I had a similar problem, although I did not S-Off my phone (I avoid doing this, as it completely voids my warrenty)
It appeared to be a software issue in the ROM I had installed, I made an attempt to give my SD card a new format, what appeared to happen in my case is the media would unmount, the partition table would be deleted, the phone would attempt to mount the media again without the partition table and simply give up
I solved this by inserting my SD card into another phone and that would give the SD card its format, works fine now
I hope this helps, if you don't have aother phone put the new format on the SD card I believe 4EXT and CWM recovery both have options for you to do so

You need to format sdcard..
The method is this way
1.flash 4EXT recovery
2.connect your phone with sdcard to pc
From recovery
3.then toggle usb storage.. In 4ext recovery
4.a pop up appears on your pc to format the sdcard.. Format it and enjoy (fat32)

Is there any reason you prefer this method? This may sound rather presumptuous but from personal experience it is best to do this away from Windows as Microsoft don't provide full documentation for all their products

n particular reason ..that way SD-card is identified pretty easily thats all
and if 4EXT identifies the sdcard formatting it there itself will do too
actually there is a better tool .. SD Formatter (windows one) and it works perfectly well ..it uses algorithms optimized for sdcards rather than generic ones

Problem solved
I flashed recovery. Now i partitioned my SD card via recovery 2gb to 0 (not sure if thats correct tho) Rebooted and it now recognizes my SD card again.
Now trying to flash a ROM.
*UPDATE*
Flashed a ROM and up and running, all is working well again. Thanks for the help!

Rather then go back wards why not go forwards?
Be careful not to quickly take out and replace your battery
Look at the INDEX sticky thread in the development Section http://forum.xda-developers.com/showthread.php?t=1146601, there you should find
1) EXT4 recovery
2) pre rooted stock roms, use the latest version
(RUU_Saga_HTC_Europe_1.47.401.4_Radio_20.28I.30.085 AU_3805.06.02.03_M_release_199410_signed_prerooted)
Download both to you PC then copy to your sd card.
Boot into recovery and first flash EXT4 zip file to install the new recovery
Then from recovery wipe cache, dalvik cache, data and system.
Then flash the ROM zip file and hopefully this should solve your problem
First boot will take longer than usual
I'm also curious as to why your bootloader screen days "meadow" I've never seen that before.

Thank you for your prompt response!
After I had written my post yesterday I took the battery out of my unit and put it in again and there after, the total death now I can not turn it on. so there is probably nothing more than to bring it back to warranty?

Bugger, when you say won't turn on, exactly what happens or doesn't happen?
Are you sure that the battery is charged (daft question) but had I ask
plug it in and see if the led lights

When I press the power button nothing happens .. no light in the LED phone is 80% power remaining.
I sent it for repair on warranty :-(

You need to follow the guides shown in this thread.
Look for this "How To: S-Off your device with Revolutionary: S-OFF & Recovery Tool" part of the guide.
Good luck....update this thread if you have any issues or problems....but first there is a lot of software that you'll need to download and some reading to do.

OK if i understand it right i wil do like this.
S-OFF your phone using Windows:
1. Download Revolutionary tool for Windows from http://revolutionary.io/ (Keep the browser open)
2. Extract the zip file to a new folder.
3. Connect phone to your computer and make sure that Debugging Mode is enabled on your phone.
4. Open the new folder and right click on "revolutionary.exe" and select run as administrator.
5. Follow steps from Step 11 above.
11. It'll provide a key. Go back to the browser window where you downloaded Revolutionary tool - there's a form to fill out. Type in the key. It'll generate a second key. Type that key into the Terminal.
12. Make sure it says that everything is successful. It'll ask if you want to flash Clockwork. Type in "Y".
13. Once it shows success, reboot phone.
And root it hwit Gingerbreak right ?
Now you have CWM, so you don't have to use Gingerbreak, just flash su (http://goo-inside.me/superuser/su-2.3.6.3-efgh-signed.zip) from recovery. You may also want to flash 4EXT recovery (http://forum.xda-developers.com/showthread.php?t=1130004) that will replace the one installed by Revolutionary.

CWM = Clock Work Mod, which is the type of Custom RECOVERY that we all use that enables us to flash custom ROMS etc, I'd recommend that you upgrade to the the EXT4 RECOVERY which is based on CWM but is more user friendly and constantly being improved.

For example two people have suggested that you now install EXT4 over CWM (RECOVERY) have you done this?

Check for super user, simply download ROM Manager (free) from android market and that will tell you.
A Backup (or a Nandroid backup or as you'll here it referred to) can be taken two ways:
1) from ROM Manager - easiest option (as it then boots device into recovery, backs up phone and then reboots again)
2) from recovery (EXT4) you need to take manually Backup option (although is good to know how to do this way)
Using both methods the backups will be stored on your SD card in clockworkmod/backup directory within date&time stamped folders

Desire S won't boot following ICS installation

Apologies in advance as I know there are many similar threads on here but I've read so much I'm just getting confused! Here goes...
Yesterday I tried to install the official version of ICS onto my Desire S. I followed all of the steps and the computer said the software had been successfully installed. Since then my phone will not boot past the HTC logo screen. I can get into the hboot screen by booting with power on + volume down and my computer recognises the phone when i plug it in in fastboot. It won't go into recovery mode and a factory reset doesn't work. I have tried re-installing the software but it makes no difference. I bought the phone second hand and it was already rooted so I don't have the programs that were used.
When booting in hboot the following info apppears:
-revolutionary-
SAGA PVT SHIP S-OFF RL
HBOOT - 6.98.1002
RADIO - 3831.19.00.110

I have read that this version of hboot is not compatible and that I require an hboot upgrade but i'm not sure how to do this as I cannot access the sd folder of my phone and I don't have an external micro SD reader.
Any help would be greatly appreciated. Even if means reverting back to gingerbread or using another ROM. I just want my phone to work again!

if your phone is detected in fastboot, the best bet you have is to install a custom recovery and then flash another ROM through it.
check this thread, you may find something to help you.. http://forum.xda-developers.com/showthread.php?t=1186401
I would recommend the CM10 by nk111. http://forum.xda-developers.com/showthread.php?t=1776256&highlight=all+credit

Errr u could try putting the file into the sd with another phone.
If u can enter recovery mode, u can connect the sd card with the computer.

I had the same issue and manage to fixed it by following this link: http://forum.xda-developers.com/showthread.php?t=1679338
I used a second sd card, I formatted it first and only put the file I need at that time on it. I
In step 5 Reboot into bootloade, select recovery wait till you see the screen with the red triangle exclamation mark and press the volume up and power button at the same time.
You will enter the Android system recovery <3e> and see the following options:
reboot system now
apply update from SD card
wipe data/factory reset
wipe cache partition
wipe the cache partition and wipe data/factory reset
Now following step B from this link: http://forum.xda-developers.com/showthread.php?t=1858508
When update is finished - reboot, then configure ICS and delete PG88IMG.zip from sd card.
Thanks very much for your help guys. I've managed to sort the problem. For everyone else out there with the same problem I'll tell you what I did....
The hboot version on my phone (6.98.1002) is not compatible with the official ICS software update.
http://forum.xda-developers.com/showthread.php?t=1488672 - this thread shows how to update the hboot when stuck in bootloop.
I then reinstalled the ICS ruu onto my phone and it is now working fine although it is no longer rooted.
Could someone please tell me the best way to root my phone again. In the bootloader menu it still says revolutionary s-off but i cannot access ROM manager or titanium backup. Do I need to fully root it again?
Since you installed the official RUU, your root is probably gone.. You will have to fully root the phone again to get root access.
You could try flashing a custom recovery in fastboot, and then flashing SuperSu or superuser from recovery.

This. As long as you're S-OFF, rooting is just a matter of flashing custom recovery (using fastboot), and flashing SuperSU ZIP in recovery. The first step is covered in so many tutorials that any search will lead you right to it.

[Q] In need of expert help: I accidently did a factory reset in HBOOT

Hi everyone n00b here and I apologize ahead of time for anyone needing to spell things out for me. I just got a new HTC One X and I had unlocked the bootloader using the CID 1111111 hack. Everything worked out great and I had even installed the Cyogenmod but I accidently selected factory reset while in HBOOT mode. TWRP is still accessible but I've lost all my android folders on the SD cared and anytime I try to reboot the phone it stays on a boot loop. It does not get past the Cyogenmod logo. Plug in phone.. go to "my computer" right click "removable storage" (if your computer recognizes it). Select format.
Once complete, I would personally download cleanrom 5.1. Transfer to phone.
Flash entire ROM through twrp and reboot. That will give you a stable ROM to work with with no fastboot kernel BS...

I'll give you hints.
You need to format your SD card with Original HTC recovery to format it correctly.
Now I don't know if stock recovery would work with Hboot 2.16.
If it doesn't, you have to run a 4.1.1 RUU which will do the job for you. Then reinstall everything (hboot 2.16, 4.2.2 rom).

Every time i Try to mount my internal memory it says "unable to mount internal memory, unable to mount /sd, etc.. and i don't know what to do
I really want this phone working and I have just bought it the other day.. it also ask me to install every time i reboot to install SuperSU because my device isn't rooted then it says fail to copy su binary from /sys, when I only know that it's unlocked
It was working 2 days ago with the MaximusHd 4.2.2 rom..
i can still enter bootloader

I assume your SD Card is wiped, containing no ROM, nothing.
-Flash the TWRP 2.6.1.0 recovery that comes with MaximusHD ROM Install Package through bootloader.
It is the one that is compatible with hboot 2.16.
-Use adb sideload to flash the CM11 ROM from Development section.
-Boot to OS, connect phone to PC and transfer Maximus HD ROM to SD Card.
-Go in recovery, flash Maximus, Reboot.

USB OTG?
You need to connect phone to PC from ROM not recovery..
It will be connected in MTP mode..
You need proper drivers on PC for your phone to get detected..
