[Q] My OPO is stuck in recovery - ONE Q&A, Help & Troubleshooting

My phone was working fine till this morning when I had some weird phantom touch issues. So I thought to simply reboot my phone and see if that helps.
When I rebooted... it went straight into TWRP Recovery (2.8.0.1).
I tried System Reboot from there. Straight back into TWRP.
I tried restoring an old backup and rebooting. Straight back into TWRP.
I tried factory reset, wiping everything manually, installing new roms, installing old roms (all of which have worked before)... everything just booted into recovery instead of system rebooting.
I tried powering off and booting into Fastboot by holding down Power + Vol Up buttons. The phone can turn off successfully, but it will not enter Fastboot. I just get a black screen after the 1+ square logo.
Whatever I do, my phone seems to be stuck in recovery. What can I do to fix it? Thank you!

[Q] SOLVED Problem flashing CyanogenMod 9.1.0 Stable

I rooted my MT4GS a few months ago but stuck with the stock ROM. I decided I wanted to try out the CyanogenMod 9.1 ICS ROM. I followed the following steps:
1) Installed ROM Manager.
2) ROM Manager indicated a newer version of ClockworkMod Recovery and I installed it (5.0.2.7).
3) Using ROM Manager, did a full backup of the current ROM. All went well.
4) Using ROM Manager, downloaded and installed CyanogenMod 9.1.0 stable-doubleshot. I specified to wipe the data and cache partitions. The installation seemed to go normally.
When the phone rebooted, it was stuck at the logo screen. Had to remove the battery to turn it off. Tried a couple times, still the same.
Since I had the backup, I decided I would use CWM to restore the old ROM.
When I boot into recovery mode, it shows CWM 5.0.2.7, but whatever option I try to choose from the recovery menu, The screen just goes blank with the CWM logo in the middle and nothing else happens. Hitting the power button returns me to the menu, but no option works, even power down or reboot. It just goes back to the CWM logo again and I have to remove the battery to shut it down.
So, I thought I would try using fastboot to flash the ROM again. When I boot the phone into the boot loader and select fastboot, then plug the phone in via USB, nothing happens. Windows does not see the phone and ADB or fastboot don't see it either. It's not a driver issue, as the computer is still setup from when the computer was rooted and I connect the phone all the time with no problems.
When the phone is off and plugged into USB or the charger, I still get the charging light, though it goes off after about 10 seconds, then comes back on and cycles like that.
Is there anything I can try at this point, or is my phone bricked? It seems like I followed all the steps correctly. Is there an update.zip I can put in the root folder and see if it picks it up?
Any help would be appreciated.

Ok, after rebooting Windows, I was able to get the USB connection to the phone to work in fastboot. It seems I missed the step described in this thread:
http://forum.xda-developers.com/showthread.php?t=1508556
which states with S-ON you must flash the boot.img separately. I did that via fastboot and the phone boots into cyanogenmod now.
However, I still seem to have the issue with clockwordmod recovery. As long as I do the recovery functions from rom manager, everything is fine, but when I manually boot into the recovery menu, I get the same behavior of any choice from the menu just blanks the screen except for the CWM logo and nothing happens. I tried reflashing the recovery image using fastboot, but still no go.

This is solved also. Seems on the MT4GS, the trackball is the selection button. The power button appears to only be a back button.
What a thread! Two nicely phrased problem definitions, two well explained solutions, and all from the same guy. Sorry we weren't able to help directly, but you obviously found the answers you needed elsewhere in the forum. And you updated the title as well to show it was solved. Thanks.

[Q] Rooted, bad recovery, and stuck in bootloop

I scoured the forums, but have not been able to find out what I can do to solve my problem. So here it is.
I rooted my phone (to play around with some settings, what have you) and flashed TWRP recovery onto it. I got an error msg when booting up, so I decided to push an original recovery back onto the phone, because it wouldn't go to TWRP. I followed a guide (which I can't find now) to push back a stock version of recovery and it failed. My phone would boot normally just fine, but when I tried to go to recovery it would loop.
The OTA update came today, and dumbass me pressed continue (not thinking) and now I am stuck in a boot loop, and cannot get out of it.
Cannot get into recovery.
Cannot get into download mode.
Got any ideas?
Thanks
Same Problem
I have pretty much the same problem. Please help!
well without recovery/download mode working or the phone being in fastboot mode there really isn't much that can be done, you can always try to claim on warranty
boot loop
So, let me get this straight. If it failed when you tried to install the stock recovery, then you still have the TWRP recovery right?
I ran into a boot loop problem earlier when I tried upgrading TWRP. I just couldn't access my recovery so here is what I tried:
Code:
Turn your phone off.
Press volume down and power button until the LG logo comes on screen
Let go of both buttons
Hold the volume down and power buttons again until you see the "hard reset screen"
Press power once and then power again to confirm. The screen says it will reset your device but it will not as long as you have a custom recovery installed.
Your custom recovery should appear on screen.
Try the steps above and see if you can get into a recovery. Once you do maybe try resetting. Hope that helps.

OnePlus One suddenly wont boot.

Hi.
My OnePlus One wont boot up anymore.
I was playing dead trigger 2, when suddenly the phone froze (first time the it happend)
I restarted it with holding down the power button.
After restart, it hung on CM11 boot screen, not boot loop, but let it load for 10min, still at CM11 boot screen.
Restart again with power button, but this time it wont boot past OnePlus One boot screen, after the first boot screen, it is a never ending boot loop.
I was able to get to recovery (TWRP), tried to reinstall ROM (CM11 nightly 04.10.14), still boot loop after OnePlus One boot screen.
Tried to wipe date, cache, dalvik, and reinstall, still boot loop.
I am able to get to fastboot.
So what is the next thing to do?
Try to flash back to stock? How can I do this without format internal storage?
Use "adb pull" to extract any files you want to recover.
Then flash back to stock. It will automatically wipe everything.

Well, I got the 64GB version, and last time I checked, I had 4GB free.
Can I pull whole titanium backup folder?

data/wipe and then advanced and wipe system as well. then reflash rom and gapps. do this after you backup all your stuff but you should not try and restore with TB after you get it back up and running. Just my opinion

I tried literally everything..
I was on multi rom and using Mahdi and CM11s(Internal).
Then i stuck in black screen that my phone wont wake up but LED was blinking. Did a reboot holding power button.
After that when I selected the internal rom it just gave a vibration and came to CM boot logo. Now I tried reflashing 3-4 roms doing whole wipe but no change. Then I flashed Stock with toolkit and it says successful but no booting..
Totally worried.. Please someone help here.. :crying:

you flashed stock with toolkit? first off leave toolkit alone.... Did you flash the .img files? or you flashed a zip file to get back to stock?
I know you are upset but you say you have tried everything. Did you try this thread http://forum.xda-developers.com/oneplus-one/help/fix-brickloop-audio-fx-fc-efs-corrupt-t2879061
Edit: Also there's no crying in rooting!
So, I just used fastboot to restore it back to stock, and it is STILL bootlooping after OnePlus One screen.
http://forum.xda-developers.com/oneplus-one/help/fix-brickloop-audio-fx-fc-efs-corrupt-t2879061
Downloaded files from this site, and extraced the XNPH33R zip file, and run the flash.all file.
It says completed after writing everything, no error. But still wont boot up:/
So I tried installing every file manually following this guide: http://www.androidheadlines.com/page/2. No luck
The same problem here. I started facebook, It rebooted, 10 mins of CM logo, power off - neverending bootloop.
Reinstalled all via fastboot, ... still in bootloop.
RMA ?

BACKUP FIRST!
I have the same issue! Suddenly the screen froze. He did not respond to any button, either physical or capacitive. It was well for several minutes (20). Finally I opted for a "hard reset". I turned it on again and this time the screen froze while loading Cyanogen (Hexagon logo). Never happened there. 30 minutes later, I opted for a new "hard reset". Since then, never goes to the home screen (1 + logo and "powered by android"), reseteándose again and again, over and over again without end. Bootlooping :crying::crying:
I have tried everything to get it back, enter in the "recovery", delete data, cache, factory reset ... nothing makes then pass the initial screen.
I even managed to flash with CWM recovery and ADB one posteriomente whole mod, nothing works...

[EMERGENCY] Bricked Nexus 5 Cant Boot Into Recovery Mode

Hi there, having just the old casual phone crisis. I was running cyanogenmod 12.1 for awhile and decided to finally upgrade to a nougat 7.1 build.
I tried using this one https://forum.xda-developers.com/google-nexus-5/development/rom-dark-rom-t3492150 using adb sideload as I had tried a earlier one and the gapps messed up so I wasn't able to put twrp back on the phone. The rom said it had installed successfully but I wasn't able to sideload gapps, I kept getting an error status 255. I went ahead and just tried to load the rom without gapps and see if I could install it slowly through APKs. I managed to get past the initial android load and the services started up, and it got in an endless crash loop because the phone service wouldn't start. So I went to try and flash another rom and was going to delete the cache. When I went to delete the cache the phone hung for about an hour so I tried to just reset it into recovery mode again.
Now when I hold the power button and volume down key to into fastboot mode I can. I have access to the initial screen with the four options, but when I try to start recovery mode or start the phone my screen goes black, and it'll flash the dead android symbol with the error saying unable to mount cache, it only flashes it for like half a second then goes black and repeats every 10-15 seconds.
I tried holding power down, doing volume up, and letting go to do a hard factory reset but it does nothing. I also can't seem to access the adb sideloading anymore, I'm very scared that it's hard bricked, is there anything I can do at all to fix this.
Flash a new version of the recovery via fastboot. Then do a full wipe. After that install the ROM only! Reboot recovery (not system but recovery) then reflash the ROM (no wipe) and add everything else. Hope this helps you out.
The phone may not boot the ROM without wiping data if installing a 7.1 ROM over a 5.1.1 ROM.

TWRP keeps crashing

I unlocked the bootloader on my phone, then flashed twrp. I rebooted to twrp and installed boot.img, TWRPWorkingInstaller.zip, TWRP-BLUE-FIX.zip, and Root.zip. Rebooting worked fine, but I saw that there was a newer twrp (twrp-3.2.1-3-wOOSkernel.img, so I tried installing it from twrp. The problem is that I installed it to the system image partition. After that, everything got wonky, unsurprisingly. Now, if I try to boot to twrp, it loads and I get to the first menu, but I cannot interact with it at all. Then the screen goes black and the led lights up blue. It doesn't respond to anything except shutting it down. I can hold the power button down to power off, but it reboots instead to a black screen with the blue LED. I can also boot into the bootloader. I can reflash twrp. When I restart to recovery then, I get the screen asking if I want to keep the system files locked or swipe to unlock it. The device seems to work fine for about 5 seconds from the time I get to the first twrp menu, then it doesn't respond. About 5-8 seconds later, the screen goes black and the blue led comes on. I also cannot power-off the device. If I try, it reboots.
Is there anything I can do to recover from this? I've got the stock recovery files (OnePlus6Oxygen_22_OTA_009_all_1805172226_f7d1518e0e704ca3.zip), but I can't copy them to the device because it won't boot to the OS.

Do you have a TWRP backup from when your phone worked on an external drive?
Can you access fastboot? Just use the fastboot ROM files to reflash the OS. Or if that doesn't work try the unbrick tool.
Yeah. I can use fastboot. However, I wasn't able to get to ROM to flash. I'm not sure if I'm using the correct parameters in the command. I googled, but all I came up with was flashing custom ROMs. Where can I find the unbrick tool?
NM. Found the unbrick tool. I may try it. I'll let you know how it works.
Thanks for your help @NateDev!. The Unbrick Tool helped me fix it.
