Can we connect in ptp or what ever options there is? I only ever seem to get mtp and clicking on the notification of it doesn't do anything. On my Nexus it gives me options for the connection
Nothing else for now.
Only media transfer mode, no mass transfer.

I cannot get my phone to enter usb mass storage mode. I am running cm7 7.03. I plug my phone in and it just charges, doesnt give me the option for mass storage like it used to.

Mine doesn't auto ask, I have to drop the menu from the top of the phone and select it. Can you select it there?
Never used CM so I don't no that you can. But every rom I have ever used had the option there.
Do you have USB debugging mode on?

Yes I have tried it with it enabled and disabled. When it is plugged in to the pc it charges it, but nothing else.
My guess is you had USB debugging mode enabled then you made a selection to charge when use USB connected to the computer and you might have accidently checked to do this everytime so you wouldn't get the mass storage button when the USB is connected

Whenever I try to connect to my pc to transfer into the phone it always has device cannot start in the device manager. Multiple computers do this. Also on my mac with the android transfer program it doesnt work neither. Doesnt even detect the phone. I can put it into camera mode and transfer into it that way but its a pain. Anybody have any ideas? I was told to fix permissions but that didnt solve the issue.

When I connect my N5 via usb, either using "USB debugging" or "as MTP device", I don't get all the stuff that I should in /sdcard/.

Also, when I connect via MTP, it always re-creates the default android folders like Podcasts/Playlists/Notifications/Alarms etc. Any way to stop that?
Anyone? Is there any way to fix or 'refresh' this list somehow?

I've seen many solutions for the reverse, but not for this problem.
I've got my Nexus 5 connected to my PC right now. I'm trying to copy files off of it, but the Nexus doesn't realize it's connected to the PC. With Android debugging on, Windows realizes there's an "Unknown USB Device" connected, but Android doesn't recognize the computer is connected at all. I've tried the other computer in our house and had the same issue.
I'm running on 4.4.2, so it shouldn't be something too out there, but I can't figure out what else to do.
On the other PC, did you use a different cable?

Using a different cable I was able to get the Tethering options to show up. Activating Tethering, the Nexus 5 was recognized as a "USB Composite Device". Trying to manually specify the USB driver to use hasn't changed anything.
With the different cable, deactivate tethering then delete the device in device manager. See if anything else shows up. Could be cable issue, could be phone port issue

Got it working. At some point I unchecked both the MTP and PTP connection options, but for whatever reason plugging the phone back into the computer didn't make the notification pop up. I had to go to Settings -> Storage -> (menu) USB computer connection in order to turn it back on.
Sorry for the trouble.
As title said, when I connect my phone via USB to my PC, I can't see the storage of my Moto G. Am I missing something?
I have the exact same issue
After connecting the phone via USB to the computer, unlock it and look for the persistent notification that says something about the USB mode. Then change it to "File Transfer" mode.

Really? Does anyone search? This has been asked an answered dozens of times.
Disconnect USB. Go to Settings, Apps, and Show System Apps, then find External Storage and Media Storage and clear data and cache on each one and reboot. Give it 5 min after full boot up to rebuild media databases and connect to USB and grab the pull-down menu and change USB mode to MPT.
