Voice Calls/Contacts Gone. Help - T-Mobile Samsung Galaxy Note5

So I got all wrapped up in the new TouchWiz Grace UX beta project. Specifically --> http://forum.xda-developers.com/note5/development/developer-base-stock-deodexed-touchwiz-t3400611
I downloaded to sd. Booted into recovery. Factory Wiped via TWRP and installed the ROM. While there are minor updates like the icons and a couple new apps, my settings, notification drawer etc did NOT change to the Grace UX. On top of that I've now lost the ability to make/receive calls. The phone and contact app are gone completely. Frustrated and disappointed, I wiped the phone again to revert back to stock and the issue is still here. What now?
I'm running 6.0.1 on jovi's Stock_Modified Kernel. Any thoughts?

[Q] Anything new as far as custom ROMs?

I, personally, loved the MIUI & CyanogenMod but they, in my opinion, have their downfalls.
MIUI: No app drawer, I like having just 1 page on the homescreen & all the excess apps/games in the drawer, but its layout (customization options (lockscreen/homescreen wallpaper, themed messaging app), bootlogo, etc.) is great but, the worst part was the simple fact that it would be fine over night, then the next day it started booting out the wifi, phone or even put my phone into airplane mode which I could not get out of no matter how many reboots or battery pulls I did.
CyanogenMod: No texting or phone calls at all, it has been awhile since I used it but it either could send & not receive texts/calls or vice-versa, either way just like w/ MIUI, its awesome in every other aspect.
I was just wondering if there are any other custom ROMs for the Samsung Mesmerize SCH-i500, in particular, that are similar to those two, themed and/or fully customizable options, I can't think of the words I'm looking for, I guess a standalone custom ROM that is FULLY STABLE (calls, texts, wifi, bluetooth, no AUTOMATIC airplane mode, etc. I mean EVERYTHING FULLY WORKING) for my, above mentioned, phone, does not have to be debloated/dexed/deodexed, either way.
i am running miui 1.12.9 with glitch kernel v13.1 EH09 radio
for what i use it all works. sms, bt, mms (wifi off), phone, alarm clock, etc..
miui control panel for the app drawer feature. maybe if you had an older version you might want to check out this newer one.
but as for other roms i have not tried any, if i get bored i might try something else but not until i read up on it so i know what im getting into.

I was using MIUI.us_mesmerizemtd_1.12.9_Eng_Deo_ZipA_Signed_dfgas-911fix and it was about 2:30 in the morning when I finally fully restored to stock rooted 2.3.4 Gingerbread, wiped data, cache partition dalvik cache and finally rebooted after installing MIUI, I finally went to sleep at 5 in the morning after setting it all up.
I awoke at about noon & it was still running fine but I ran busybox installer so I could update the su binary & had to reboot, this was roughly between 3 - 3:30 in the afternoon &, after reboot, it just started f***ing up.
Absolutely no phone signal at all, I tried the phone activation code but it was never activated, wifi, even right next to the router, was not detected, tried a few reboots but nothing, it kept saying it was in airplane mode, when I opened the notification bar, deactivated airplane mode, it never turned back on again but only in the notification bar, in the settings menu, after turning it off & exiting the menu or going to another menu, regardless if it was from within the settings menu or not it automatically switched back on when I returned back to it, I don't know, if I can get it to work, I can live without the app drawer, its just that good to me.
I even tried the MIUI360P_ENHv3.1_universal_s enhancer thing but that just mad it so that, I forgot exactly what it was called, something about com.android.phone. (something) process kept having to be force closed but every time I pressed force close within 2 or 3 seconds it did it again & it kept repeating this over & over again, I had to pull the battery just to get out of it, but by what I saw in the forums after I tried it, its actually meant for Samsung Galaxy S II I9000, whereas I have the Samsung Galaxy S I i500, I kind of rushed into it in the hopes that it would fix the issues.

Widget removed and folders emptied

So I just switched from The LTE/HSPA/GSM (auto) network mode to the HSPA/GSM (auto) network mode and rebooted my phone. When it turned back on for some reason it booted into safe mode (I didn't even know phones have safe mode). So I rebooted again and some of my widgets are gone and all the apps I had sorted into folders were removed from the folders and are just loose in the app tray (the empty folders are still there). This is so weird, anyone know whats going on or why this suddenly happened?
what launcher, and did u change anything else at all? Install another app perhaps
Stock Launcher. Installed GO SMS early this morning (don't think it's the problem). This issue happened in the afternoon right after I rebooted after switching network modes. Also the weird thing is that a couple widgets (email and stickies) stayed while all the rest (clock, minimalist text, beautiful widgets) disappeared. Also I use swiftkeys as my keyboard but the phone resorted back to stock keyboard. I haven't gone through everything yet to see what else changed.
stock gb 2.3.6?
Stock GB 2.3.5. Has rogers released 2.3.6? I tried flashing CWM a while back which didn't work and soft bricked me so I had to flash the stock recovery tar from the dev section. My phone says I'm running official firmware though so I don't think thats the problem.

[Q] Notification icons gone after crash

I've got a Samsung Infuse 4G, originally Rogers now on Telus. It's been running ParanoidAndroid 2.53 from scott.hart.bti (build pa_infuse4g-userdebug 4.1.2 JO54K eng.scott.20121104.152548 test-keys) nearly flawlessly for a few weeks. I've installed a few apps (facebook, twitter, wikipedia, imdb, accuweather, etc.) but none recently.
Today, I answered a phone call from my brother, and when I took the phone away from my ear to hang up, it stayed black. I heard the disconnect sound, then it pretty much froze. I powered off to the Power Reset / Debug Mode screen, then rebooted. When it came back on, I'd lost a bunch of my settings, but not all. I currently have been unable to get back my full menu when I long-press the power button - it only has power off and reboot, no airplane mode, no volume, no profile choice (yes, I have configured these options in the settings). Further, my notifications work (ie, sound and vibrate), but the notification icon does not appear.
Anyone have any ideas what I should try? I could reflash and start over, but I'd rather not if I don't have to.

Can anyone comment on whether or not this make be a problem with Trebuchet? Are the problems I mentioned the domain of the launcher, or is it something else? If these things are controlled by the launcher, am I able to reinstall / reset it to possibly fix the problem?
It sounds more like a SystemUI problem than a launcher problem. You can reflash the ROM without losing any data or settings by formatting just the cache partition and /system and also clearing dalvik cache. That will reinstall the ROM without data loss but if the problem persists I recommend a full and complete wipe and reflash.
If you believe it to be a trebucket problem though then download a free launcher replacement like Holo launcher from the store and see if that fixes your problems.

[Q] Stock rooted reverts back to default keyboard

I have two issues, both of which are very frustrating. I have changed my default keyboard to Smart Keyboard Pro and it keeps the change for a day or two and then it reverts back to the Samsung keyboard. It is not tied to restarts, and I cannot figure out what causes it t suddenly change back.
The second issue is the call log shows everything (including messages) even though I tell it just show "all calls". The issues is kind of the same where I change it and it stays that way for a few days and then reverts back but the tow changes don't even take place together.
Any ideas?

Sounds weird, have you installed a mod recently that may have messed it up? Sometimes doing things to your phone can cause these little issues.
I'd restore a recent backup if you have it, you can try to factory reset also but you'll lose your app and system data.
If that still doesn't work, I'd pull off any files you want to keep and format the internal SD card. It'll return your phone to a very clean state, but you'll also start from scratch.
I also think flashing a custom rom should clear things up. If you find any you're interested in give them a shot. cleanrom, bonestock, and obsessed all are extremely stable roms and as close to stock as you'll get. There are also many other roms that are more heavily modded and themed that you'd enjoy. I think you'd probably like them more than a stock rooted rom anyway.

Help - Can't change screen grid size after factory reset

What's up everyone, this is my first time posting here -- I've come across a strange and annoying problem and I'm hoping the knowledgeable folks here can help me.
In short, after installing the 5.1.1 update, my phone seemed to lag a little on menus and such, so I did a factory reset. Everything was fine, except now after the reset, for some strange reason I can't change the screen grid size in the TouchWiz panel customization menu. The button that says "screen grid" is there, but it's now greyed out and I can't access it. I did some Googling and found that the newest version of TouchWiz (that is stock installed on S6's) is TouchWiz Nature UX 5.0; I checked "TouchWiz Home" app info under my applications manager and it says "Version 4.0". Could it be that I am running and older version of TouchWiz (because I know the screen grid customization is a feature of the new TouchWiz) -- I don't understand that though, how could a factory reset revert to an older version of TouchWiz than what was stock installed in the first place? I know I could just install a different launcher but I am completely fine with TouchWiz, I just want to be able to change the grid size like I could before the factory reset. I am puzzled and hoping someone can figure this out, it doesn't make any sense. BTW I have a Sprint 32GB S6 with everything unaltered -- non-rooted and everything stock. I would post a screenshot but I can't figure out how to upload an image from file.

Check and see if you have an update settings>about device....
Try going to recovery mode and wipe cache.
I have already tried wiping the cache...
as well as the cache and app data of the TouchWiz Home app itself. If anyone is using the default S6 TouchWiz launcher, what version does it say you're running in the Applications Manager? Mine says "Version 4.0" and I know the version of TouchWiz that comes with the S6 (and the one that includes the screen grid customization feature) is called TouchWiz Nature UX 5.0, so I'm thinking somehow it reverted to an older TouchWiz version (which wouldn't support changing of the screen grid)? That wouldn't make sense though, because why would a factory reset revert to an older version of TouchWiz than what was stock installed in the first place? Very puzzling.
