I assume your SD Card is wiped, containing no ROM, nothing.
-Flash the TWRP 2.6.1.0 recovery that comes with MaximusHD ROM Install Package through bootloader.
It is the one that is compatible with hboot 2.16.
-Use adb sideload to flash the CM11 ROM from Development section.
-Boot to OS, connect phone to PC and transfer Maximus HD ROM to SD Card.
-Go in recovery, flash Maximus, Reboot.

USB OTG?
You need to connect phone to PC from ROM not recovery..
It will be connected in MTP mode..
You need proper drivers on PC for your phone to get detected..

I had a similar problem, although I did not S-Off my phone (I avoid doing this, as it completely voids my warrenty)
It appeared to be a software issue in the ROM I had installed, I made an attempt to give my SD card a new format, what appeared to happen in my case is the media would unmount, the partition table would be deleted, the phone would attempt to mount the media again without the partition table and simply give up
I solved this by inserting my SD card into another phone and that would give the SD card its format, works fine now
I hope this helps, if you don't have aother phone put the new format on the SD card I believe 4EXT and CWM recovery both have options for you to do so

You need to format sdcard..
The method is this way
1.flash 4EXT recovery
2.connect your phone with sdcard to pc
From recovery
3.then toggle usb storage.. In 4ext recovery
4.a pop up appears on your pc to format the sdcard.. Format it and enjoy (fat32)

Is there any reason you prefer this method? This may sound rather presumptuous but from personal experience it is best to do this away from Windows as Microsoft don't provide full documentation for all their products

n particular reason ..that way SD-card is identified pretty easily thats all
and if 4EXT identifies the sdcard formatting it there itself will do too
actually there is a better tool .. SD Formatter (windows one) and it works perfectly well ..it uses algorithms optimized for sdcards rather than generic ones
Problem solved
I flashed recovery. Now i partitioned my SD card via recovery 2gb to 0 (not sure if thats correct tho) Rebooted and it now recognizes my SD card again.
Now trying to flash a ROM.
*UPDATE*
Flashed a ROM and up and running, all is working well again. Thanks for the help!

[Q] [HTC One S][S3] Urgent Help Needed

Hey All,
I recently tried rooting my HTC One S (s3) and have had a few problems.
1. Unlocked bootloader successfully, flashed TWRP 2.3.1.0 successfully but won't mount to USB or won't mount sdcard or storage when I try and flash a ROM.
E:Unable to mount '/sdcard'
E:Unable to mount 'storage'
Or when I mount USB my computer's asking to format the device.
2. I lost my OS (I had Android 4.0.3 and Sense 4.0) and pretty much can't use my phone as a phone. Tried looking for the right RUU but none of them fit my device.

Did you install the S3 version of TWRP? If you have and didn't get to flash a rom then you need to flash stock recovery then factory reset in bootloader.

So is SD working now then since you said it didn't let you flash a rom or did you mean recovery? I see you was looking for that so I go with that, so what you need to do is hold Vol- when turning the phone on and it should load the bootloader, from there you should see factory reset select it and after it's done it's thing everything should be working.

Bricked Htc One S

Hey I am hoping someone can help me. My friend recently bought an HTC One s off of craiglist and it was rooted with a rom by "Team Venom" immediately after purchasing it started acting up and now it is stuck on the boot screen. I can get into the bootloader but then it only starts in to "team win recovery project" and it will not mount. Where can i go from here? Any info would be great.
What version of team win? Is he s off? You may be able to push for via adb. I can never remember how so you'll have to Google it.
In some versions of team win you can push via the advanced menu.

It's team win Recovery v2.1.7
And I believe the S is on
The phone reads:

In the advanced menu I have four options Copy Log to SD, Fix Permissions, Partition SD Card, File Manager
Thanks in advance
Seems like the kernel (aka boot.img) is corrupted.
Mainly you should try to reflash the kernel in recovery. As far as I understand your post, you can't mount the sd-card in recovery. Correct me if I'm wrong.
The first thing to do is to flash an actually working recovery. You can find an overview here, simply download TWRP 2.3.3.0 in the S4 section. As there is no Viper rom available for the S3 I simply assume you got an S4 device.
Boot your phone into fastboot mode and flash the openrecovery-twrp-2.3.3.0-ville.img file by typing 'fastboot flash recovery openrecovery-twrp-2.3.3.0-ville.img' from within your Android sdk workfolder (platform-tools). You should be able to boot into TWRP 2.3.3.0 afterwards and check, if sd-card can be mounted. You should see a mount button in the menu...
Also you should do a nandroid backup of this device at this point. You'll find backup in the menu.
If this is working, you'll need the kernel. Corresponding to your bootloader version and your radio I think we need the ICS version of the Viper rom. You can find it here #6. Download it, unzip the file boot.img into your workfolder, then type 'fastboot flash boot boot.img' in fastboot mode from terminal/cmd prompt.
Try to restart the phone and wait some time. If it is the wrong kernel simply download the JB version of Viper from here and follow the instructions above.
Flashing the wrong kernel (ICS <-> JB) won't affect your device as long as it is for the S4 device. It simply doesn' boot or work correctly. Reflashing the right one fixes that.
If none of the above helps, I'd simply suggest to restart from scratch with Viper 2.1.0 #6.

Since the device is not new, I would suggest to start from scratch anyway, just to be as clean as possible.
Run the oldest RUU that you can with your hboot version. (I think hboot-1.13 is RUU-1.78.401.2 but make sure..) Then clear storage, factory reset in bootloader, then flash a custom recovery (TWRP-2.3.3.0 or latest CWM), root your OS, and get S-Off. At this point you can format everything including sdcard (in recovery) and you're ready to push your new custom ROM and do a completely clean install.

I'll give you hints.
You need to format your SD card with Original HTC recovery to format it correctly.
Now I don't know if stock recovery would work with Hboot 2.16.
If it doesn't, you have to run a 4.1.1 RUU which will do the job for you. Then reinstall everything (hboot 2.16, 4.2.2 rom).

I did install TWRP 2.6.3.0 after posting this thread...
and mount the sdcadr to my computer and format it... with quick ntfs.. and try to copy different ROMs
but none of them worked...\
then I try to mount only data and push rom using adb push... but still didn't work
To install RUU, you must have stock recovery and locked bootloader. Also CM11 needs new recovery which you can find in first post of CM11 development thread for this device. Finally don't forget to flash boot.img with fastboot whenever you flash ROM.
where I can download ruu, cm11 and boot.img
and can you please explain in detail how I can install new rom... i'm totally screwed and I have a brick htc one s right now...
with trwp 2.6.3.0 recovery... don't have any backup..
I think you should follow the instructions from this thread:
http://forum.xda-developers.com/showthread.php?t=2528113&highlight=back+to+stock
After flashing one of the recoveries from this thread you should try to wipe sd from recovery, then copy cm11 to your sd and flash it
