I updated using the OTA update and the rear camera option is now gone completely from my phone. It looks like the camera doesn't even see the rear camera.
I have now rooted it and uloaded The latest Dirty Unicorns 4.4.3 rom thinking it was the OTA update, and looks like that wasnt the cause.
I have tried multiple apps, and they just crash immediately. Tried different roms, different Gapps, as well as wiped everything multiple times. I am worried that the OTA update has somehow killed my camera.
Any thoughts?

when you say wiped everything, did you wipe system and userdata?
did you restore any apps or anything with TiBu?

I wiped everything i could find on the phone. lol
I used the recovery and the factory reset to wipe it all.
Wiped data and cache in the recovery. The only thing i didnt wipe was the Dalvik, as there was no option to do so.
I did restore the apps through the google play store, but i also tried uninstalling the app and re installing it.
I have not wiped everything, and updated manually. I could try that I suppose. I dont see how that would make a difference, as restoring the apps comes from the Google play store doesn't it?

I just tried resetting EVERYTHING. reformatted it all, as well as the cache, data, dalvik, and factory reset.
Nothing brings back my rear camera on the phone. I am going to look for a 4.4.2 stock rom and see if that brings it back. If that doesn't bring it back. Its obviously a hardware issue.
Flash the 4.4.3 stock images from Google.

I switch the camera to the panoramic or sphere camera setting, and the phone gives the error that the phone does not have a rear camera. I am so stumped its not even funny. I am taking it into T-Mobile tomorrow.
I will update if I get a resolution.

I had wiped it multiple times, unrooted, flashed stock recovery, got the icon to reappear, and the phone still didn't see the rear camera. It was gone. The camera app finally started giving me the error that the rear camera didn't exist.
So, after speaking with T-mobile and speaking with Google (neither had a clue) they set me up with an hardware exchange.
I got the new phone, updated without any issues, and works as intended.
If anyone comes up with the same issue. Save yourself some time and effort. Get an exchange on the unit if its still under warranty.

Give us more history - have you done anything major with your phone recently?
For example, cruft remaining in /sdcard after a return to stock using the "ultimate unbrick" approach can cause the market to go nuts unless you do a factory wipe in Settings.
did a factory reset that seemed to have fixed it, the lousy serendipity rom screwed up the phone royally

There are a handful of people running that rom without any issue...
unfortunately you failed to provide any real details of what was going on, so it was impossible to give you any help
I have had some issues with google authentication...it usually happens when one fails to wipe everything before installing a new rom or by restoring system data with tb.
Typically it can be solved by clearing cache for google framework services , rebooting and then logging into youtube.
So, by factory reset...you are still on Serendipity then right? A factory reset deletes user data, not the rom.
I am currently running Serendipity, no Market issues.
I wanted to post this in the BAKED thread in the developer section but I don't have enough poststo do it there, so hopefully someone here can help.
I just flashed crespo BlackBean 7 (BAKED) ROM (on my Nexus S) and inverted gapps. It seems to have installed properly but whenever I reboot I get a message that several processes have stopped such as YouTube and GMail, and when I go in to system settings -> storage I get a message saying "Unfortunately, Settings has stopped". I get that message every time consistently. I've tried re-flashing the ROM but to no avail. When I installed the ROM I wiped "Dalvik cache", wiped "Battery Stats" and wiped "Cache partition".
It appears I've not done something or done something wrong, so if anyone here can help I'd appreciate it.
Did you full wipe before installing the rom? That means factory reset/data wipe.

I did not do a factory wipe because the last time I did that I lost everything, even after using Titanium backup to back up all my apps first. When I tried to restore there was nothing there to restore. Is there a way to use Titanium backup so that after doing a full wipe I can still get my stuff back?

So I bit the bullet and did a full factory wipe, but I still get the "Unfortunately settings has stopped" error showing up. I wonder if it's a bug in the ROM.

Storage settings fc is a bug in the ROM. It will be fixed in the next version. You should not get fc for any other settings. Back to kill is faster than normal. Everything else works fine on bb7.

I saw the back to kill thing, someone else had mentioned it on the BAKED thread too. Thanks for the info on the fc.
I got another problem though, that made me revert to my previous ROM. I was about to put my phone away for the night and decided to do one more reboot, but it never went past the BAKED splash screen. Even after pulling the battery it wouldn't boot. Is this a known bug too? Or did something happen to my machine? After booting to recovery and re-installing my previous ROM, everything seems to be fine.
Any thoughts?

There isn't any boot issue with the ROM. If you decide to try this ROM again, try to do a clean install.
Boot into recovery, do a factory wipe, go to advanced & wipe dalvik cache, go to mounts & format /systems. Flash ROM & gapps, reboot.

I flashed Moar a couple months ago. Also added custom recovery. Just yesterday the infamous "warning camera failed" came on. I want to take my phone to sprint so I need to flash it back to stock. Please help!

Before you unroot, and take your device back to Sprint. Have you tried booting into Recovery, and clearing caches ? Did you do a clean install of MOAR ?
Also, just an FYI. Your post should've been made in Q/A [emoji6].

Yes I did clear cache, I was just a bit confused since I installed philz recovery I don't know if I need to uninstall that's or just unroot?

Had you installed any other tweaks, mods, or Xposed Modules prior to the issue ? Using Philz Recovery should have nothing to do with the camera issue. I would suggest force stopping the camera, and clear data in Applications manager. Then immediately boot into recovery , clear caches, and reboot. After the device fully boots, check to see if the camera is working correctly. My next step after that would be a fresh install ( Full wipe ) . Then let it settle in for a few hours without installing any apps. Check the camera to see if it's operating correctly. If it is install your apps one at a time checking the camera afterward to make sure that nothing interferes with the operation of the camera.
The last resort if the issue just does not get resolved would be to ODIN back to Stock. If the issue persists, then take the device in for service.

Idid a full wipe....then did a full wipe and installed the new version of MOAR and I am still having the same problem *sigh*
how would I go back to stock do I need to upload a different recovery, the changes I've made are philz and Moar, or do I just simply unroot and put stock firmware?
So I dirty flashed the new build of mahdi over my old build. Newest gapps as well. I rather flash dirty cause of all the settings I have them the way I like them.
For some reason it doesnt let me sign in to the google play store at all. Tried reverting to the old gapps as well and nothing seems to work. Let me know if anyone has any idea besides doing a clean flash.

Thanks for the quick response. I could have sworn I tried that. I cleared a couple of the apps cache. Including the google play store cache. Anything else you might suggest I try?

Sorry Let me re answer that, i cleared the cache, data, and force closed the app.
I also went back into recovery, formatted system, cleared the cache, cleared the dalvik cache , flashed the rom once more, then flashed the latest slim gapps.
Try clearing data on play store, Google services framework and Google play services. Reboot and see what happens.
If not, try different gapps. And you've done everything wipes wise, except wiping data it seems. A clean install is wiping system, data, cache and dalvik. ?
The problem exists mostly because of an update to google play services. I was somehow lucky in that I managed to do a full wipe and for some reason (even though it shouldnt) custom nandroid restore of Data only the problem went away,
There are various methods of trying to fix this without a wipe but they haven't worked for me and many others. A clean flash is the best way really.

I have since then fixed the problem, as nothing worked like I wanted it to. A clean flash did the trick, losing all the settings and notes I had in my notepad apps. Should have backed those up before clean wiping my whole device. Thanks anyways guys.
I flashed the 5.0 factory image (just the bootloader and system parts, franco.Kernel and superSu, did a Factory Reset). When I try to access my data via the Photos app or ES explorer, I can't see anything. However, when I tried to send a pic via whatsapp, I could see my entire gallery there, as though the information still exists on the device.... This is especially weird because I was using an AOSP built Lollipop ROM for a couple of days and haven't encountered any issues whatsoever.
How can I solve this problem?
Same crap happened to me. I think something happened when dalvik was converted to art. It took me a while to figure it out but clean flash back to a dalvik 4.4.4 rom, I used dirty unicorns. Afterwards clean flash lollipop. Good luck.
