Open ur dialer and insert:
*#*#7378423#*#*
Now Go to: servis info > configuration >
Now,
bootloader unlocked allow: yes = your bl is lock now.
Bootloader unlocked: yes = ur bl is unlocked now.

No, it doesn't require unlocked bootloader

No, you can be root and don't have unlocked bootloader and have unlocked bootloader and don't be root
