That means its modified. You can only accept OTA if your /system is unchanged
Although OTA is pointless if rooted anyway
Just download the full rooted stock ROM in android development and flash it without wipe from recovery.
It's actually fewer steps anyway.
OTA = revert system changes and/or kernel, reboot into recovery, flash the OTA , boot to bootloader, connect to pc, fastboot flash recovery, reboot into recovery, flash supersu.
Rooted stock = reboot into recovery, flash the ROM.
No brainer really. Flashing an OTA if rooted is a fool's errand.

How to upgrade a Stock+Root+Custom_Recovery I9505G from 4.3 to 4.4 and keep all Data

If you have a Rooted+Custom recovery Galaxy S4 I9505G with 4.3 and you want to upgrade to 4.4 while keeping all your data untouched and restoring root +custom recovery, here is what you need to do.
You can't upgrade with OTA. root is ok but a custom recovery changes the partitions and will cause the upgrade to fail
To resolve this:
1. Download stock recovery and install it using ODIN
2. Download the OTA update to your device and use the stock recovery to upgrade (or use sideload from adb)
3. You have 4.4 and all your data.
4. You now don't have root so download SuperSU
5. You still don't have root so download Clockwork recovery for I9505
6. Fastboot boot the clockwork img
7. Flash the SuperSU
8. Reboot
9. Download SuperSU from the PlayStore
10 You have root!
11. Downlaod twrp openrecovery for I9505G
12. Enter download mode
13. Use odin to flash your new Recovery
14. Reboot
15. Enjoy your new 4.4 Stock+root+recoveryGalaxy S4 Google Edition + all your data !
i only picked this up fro this forum and used other users methods combined
so i can't post any links
you can pick up all the files needed at this Forum (use search)

You can skip half those steps and flash supersu with twrp at the end instead.

Can you update TWRP from TWRP?

I currently have 2.3.1 installed on my note. I want to update to 2.6.3 so I can install CM11.
I only have TWRP installed and my current ROM (some verzion of PA) has already been factory reset (aka I can not actually use my ROM/Phone).
In order to update to CM11 I need to use the latest TWRP.
Edit: When I try to install the .zip for 2.6 from 2.3 it gives me an "error" message
Yes, you can. But after flashing the update, you must reboot into recovery (restart TWRP) before flashing anything else.

How to flash OTA update on rooted phone with custom recovery?

Trying to flash 5.0 OTA from 4.4.4 without losing data on Nexus 5.
I don't know how to remove custom recovery so I can sideload the OTA zip in the stock recovery. It keeps loading into TWRP instead.
Can someone explain step by step how I flash the factory recovery.img?
That's not even possible (as far as I know)...

Removing a custom recovery while preserving data is not possible, you should backup all your data, remove the custom recovery, flash 5.0 and then restore your custom recovery.
Are you using the Nexus Root Toolkit? It's incredibly easy and I use it for almost all my flashing, rooting, etc.
You can't apply an OTA on a rooted phone. On Kitkat and before you could end up in a boot loop. After Lollipop the OTA changed to system-level verification, and any change to the ROM stopped the OTA - all you get is a reboot to an unchanged system.
In any case, major ROM jumps usually work best with a total wipe and clean install of the new ROM. somerandomatoms is right - back up and install the new ROM. But why go to 5? You may as well go straight to 6.0.1.

[Q] Update to Lollipop without USB and with root

Hi, my N5 has a problem, it has the USB port broken so i can't connect it to the PC. It now has KitKat 4.4.4.
Here is my problem:
I want to update to Lollipop but i can't do it with Factory image and/or Sideload because the USB problem.
I tried to "unroot" but i couldn't do it right, i've downloaded the 5.0 OTA and my phone rebooted with a red triangle and it said "error" i know this is because i still keep my phone rooted.
So i would like to know 2 things:
1) How do i unroot definitely? because i did it before but it seems it didn't do right, i clicked "unroot" on SuperSU (i read that sometimes when you press "unroot" it doesn't erase all the folders and files so you have to do by your own, but what files and what folders are they?)
2) If i unroot and i if update to Lollipop, can i be root without using the USB port?
Of you're rooted, why would you want the stock ota anyway? There are PLENTY of 5.0 Roms in the development section with custom kernels (custom kernels are needed in 5.0 lollipop for root). Flash one of those in recovery. Make sure you have the latest TWRP which you can flash with flashify if you can't fastboot flash using USB.

Is really needed to flash a custom kernel to get root on Lollipop? I'm asking because in this thread [ROM] [STOCK] Android 5.0 LRX21O - Stock/Flashable (11/12/14) FAQ nº 2 it says:
Q: How do I get root?
A: If you want to keep the stock kernel, use CF-Auto-Root. If you flash a custom kernel, just flash the newest SuperSU ZIP afterwards.

So i understand that we can have root with stock kernel, is this ok or did i misunderstood something?
If i want to flash a custom kernel, where can i get "SuperSU" Zip to get root? and does it need to clean something after or before flashing this file? caché, data, etc?
Yes CF AUTO ROOT will allow your to root the stock image. However since youre already rooted youre running into issues flashing it successfully. I attached the SU zip below. My recomendation is if you want the stock ota image, download it from the developer thread, flash francos custim kernel for lollipop ans then flash the root zip i have attached below. I would complete a full wipe of system, data, and both caches (not internal storage or you will lose all on sd card) before flashing. Once you wipe just flash the the rom, kernel and root zip in that order.

Thanks!!!
It worked perfectly!
I tried first with CF AutoRoot but it failed, but it worked with SuperSU.

[Q] Need help with stock recovery on OnePlus

I rooted my OnePlus with TWRP recovery sometime back and current image is CM11. I need to upgrade it to CM12 however all the manuals I found require stock recovery to upgrade from CM11 to CM12
I was wondering if it's possible to upgrade to CM12 with TWRP recovery? If not, how I can get stock recovery restored?
Also, if I manage to upgrade to CM12, is there a possibility for OTA updates in future?
Dud download lollipop bootanimation and flash through TWRP

1. Download the flashable CM12s zip file.
2. Store it under the root directory.
3. Go to TWRP recovery (Please make sure you use the latest version 2.8.6.0).
4. Do factory reset (wipe dalvic cache and other memory)
5. Flash the CM12 zip.
6. Reboot and enjoy....
Also, you will get OTA if you manually flash the CM12s.

You don't need stock recovery, just flash with TWRP.

Thanks for your help. Was able to flash CM12 successfully.
But facing another issue now - As soon as phone booted with CM12, it downloaded OTA system update. When I try to install the update, phone reboots in twrp recovery and update doesn't complete. Tried numerous times (with and without 'update CM recovery' toggle) but with same result.

Some md5 error. Was able to workaround it by flashing the OTA update via TWRP directly

Good to hear that.... MD5 error occurs when the downloaded file is corrupted.
