Sound issues persistent even after september OTA update - Nexus 5 Q&A, Help & Troubleshooting

Hi everyone,
I'm having a weird sound issue with my Nexus 5. I am running everything stock and updated via OTA to the latest version.
It all started last month when in-call sound stopped working. It works fine if i switch to loudspeaker but normal speaker is not working.
Then for all other sounds, music playing, youtube, etc. the loudspeaker doesn't work. The volume setting is doing some strange things like resetting itself to about half.
Then if i plug some headphones in, i get sound through them, and when i take them out, the loudspeaker suddenly starts working again, but not for long.
I'm quite confused about all of this. I know the July update had some bugs messing with the sound, but i am now updated to the september release and i still have these problems.
Any thoughts?

Help losing all volume at times

Seemingly at random times I am losing all sound on my phone at times. I have my phone set to vibrate and play a ringtone. I am running Cognition 2.2b6. Any ideas on how to fix? Thanks!
I just called my phone and the ringtone started to play but then stopped after the first level of sound (normally the volume gets louder and louder). It seems like the external speaker is not working. I tried to use the speaker phone during a call and I couldn't hear anything.
well you are on a very early beta build rom...why not try flashing to newer more complete rom. like cog 4.5...

Speaker Output Issue...

My Nexus 5 is acting like there are headphones connected when they are not...I have not installed any new programs and the speaker was working fine earlier today. When I power down and reboot, I can play music through my speaker, but after about 15 seconds it acts as if headphones are connected again and goes silent..gives the "audio warning" when you press the volume rocker up...sound works through headphones when connected. Anyone have any thoughts? This all started with the 4.4.2 update...first weird thing happening is my wallpaper randomly changing on its own..daydream is not enabled. Thanks on advance guys for your thoughts!! I'm clueless as to the fix... Should also mention, I'm rooted and unlocked boot loader. I've had the 4.4.2 update since the day it hit OTA and haven't had any audio issue until now...the wallpaper changing itself has occurred for a few days now though. The speaker does work, just not sure why it keeps thinking a pair of headphones is connected...
UPDATE : While waiting for a reply to this issue...my wallpaper has changed again, and now sound has returned?? Wth... Stock Google Experience Launcher...maybe I need to clear settings and see if that helps..
Someone with Bluetooth headphones playing a trick on you maybe? More then likely its some app u have installed causing an issue.

Very low speaker volume after latest update

Has anyone else been experiencing very low speaker volumes after the latest update? My speakers used to be great but they sound extremely muffled now. This happened right after I did the latest ota update.
Update: apparently a simple reboot fixed the issue.

Media Volume Mutes on Playback with Headset

Hi,
I've got a strange problem which no amount of searching and experimentation has fixed...
Whenever I start playing audio the media volume reduces to zero / mute by itself. I can then turn it back up and all is well, until the end of that song / video at which point the problem repeats. This occurs in all my media playing apps.
This only happens when using my wired headphones that have a builtin microphone. I don't get the issue on speakers, with a non-microphone headphone / external speaker, or with a bluetooth device.
I've tried the headphones on another phone (not OPO) running the same version of cm13 nightly, and the problem doesn't occur.
So I'm fairly sure that its a software bug/setting issue, but I don't know what.
I've tried making sure all other possible audio players are not started at boot and force stopped to make sure its not some weird app/clash.
Would appreciate any suggestions!

Glad to hear its not just me! It even does it in safe mode.
I guess the only thing to try is a different ROM...

Sultanxda's comments describe whats happening nicely.
So in a last and final attempt to fix this I tried some different ROMs. I tried the latest cm13 stable, sultanxda's latest and even went back to good old kitkat! But all had the same problem....
It would seem this is a hardware issue that is only getting worse. I've had instances now where the volume will mute whilst playing a song). Sometimes it mutes and then turns on Do Not Disturb!
Oh well.... time for some bluetooth headphones.

Phone sound coming from both speakers

Just recently upgraded to the new Oxygen OS version 11 (beta) last night. Now all my phone calls are coming over with both speakers working. What's weird is if I choose speaker, the bottom speaker gets louder and the earpiece speaker is off. Yet when I turn speakerphone off, it will still come out the earpiece and bottom firing speaker. Any way to change this? I looked in the setting for over 1/2 hour and nothing. I've all rebooted the phone several times.
Don't want everyone hearing all my phone calls, lol.

Same. Upgraded with latest Beta OTA, and having this exact problem. Someone suggested disabling "Phone Calls" in bluetooth settings for my smartwacth. That did not fix it. I really do not want to start over from scratch right now.
Ugh same here, ever since beta 1. I wish there was a solution other than hoping the next beta fixes it, whenever that may be
