can re-install TWRP and root your device. See: https://twrp.me/devices/oneplusone.html
I hope it will work for those who are also waiting for the signed image which doesn't come!!
Remember to backup your device with TWRP before doing anything (and to transfer your backup files to a PC, just in case you would fully wipe you OPO by mistake!!
