The same thing just happened to me on my Samsung S9. I found this post and re-applied the default theme. The icons came back but I had to go back and reset my wallpaper. I had settings set to update over wifi only. It did say there was an update so I switched that setting off and let it update. Rebooted again and still lost the icons. Still fixed by reapplying the theme.

Yes this is the only fix for the moment I'm afraid or you can go and kill your current launcher process. This will bring the icons back... but after rebooting the icons disappear again. I contacted Samsung here in Greece and they connected with me with Smart Tutor. They said it's probably a bug but they recomended a factory reset... They said after an update they recomend a factory reset for the patch to install more smoothly... This MAY eliminate the bug... MAY... don't know if it's worth all the fuzz of a factory reset just for a MAY... The funny thing is that I have a folder on a home screen with all the games that are currently moved at my SD card and they show up just fine after a little while (time to load from SD card). This is the opposite... instead of losing icon the shortcut the icon of the app its self gets lost. You can delete and replace the icon of a shortcut from the appdrawer but... you cant replace something in the appdrawer...

[Q] launcherpro and icons

using launcherpro plus on a htc desire running on cm6.
everytime i reboot my phone, icons inside folders(i have a Games folder which i put all my games on) would change to the default green droid icon.
its only for the apps that i store on my SD card.
anyone know a fix to this? or is there some setting?
just started on android so not 100% sure, but I noticed that when you reboot the phone it takes a few seconds to mount the sd card even though launcher pro has loaded. Try storing just the icons themselves on the phone memory and when creating the shortcuts select a custom icon, ie the ones from phone memory and you should be sorted.
It will happen everytime you restart. All you have to do is restart launcher pro and your custom icons are back...

if i store icons on phone memory, it works fine. the icons dont change even after reboot. its when i put on SD Card, and then i do a reboot, the icons will change to the standard green/white droid guy.
and the problem is only for icons withing FOLDERS. if i put the apps outside of folders, they wont have this problem even if they're stored on the SD card.
so what you're trying to say is, store them on the phone, then create a shortcut to a custom icon, and put them in the folder, then put them back to SD card? i'll give that i try.

yea that works, i guess ill have to do that each time i reboot then. so whats the best way of doing this.
So far i have launcher pro set as default and eveyrtime i boot up , i go to manager applications > laucher pro > force stop
then it should be restarted. is this the right way of restarting it? it seems to work. i just dunno if theres an easier/simpler way of doing this.

From your homescreen:
* Hit Menu key
* Select Preferences
* Scroll to the bottom of the list
* Select "Restart LauncherPro"
probably has something to do with the memory settings, try playing with it?

Did you move any of the affected apps to the SD card after installation?

Yes i have done
you need to "reload" sense
you can do this easily by changing the skin to any other, then revert to the one you like, and the icons will return to normal
this appears to be a sense 3.0 issue that hopefull is resolved with an update, as it bugs me too

well its not the fone issue nor sd card...its app compatibility issue... most apps were designed for froyo...but now running them on 2.3.3 they waork on sd-card as well but their shortcuts do not identify them after restart coz they do not have exact icon update path...its an appp issue ...till its resolved plz keep apps on fone....do not worry u need to work hard to finish 1 gig by apps....and plz dt ever think apps on fone mk it slower...use advance task killer as well....

[Q] Apps on SD not showing Icon

Hi everyone,
It seems when I move downloaded apps to my SD card and then restart the phone, their icons show as the default app icon if they are on one of my home panels.
Their icons do show up properly in the app drawer, but thats the only place. Also, this problem only happens after a restart. If i remove them and then put them back on the panels, the icons show up!
Anyone else have this problem? Anyone have a work around?
I have the same issue, I removed the shortcuts and replaced them, but they go back to the generic Android icon after a little while. I have my shortcuts in folders if that makes any difference.
C my post on speed + batt calibration + apps....its explained there

I am not sure of a permanent fix but if you change the scene (long press on an empty bit of a home screen) choose the scene menu and choose a new scene, Then just go back to your original scene. Quicker than recreating your icons...

Are you guys still on GB 2.3.3? I had the same problem before the update to 2.3.4
With 2.3.4, the icons always go back to normal after a few seconds.
Had this strange behaviour too. But after moving the apps to SD you have to remove the icons and put them afterwards back on the home screen. This always worked for me.

Running OTA GB 2.3.4. It happen every time I reboot. Normal icons persist on the homescreen but inside any folder the icon lose their mojo. They work perfectly but have the generic icons. I have had this sporadic issue through multiple ROMs including the OTA. That what I like about the "scene" fix. A few click and you are done without recreating the whole folder. (I have a folder full of games for my kids, about 20 items)
I have searched the threads on XDA and found nothing helpful for a permanent fix. (Perhaps my google-fu is weak?) It seems to happen on a lot of HTC devices. I personally have a DINC but I posted here because it was a nice fix that works on all HTC sense ROMs.
Apparently the official answer is to restart sense: http://androidforums.com/htc-evo-3d/361453-icon-bug-apps2sd-solution-post-47-here.html#post2907016
I have seen threads where this happens with other Launchers as well.
It's because when the phone reboots it has to wait for the SD card. Why are you moving apps anyway? You using more than 1GB of apps?!?!

In my case I currently have a DINC, my data/data is considerably smaller and I have not A2SD+ yet. I just found this thread through a search and added a suggestion that worked for me. 1 GB Data partition for phone storage! Man you guys have a nice piece of kit there.

Apploication not found ...

Greetings,
My M2 recently threw a wobbler and decided that it didn't like the SD card which wa sinstalled, so reported that there wasa problem with the card, and then promptly decided to format it.
The problem that I now have is that in the Alphabetical list of apps there are two icons/short cuts with a red circular sd card image in the top right hand corner of the icons and the icons simply show a rectangular blue image in perspective. These are now the first two in the list.
Code:
https://www.dropbox.com/s/ae4bhsbzk39ppg7/Screenshot_2015-12-22-19-59-26.jpg?dl=0
Pressing either returns a message saying, 'Application not found' and I cannot figure out how to remove them having tried everything I can think of, including reboots and resets etc.
Apaert from a factory reset is there any way to get rid of these useless icons ???
Thanks
I also am having this problem. Except my phone recognises the sd card, but I can't transfer items to it, I have r/w permissions, the apps I have on there I can't use, but I can listen to my music and view pictures that I have on there, I have stock lollipop rom and rooted

App Icons Disappear from Home Screen after Reboot

Every time I reboot, some of my app icons disappear from my home screen and I have a to manually put them back to where they belong. I've noticed that only the apps that are stored on my SD card (adoptable storage) are effected. I am currently running CM 13 Nightlies with the Squid Kernel (http://forum.xda-developers.com/201...ment/kernel-squid-kernel-moto-g-2015-t3215726) but it has been occurring when on the default kernel as well. I don't want to just move all my apps to internal storage because I have a lot of them with 16 GB of internal. Thanks for any help!
I have the same problem.
Even worse - some icons disappear from the list of apps too. The one I noticed is Pixlr-o-matic - when I tap the middle button (the list of all apps), usually the icons from SD card which after reboot disappear from the screen could be found here, but Pixlr-o-matic disappears even there. However Google Play Store says the app is installed and gives me the "open" button which really opens the app, so the app is still on the phone.
So is there such a thing like forcing the launcher to re-scan all apps and validate the icons again?
Thanks
Moto G 3rd, with default Marshmallow rom from Motorola/Virgin Mobile/whatever it is (I mean I didnt modified the phone in any way pretty much)
I had a similar experience when I tried switching between the Google Launcher and Nova Launcher. Nearly all my app icons disappeared. I have no clue why. To fix I restored a TWRP back up and stopped switching launchers.
