Method 3 (CONFIRMED WORKING on XT1028):
As performed in this video (Russian, but language doesn't matter guys)
You can get past the activation call by following these steps:
On the first screen "Setup Wizard" appears on top, press START.
On the second screen is the Verizon Wireless Activation screen.
Press "Activate" in the lower right hand corner, then press "Cancel" in the lower left hand corner, and repeat this process quickly over and over until the screen freezes.
When the screen freezes, you will see the navigation bar back button in the lower left of the screen.
Press it, and WiFi Settings should pop up.
NOTE:
Before I took the OTA upgrade to 5.1, I was on 4.4.4 and had previously bypassed the activation screen, had entered by Google account info, and the phone was on airplane mode.
After the OTA upgrade, I did not need to bypass the activation again in 5.1. Apparently, you only need to use the above method after resetting the phone to factory settings, or if you didn't bypass before the OTA. I am using this phone as a wifi-only device.
I have the VZW pre-pay variant of the Moto G. I tried "method 3" and got to the point of freezing the screen but the back button didn't show up. However, by chance I accidentally hit the power button and turned the screen off. When I turned it back on, the frozen screen was still up for about a second and then the wifi settings window popped up.

I have searched in Nexus S forums but don't think there is anything that may help me in this situation.
My phone is ICS 4.0.3 Stock, OTA Update done in December last year.
Someone tried too many incorrect unlock patterns on the phone and it is now asking for Google username and password. The problem is that it won't accept the password. The same password works on the computer for Gmail etc. so password is not the issue.
I found out that it is apparently a bug. (Sorry can't paste links as I am a new user.)

The problem is that we are not able to do a factory reset on the phone. After booting the phone by Volume up + power, we select recovery and press the power button, a Triangle with exclamation mark with an android appears and then the phone reboots normally. We have tried multiple times to press the Volume up + power button again at this stage but the phone reboots normally.
I can't follow the sticky thread on nexus s forum to unlock bootloader because it involves putting the phone in USB debugging mode by going to settings which I can't do.
Is there another way to hard reset the phone without unlocking the bootloader?
Some key combination, don't know exactly what, brings up the unlock bootloader screen. Like this

Have you tried bypassing the login screen as thee second "Best answer" describes in the google support link you pasted above?

Bypass the Login and get back to your home screen!
This might sound silly or a prank to make you into doing stupid thing. But it is the main ‘cure’ that I have found.
Step 1) Ask a friend to call you. Answer it.
Step 2) Flip open and close the screen while pressing the ‘Home’ button repeated.
Step 3) You will get to your home screen! Surprise~!
Step 4) IMPORTANT! Go to <Settings> -> <Security & location> -> Uncheck <Require pattern> (Now you can get to home screen by flipping your screen)Clear your Google Apps cache!
Step 1) Go to your <Settings> -> <Applications> -> <Manage applications> -> <Goggle Apps>
Step 2) Hit the <Clear data> button!Access Android Market!

How would I flip open the home screen on Nexus S.. I think the instructions are for G1. The post is dated 16th November 2009.

I haven't yet but will.
Theoretically, when getting a call, you should be able to access your home screen using one of the capacitive buttons or the dialer button on the in-call screen. I haven't tried if this works when using pattern unlock though :/
Also, when in the bootloader and seeing the triangle with the exclamation mark, getting the actual options menu is a chore indeed, it messed up a lot for me too when I first messed around with it.
Try pressing the power button and while keeping it pressed tap the volume up button while on that screen. It might (should?) activate the menu you're looking for. For me, the problem was usually trying to do it too fast or too slowly :/
It's been a while but if I remember this correctly... when you are at the triangle with the exclamation point... try pressing the Home or Menu buttons and it should bring up some text in the upper left corner... you can then cycle through the options using the arrows.
(I could be totally wrong here but give it a try)

Is there anything I can do for a T-Mobile ZTE V768 gingerbread phone that doesn't enter recovery mode(it freezes) or download mode(it reboots),
and can't start because it has a pattern and it is locked for T-Mobile? The phone was sent to a friend here in Honduras from the USA. I have really tried to help this guy but have not met my goal yet.... Any ideas would be totally useful....

A couple of weeks ago, the iPhone world discovered an exploit (not fixed) that allowed anyone to bypass the lockscreen and access the phone, messages, and even pictures.
Well, the bug has been caught in the GS3 world now, too. A few days ago, mobile enthusiast Terence Eden discovered a flaw that also allowed limited access to certain features of your Samsung Galaxy S3, and only in very certain circumstances. And it works no matter what protection you have enabled...Pattern Lock, PIN, Password, or Face Unlock.
Steps to Exploit #1
Lock your phone and turn the screen back on.
Go to Emergency Call.
Select the Emergency Contact icon on the bottom left.
When in the Emergency Contact screen, hit the Home button.
You will see a flash of your Home Screen (no matter what launcher you are using).
In that second when the Home Screen flashes, you can select an app/widget to execute.
The limitations with this exploit are that almost anything you select will run in the background, and you will be back at the lock screen. Where this exploit can be effective is if, let's say, you have a Direct Dial widget on your homescreen. In this case, someone can hit this widget, and the call will go through.
While this is something that should be fixed, it doesn't actually allow you to do much, so really, it's not all that scary. Unfortunately, the fun doesn't end there.
Yesterday, Sean McMillan of Full Disclosure opened up the initial exploit and discovered something much scarier. If successful, not only will this exploit open up the full contents and capabilities of your S3, but it will disable the lock screen completely until the phone is rebooted.
Steps to Exploit #2
Lock your phone and turn the screen back on.
Go to Emergency Call.
Select the Emergency Contact icon on the bottom left.
When in the Emergency Contact screen, hit the Home button.
Immediately after hitting Home, press the Power button.
If you did this correctly, the next time you press Power, your device will go directly to your homescreen.
This is obviously not good. Sean does note that you may need to do this multiple times to get it to work. Also, it doesn't matter what launcher you are using, or whether you are using a lockscreen replacement or not.
In the interest of full disclosure, I tried about 30 times, both with my rooted/modded phone, and with a bone-dry stock phone, and I couldn't replicate it.
But, just because I couldn't do it, doesn't mean it isn't real and dangerous. At this point, there has not been any word out of Samsung regarding this exploit, but I imagine a response and a patch will be on their way shortly.

DISCLAIMER: I do not guarantee that this method will work for you, but it doesn't require ROOT or a custom recovery, etc. So, it's safe.
So, three days after OTA updating my OPO to XNPH38R, I randomly got the "unfortunately, CM Lockscreen has stopped working" error, and my phone became virtually unusable. I was devastated! I didn't want to wipe my phone. So, I decided to just got into Recovery and wipe the cache. But my custom recovery was freezing, so I decided to boot up my computer and try to re-flash recovery... And while I was waiting for my computer, I stumbled upon this work-around. I hope it helps someone.
WORK-AROUND METHOD:
Step 1.) Reboot your phone. (You'll have to hard reset it by holding the power button down for 5+ seconds.)
Step 2.) After the phone restarts and you're faced with the persistent error of death again, just press the power button once to turn off the screen. Then, turn the screen back on.
Step 3.) When you turn the screen back on in Step 2, the screen should be totally black with the capacitive buttons still lit. If this doesn't happen, try repeating Step 2 until it does.
Step 4.) Just leave the phone on with a blank screen and lit capacitive buttons for 20+ seconds. This seems to be an important step.
Step 5. Turn your phone screen off and back on again. For me, this fixed the persistent error. Yay! Hopefully it works for you. However, for me, the error came back again in only 5 minutes, and I had to repeat the entire procedure. Which is why we need Step 6.
Step 6.) Disable the lockscreen. This can be done by going to System Settings > Lockscreen > Screen Security > Screen Lock, and then choose "none" to disable the lockscreen entirely. This has kept the persistent error from recurring for me for 12+ hours.
And, that's it! That's all. You get to keep your data and information. And you get to keep the latest OnePlus One software version... However, unfortunately, you lose all lockscreen Security. But, you could maybe try installing a third-party lockscreen like "GO LOCKER" until OnePlus recognizes this problem and fixes it in the next update.

This is not working for me as i cant get my phone to the stage where the screen is black with only the buttons lighting up
Alternative solution
I too faced the same problem with the lockscreen after changing my wallpaper.
However the method mentioned above did not work for me.
I tried this instead.
1. Restart your phone by holding down the power button for a few seconds.
2. Turn off the screen.
3. Use the play music gesture: swipe down 2 fingers to start the music player. (tried the camera gesture but did not work).
The lockscreen problem disappeared since then. I have also turned off the lockscreen in the settings in fear of it reoccurring.
Hope this helps any of you suffering from the same problem.

I found another way to get your phone unlocked
So the solution that was provided by Marvelite did not work for me. I tried many many MANY times, but to no avail.
After looking around the interweb, I found some other instructions to root my phone (FYI I had no experience in rooting) and I was able to fix it without flashing or re installing anything.
So... what did I do. All I did was unlock my phone. I got my phone and remember saying it was unlocked, as you could use any carrier. That is not what I mean by unlock. What I mean by unlock is you cannot flash it or erase it. See the command line window below saying the phone is not unlocked and that is why I cannot flash it. I thought I had to flash it or erase it to get everything working. Just unlocking it fixed the CM Lockscreen Has Stopped Working error.
So what will you need to install on your computer and do to your phone.
1) Download adb-setup- 1.3.exe (if you type "adb-setup- 1.3.exe" into google, you will find a download) and install ADB system wide on your computer.
2) Put your phone in Fastboot mode and plug it into your computer. To do this, have your phone completely off and then hold the volume up and power button. You will see the screen of your one plus one in fastboot mode.
3) open up a command line window (as in my attached picture) and type fastboot oem unlock
THIS WILL RESET YOUR PHONE! After I typed in fastboot oem unlock, my phone reset itself and it was though I had hit the factory reset.
You can see from the command line window attachment, that I tried to flash my phone based on this workflow (http://forum.xda-developers.com/oneplus-one/help/fix-brickloop-audio-fx-fc-efs-corrupt-t2879061), but I did not even need to do this, because my phone was not unlocked. Then when I unlocked my phone, the problem was fixed.
I hope this works for those that have this horrible bug.

Interesting problem I've been working on, my friend's sister has a tablet (did some research, 99% sure it's the Ubox A13-MID) that has a pattern lock. She forgot the pattern, and also doesn't know what the main gmail attached to it is. I've attempted several unlock patterns, no 'forgot password?' prompt comes up. The tablet also doesn't appear to have a fastboot or recovery menu... pressing power+vol up, power+vol down, and power+vol up+down doesn't do anything, no matter how long I hold the buttons, no matter if I let the power button go after a couple seconds, nothing happens. She's not tech savvy, so I know USB debugging isn't enabled. The tablet has an SD slot, and I'm not certain if it supports OTG. But even with these considered as possible ways of restoring the tablet (by any means possible, including root), I don't see any way of doing anything with them without any system menus.
By the way, I hope this isn't a violation to ask this (any kid could come on here and ask how to unlock a stolen device, lol), so please forgive me if it is. If it would be of any aid, I have two rooted androids of my own, both support OTG. Just throwing that out there in case either of them may be able to aid in unlocking this crapplet.
Safe mode doesn't help, and launching the 'power off/airplane mode/sound' window DOES reveal the back button, home button, and screenshot button along the bottom of the device's screen, as well as the option to increase/decrease volume (I was hoping there would be the tiny chance of a settings icon there to bypass the screen). Been swiping up to see if I could launch something to bypass it, as a bug. I can take screenshots this way, but that seems to be all.
I was able to bug out the tablet by dragging the clock to the trashcan, the 3 dot icon came up on the bar but when I clicked it, the tablet just powered off
When it came back on, the clock was back, and the same cycle occurred when I did it again.
Is this a lost cause?

This thread should be totally in-bounds, as should all future "FRP bypassing as a hobby" threads I intend to do as the opportunities arise, for I will be the legal, legitimate owner of the devices I bypass. You see, I've been doing this since the "beginning", and have always been fascinated on how varied the FRP methods have been from the different manufacturers.. So anyhow, as I get new devices, I intentionally FRP lock them, then figure out a way around it. So yea, todays exercise is the Galaxy S8 Active (ATT - SM-G892A). I recently scored one for silly dirt cheap, and drove it like I stole it heh..
I made the mistake straight away of updating the software to the December 2018 rom, which as it turns out has pretty much all of the vulns patched (with the exception of ther HushSMS trick..) so I kinda shot myself in the foot right out of the gate. What one then has to do is downgrade from the G892AUCU3BRL1 rom to an earlier bootloader three rom. I managed to find JUST the AP tar from G892AUCS3BRG1. This allowed the exploitation of a emergency call trick, which kicks off the steps needed to bypass FRP.
So, from bit3 December G892AUCU3BRL1, you flash G892AUCS3BRG1 AP. Upon completion either manually drop to recovery mode (Vol+, Bixby, Power) and wipe\reset, or just let it boot and it will figure it out Then, the steps are as follows:
1) Connect to wifi.. Internet WILL be required
2) Enable Voice Assistant. Press two fingers down on screen and hold till it enables.
3) On "Start" screen, click Emergency Call
4) On dialer, enter 112 then dial
5) New screen pops up. Click the "+ add call" button
6) New screen appears. Click Contacts, Then Create Contact
7) Give contact a name (for saving purposes) then click view more down towards bottom of form
8) In the werb address field, enter a URL you want as a jump point, then click SAVE.
9) New screen appears with newly saved contact info. Click globe next to the URL you entered in step 8.
10) Presto! Your half way there. I'm sure you can figure out the rest
