Here is a new one I hope someone has seen and fixed: My M8 was working perfectly, rooted but not s-off, stock ROM, not allowed to update or anything. One day out of the blue I tried to call someone on the phone, had full signal, but when I pushed SEND signal went away, said call failed, and signal came back full signal. I looked at the screen in LTE OFF which shows the options and the CDMA radio options were gone. LTE and GSM are there, but EVDO or any CDMA options are just not listed as if they were never there. I can use it as a 4G LTE hotspot and any app which does not need CDMA. Anyone seen this before ?? I wonder if it corrupted the radio ROM by itself ?

Exact same thing happened to mine a few days ago I still have no idea how to fix it. Anyone have any ideas?

Put device in fastboot mode and enter the following commands:
fastboot erase modem_st1
fastboot erase modem_st2
Then reboot!

Sounds like you're in bootloader, not in fastboot mode.

Im rebooting into bootloader then im hitting power on fastboot usb. Once in fast boot I am loading the commands in adb. Am I doing something wrong seems right to me. I just reflashed my firmware to see if adb was working with the device and it flashed fine.

In bootloader I just get waiting for device. I don't have a sim card in this phone could that cause any issues with those commands?

Looking for some insight here. I have a second-hand Nexus S GT-I9020A that was replaced because it would no longer boot. I took on the challenge of trying to fix it. Based on the reading I've done so far it may have bad a bad NAND chip, but I'm too stubborn to give up on it just yet. Here are the symptoms:
When booting normally, it hangs on the Google screen with all 4 hardware buttons lit up.
Fastboot works, however I can't flash anything because its bootloader is locked, which brings me to my next point.
When issuing the command fastboot oem unlock, I am prompted with the familiar Yes/no screen but when selecting Yes, nothing seems to happen. I've left it for about 6-7 hours and came back to a message in my PC command window stating "Status read Failed (too many links)".
I can get into the stock recovery but that's really of no help at all. I can wipe cache partition without an issue but get an error when trying to wipe data/factory reset.
I've attempted the One-Click UnBrick software which I read somewhere is compatible with the Nexus S. It did recognize the phone, send a command, and give a success message. The phone rebooted but nothing changed.
So, this leaves me with nothing left to try but to post here. It seems that since I cannot even unlock the bootloader that my hands are tied. Are there any other tools that I should be aware of that can diagnose bad NAND flash? I don't want to toss the phone until I know I've tried everything I can to troubleshoot.

Try flashing the official Android 4.1.2 image from Google in fastboot. See if it boots... Then maybe you will have more luck unlocking it! I would also try to wipe as many thing as possible before doing it. Try system, data, boot, cache, dalvik... Even sdcard might help...

Tried this but as said fastboot is not recognised by my pc. Have installed almost all the drivers I can find to no avail. Must be something else to try.

Well, you should be able to flash official Google images in stock recovery too... Have you tried that?

mY COMPUTER ONLY RECOGNISES abd IN SIDELOAD MODE. tHE FASTBOOT ANYTHING IS NOT RECOGNISED SO i CANNOT UPDATE OR FIX anything. Thanks for trying.
Can you try doing all the stuff you did in some another PC.

It would seem that I can't flash anything, including official firmware, if the bootloader is locked?
On the plus side, the phone responds fine to the fastboot commands and seems to transfer the images to it just fine, just still no luck getting the darn bootloader unlocked

Normally it is possible to flash official ota from Google even if the bootloader is locked. What version did you have prior to your problems? Try flashing the ota package that came just after your version as it seems that the package your are trying with cannot be flashed over the version that you have right now!

Currently the phone has 2.3.6. I tried flashing 4.0.4, which was the next one, and it failed with the same error, indicating I need to unlock my bootloader. Also tried flashing the 2.3.6 firmware, received the same error. I'm starting to think I should probably give up on this phone.
Any more suggestions?

Did you try the OTA update from Google? That's weird cause I flashed it back then and I was on stock 2.3.6 with locked bootloader and stock recovery and it worked. Have you don't anything particular with your phone or is it pure stock 2.3.6? Start there

Thanks for posting that link. Still no luck though. I tried the 2.3.6 (GRK39C) -> 2.3.6 (GRK39F) OTA file and the 2.3.6 (GRK39F) -> 4.0.4 one as I am not sure which version of 2.3.6 that I have. Basically I am trying to use fastboot to apply the OTA update using fastboot -w update filename.zip however it is failing with the following message:

You have to rename the file to update.zip and place it on /sdcard. It will install it automatically. Stock recovery won't install custom zip file so that's why you need to do that. Let me know if it works...

so i have to send my 2nd tablet in for my 2nd RMA. it is rooted and running one of the custom roms on here
i want to return it to stock before sending it back (does it matter? do they care? do they check? anyone know? --- this could save me some headaches i suppose)
anyways, i used Philz recovery to wipe all partitions, although data is giving me some issues, saying it cannot mount and cannot unmount.... hmmm... well, i get to fastboot to wipe everything and reflash stock images and it goes a little something like this:
fastboot flash boot boot.img - works
fastboot flash recovery recovery.img - works
fastboot flash system system.img
sending 'system'

so that doesnt work

does anyone have any clue what to do here? is there a corruption of my data partition? anyone know the commands to rebuild the partition (is it possible?). i remember there was some tool or command that was used on the old first gen amazon fire partitions after cwm bricked the devices that people were trying.
does any of this even matter at this point? my new tablet will be here on tuesday, and i can certainly just relock the bootloader so it appears stock, but not booting properly. they will obviously be able to rebuild using their internal nvidia tools i believe.
thoughts? comments? suggestions?

Are you RMA'ing because of cracks or anything else? They do not care about root, and I'll supposed neither for custom roms. But if the reason is related to battery life or anything elese that could be hooked to software, go back to stock.

I have a OPO Chinese 16GB version. When I bought it, it was running CM11S(Seller flashed it). Now I'm trying to wipe my phone and install cm-11.0-XNPH38R-bacon-signed-fastboot.zip on it. I have installed ADB Drivers on my Windows computer and when I type adb devices it shows me the serial number of the phone. And also when I type in fastboot devices, it shows me the serial number. But I can't get it to unlock the boot loader. When I type in the command, fastboot oem unlock, I can see the following message.

But it does nothing after that. Even when I reboot, I can see all my previous apps, files etc. are there. It doesn't make any change. How can I get this phone to unlock the bootloader and allow me to install a custom recovery and let me flash the above file I have mentioned?

There are How-to's on this site. It's probably unlocked now. Try flashing a custum recovery in fastboot.

That means that the device was already unlocked. Where did you buy yours? Ali Express? Mine's from there and I have gone through what you're going through right now. No worries. Bootloader is already unlocked so you can root it already.

I bought that off of eBay. I have tried to flash a custom recovery but it doesn't seem to get installed as well. After I do it, I only see the stock recovery but when I run the command fastboot flash recovery recovery.img it gives me the following message.

Settings > Developer Options > Update CM Recovery (untick)
Then flash your recovery via fastboot and it'll stick.

I was able to flash a custom recovery. Thank you. I'm now about to CLEAN INSTALL XNPH38R on my phone. Any ideas on how to do that?

Download the factory stock images on cyngn. Find it online and fastboot flash it. There's a good guide online in the general forums that shows you how to go back to stock

I got it working. I flashed TWRP on my phone and wiped the phone. Downloaded http://builds.cyngn.com/factory/bacon/cm-11.0-XNPH38R-bacon-signed-fastboot.zip , extracted the contents and flashed it with fastboot using the following commands.
fastboot flash modem NON-HLOS.bin
fastboot flash sbl1 sbl1.mbn
fastboot flash dbi sdi.mbn
fastboot flash aboot emmc_appsboot.mbn
fastboot flash rpm rpm.mbn
fastboot flash tz tz.mbn
fastboot flash LOGO logo.bin
fastboot flash boot boot.img
fastboot flash cache cache.img
fastboot flash system system.img
fastboot flash userdata userdata_64g.img

I got an xt1031 on ebay that was flashed to work on Page Plus. I have restored the ROM to falcon_KXB21.14-L1.57.zip but the baseband still says "VZW_CUST" and I get activation stuff when I start it up. How can I get this working like a regular xt1031 again? I am rooted, unlocked and have CWM installed. I also have a regular, working xt1031 if I need anything off of it.
I can't find any information about restoring baseband or anything like that. After flashing the stock ROM I restored the phone.

I would start by only flashing the 'system.img.sparsechunk' files:
mfastboot flash system system.img_sparsechunk1 (CHANGE NAME TO MATCH ACTUAL FILE IN FOLDER)

Would that be more effective? Because I would be just as happy to flash the entire ROM again and start over if it would work as well.

My recommendation is for your safety. Messing with fastboot has destroyed many phones.

Everything seems to have gone well but I still get the activation screen and the baseband still has VZW_CUST.

From memory, I do recall Verizon software being present in the Boost Firmare Images. But you can delete certain apks to stop those activation screens appearing. If you install TWRP, you can use the file explorer to delete the folders. I will try to get the correct names soon, but you might be able to guess them - they are located in /system/priv-app

But I have a bunch of these phones and none of them do this. There are some activation apps, but they are never intrusive like this.
On one hand I'd like to get it to go away any way I can, but I would most prefer all of my devices to be set up the same. Is there anything I can do given that I have an identical model that is set up the right way? I don't know enough about how android partitions work to understand why flashing a stock ROM and resetting it wouldn't make it act exactly the same as my other devices that I have done the same with.

Those are the stock activation apps that are needed if you plan to use the phone. Typically, they "go away" or are "unseen" when the device is activated.

@HardOnChairs - Apologies, I got confused between Verizon and Sprint. There are no Verizon files in the Boost Firmware, only those for Sprint.
The conversion to PagePlus includes flashing the XT1028 Bootloader - so that's still the Bootloader you have. Flashing the XT1031 Boost Bootloader may be a necessary step, but if something goes wrong you have a brick on your hands.

I do still get activation alerts on my other ones but they are not intrusive and they can be disabled. All of the xt1031 phones I have are on wifi only. Does that mean that they are still "activated" from before? In that case is it possible to activate these two that I have without actually buying service for them?

I have managed to update the bootloader of one of my devices and it actually caused some issues. The display constantly glitches. This was when I was trying to update to bootloader to work on 5.x. Do you know where I can find the original bootloader for 4.x? I never managed to find it. Is it possible to pull it off of one of my other devices? Or do you know if that zip I got has the original one?

Be very careful with Bootloaders. You cannot go back now that you have upgraded. The display 'glitch' is caused by running KitKat ROM with a Lollipop Bootloader. Turn screen off once to stop it.

I see. Well that aside, thats a different phone altogether, is it possible for me to check which bootloader these new phones have? And if they are on the 1028, is it possible for me to go back to the 1031 KK version?

Check the Fastboot screen:
i) The 'hardware' way.
a) Unplug the USB cable and power off the phone.
b) Press and hold 'Volume down' then power on the phone.
c) Connect USB cable to your computer.
ii) The 'software' way.
a) Enable developer mode on the Moto G (Settings-> 'About Phone'. Tap 7 times on 'Build Number')
b) Enable USB debugging. (Settings -> 'Developer options' -> 'USB Debugging' -> check)
c) Connect phone to computer via USB, you should see a new ADB device detected and drivers installed.
d) Open Command Prompt or Terminal, navigate to the Firmware / ADB folder (i.e CD C:\motog_firmware ) and type:

I don't know if you can safely flash the XT1031 KitKat Bootloader now that the phone has been converted to XT1028 - but it's likely considering that's the original Model of the phone.

To be sure what is KitKat? v41.13 is 100% not a Lollipop Bootloader.

I have a small problem. A week ago died my phone when i unlocked the lockscreen one evening. Thought that the battery was empty, i put it on the charger and started up the phone a few hours later and the only thing that happens is that Google logo is displayed, then it starts over again. I can go into recovery and fastboot, but it does not matter what i do there. Managed to get it to be connected as a USB drive so I got out the pictures from it. But if you make a complete factory reset including data and restarts there everything left! Tried to run "fastboot format cashe" but it only return "FAILED (remote: flash write failure) "
The phone is rooted and bootloader is unlocked, i had a custom rom installed, and it had worked since beginning of summer.
Have you already tried to reflash stock rom with fastboot? Maybe that will help to get your partitions working properly...
i followed a guide on androidrootz dot com, "how-to-unroot-nexus-5-complete-stock" but directly when i try to flash bootloader i get a error message.
Does the bootloader remain unlocked? What error are you getting when flashing stock?

Yes! It remains unlocked! Heres the "print screen"

You could try Wug's Nexus Root Toolkit and get it back to working. If you can get to fastboot screen, then it can get you up and running again.

Looks like the onboard memory chip is damaged or has failed.
My N5's bootloader would always relock on reboot so I could never flash a stock ROM.
i don't think you have unlocked bootloader , maybe there is some errors try with "fastboot oem unlock" and then try to flash everything

The telephone is unlocked, the lock is open when i reboot the phone.

I don't know how it got damaged. One day, ti started bootlooping and the bootloader had re-locked on its own even though I unlocked it straight out of the box. I booted into fastboot, unlocked the bootloader, and it only too a few seconds to erase everything. I immediately rebooted in fastboot and the bootloader was locked again. This unlock/relock happened over and over.
I booted into recovery and tried a factory reset but recovery could not mount cache or data to perform the erase.
My phone just kept getting stuck at the Google logo or bootlooping.

I have tried that also, when im just type "FASTBOOT OEM LOCK" it lookes like this:

PERFECT!?!? But when i reboot its still unlocked.... if i do first lock, then unlock without rebooting, i get a new meny on the phone where i have to chose if i want to unlock my device. and if i chose yes it will reboot and its the same again....
How do i download that file??? I just got an EXE file, and i dont want to run that!
If it's not sticking on a reboot, that means the memory is most likely gone bad. I guess you could try using the diagnostic tool, which would confirm if the memory is bad.

after flashing it can take up to 15 mins to turn on because of first boot

I'm getting more and more sure that the memorymodule is faulty in the phone. If you reboot in Recovery and select filemanager, for example you remove a directory with files, if you restart the phone, the directory is there again! And when you start it up in recovery and connect it to the computer, theres a few directorys you can write/delete/. Similarly, if you run "fastboot oem lock" and reboot it, its locked the first time it is started up and then the second time it is unlocked again.
Lost hope a little there and went off and ordered a Nexus 6P, hoping it will arrive on Wednesday.
