OPO root dissapearing after cm12 update - ONE Q&A, Help & Troubleshooting

im sure this question has been answered for different handsets etc before but i wanted to ask here.
I decided today to root my oneplus one. I had installed the CM12 update before hand and thought that it would not cause any issues. unfortunately after i installed the custom recovery i tried TWRP and clockwork mod and then rooted it. when i booted into the cm12 the root binary was not present and the recovery had reverted to the stock cyanogen recovery.
eventually after many attempts i decided to wipe the phone entirely and install cm11s onto the phone and wouldnt you know, the root and recovery all worked perfectly. i even installed titanium backup just to make sure. everything checked out 100%.
Then I proceeded to install cm12s again, thinking all was good but then it all reverted again. I really do not know what I am doing wrong. I am using the root toolbox that was shown on the root video in the root tab of this forum.
Before you ask, yes i have unlocked the bootloader. i followed the video to the word
I really do appreciate all help and advice given

[Q] Possible to root 4.1.1?

My main question is are there currently any working methods to root my phone from 4.1.1
BackStory:
I rooted my phone about 4 months ago successfully. about a month ago i started getting update notifications from VZW to update to 4.1.1 Which would have been great since i was looking forward to jellybean.
Unfortunately their update kept failing. so i decided this morning to use Synergy and just run a custom rom. Followed all the instructions and everything went very smoothly while playing around with my new rom i decided to reboot to test the speed of the boot time. upon reboot im greeted by the VZW soft brick screen.
So i then followed the instructions listed here: http://forum.xda-developers.com/showthread.php?t=1840030
To restore my phone to factory default. which was fine i don't really mind losing my data nothing that isn't backed up on Google.
I decided to fully update the phone and then try to re-install the synergy rom but i cant seem to find any detailed threads on the subject.. Help?

I think i actually locked it after i finished the install. And then restarted, do softbricks occur when you re-lock your bootloader?
Also another question..is there any way to check the /system /data partitions from inside the phone (terminal app maybe?) are correctly set to r/w over r/o (i don't like unplanned installs )
Soft bricks always occur on a locked bootloader. There are file managers that can do that like solid explorer.

Just had this happen to me to lol, spent the last three hours fixing everything and lost all my music because I didnt have a nandroid backup . Anyways just use this guide and revert back to stock rom http://forum.xda-developers.com/showthread.php?t=2432476. You'll lose root and I think I've also lost recovery but its better than just have a $600 brick.

[Q] random shutdown and bootloop

Hello First post here so hopefully in the right place! I've got a nexus 5, UK model purchased around January time. I had rooted, running stock with SuperSU installed and a few apps installed with xposed mod.
It was running perfectly fine, until a few days back where I was required to reboot, it then got stuck at the Google screen and wouldn't go past that, I turned the phone off for a while and eventually could get into it.. Great, until it happened again.. I spoke to a friend who said perhaps it was the xposed which was causing it, so I flashed back to factory, ran it for a while and it was fine, so went back to root and tested it, was stable so I reinstalled xposed.. It then got stuck in the bootloop again, so flashed to factory certain that it was xposed causing issues. I'm now running factory, however the phone will still get stuck in bootloop but will also randomly reboot itself now.. Has anyone experienced this? After scanning the forums I can't find anything so hopefully someone can help!! My boot loader is still unlocked.
Thank you, Jordan.

Are you able to flash a custom recovery? like TWRP, PHILZ, CWM? try it.
if successful, flash a custom Rom. just to discard the possibility of emmc failure.

I am able to flash a custom recovery and so forth, so wouldn't think it to be emmc failure.

It doesn't really exonerates emmc from being bad yet. now, wipe and flash a Rom, a stock/aosp based one

[Q] Recovery Disappears After Reboot

Hello!
Im not really new to rooting and custom roms. Had the Nexus 4 until about a year ago, so been away from Android for about a year! Got the Nexus 5 a couple weeks ago, but been too busy to get around to doing anything with it. So, well, now its time! Ive unlocked the bootloader, and installed ClockworkMod Recovery (Only cuz thats what I used to use, and I like it! The installation goes fine, and the recovery works fine! So, I restart my phone so I can see if the phone is working fine before I install a rom. Everythings fine! So, I resart and go into recovery and that's where the problem is! When I go into recovery, it shows a picture of a Android under service. Well, I did some research, and from what I can tell it looks like there is no custom recovery on the phone. I repeated this a couple times, same problem! can anyone help me out? Someone here mustve faced this problem too... or maybe Im just screwing up somewhere and I dont realize it.

It happens with stock lollipop and maybe some aosp ones too. Flash latest SuperSU from recovery or delete recovery-from-boot.p (I think in system folder)

Fastboot boot cwmrecovery.img
Flash supersu in recovery
Then
Fastboot flash recovery cwmrecovery.img
Btw, I recommend you use twrp

+1 for TWRP, CWM is practically ancient.
Live boot TWRP, flash SuperSU, then use Flashify (root) to install TWRP.

This. This is the key for maintaining your custom recovery after a reboot.

Removing my custom rom from SM-G930U

So i tried to root my phone through a video, I got SuperSU to flash and everything but my phone wasn't "rooted" (according to root checker) and also SuperSU wouldn't update itself. So there goes that, so i decide to factory reset my phone, even though it's brand new, but then i realize i still have the custom bootloader. Well crap, i forgot i already loaded in a custom rom even though it was factory reset. So my question is, how do i get rid of my "root" all together, should i just reinstall the official firmware?

Yes, by installing the stock firmware the "failed root" and the custom bootloader will be gone.

Well, if you restore it via smart switch, then your data will also be lost. But at least you'll have a stock rom.
Hey I have the same exact problem, I downloaded a custom rom, the root failed and it left my phone really buggy and slow and it overheats. I have tried multiple times to flash the stock firmware onto my phone but I have had no luck. Can you explain to me how you were able to get the stock rom back onto your phone step by step? Thanks, this would make my day and as a matter of fact, my next two years cuz I just got this phone in the spring and I'm already having issues but I can't get a new phone for another two years.
What custom rom are you used ?
