Hi,
I followed the sticky guide from the general section - I'm able flash roms, and cwm, root, etc.
However
Although I have adequate signal - i'm able to use 3g data - I can't make or recieve calls or texts.
Everything seems to be working right up until dialling a number - when the phone immediately hangs up.
I've spent quite a few hours over the last couple of days looking round XDA for similar problems - and the issue (phone immediately hangs up but data is working) seems relatively common - but no joy yet - except for trying a new rom, toggling data on and off, and a new motherboard.
I've tried several roms, including an out-of-region stock, and formatted everything except the sdcard, and nothing is solving the issue.
I'm guessing I've mashed up something to do with the radio- as I think it's not starting - but I don't understand how flashing a new rom doesn't replace the corrupted files and resolve the issue.
is there a way of formatting absolutely everything and then reinstalling everything from scratch?
Will a phone repair shop on the high street be able to flash something I can't flash over usb?
Further details: opening call forwarding settings gives a "Call settings error - network or sim error" message
likewise "Caller ID" etc.
The IMEI is displayed correctly, but the phone does not show my phone number.

Do you have to be on T-Mobile network for OTA update/upgrade to work or what?
Sorry to tell you this but u have missed the update. I don't know no other way to get it but your not missing nothing. Hell I'm not even using the stock launcher so yea.

For the OTA, yes, you would have needed to be on T-Mobile. However, there are pure stock 1.63.531.2 as well as "tweaked/optimised/debloated" versions of that ROM now available here to download and install, if you're rooted.
I am running the MikTouch 0.7 ROM, which is an optimized version of the stock 1.63.531.2 ROM. It is fast, clean, everything works as it is supposed to work without any weirdness.

I can't get "MikTouch" or any other ROMs because I haven't "rooted" my phone yet. I never do anything if I don't fully understand "what it does", and since nobody have answered my questions here:

and being new to Android and to this phone - I only did what I did understand... so my phone is unlocked but not rooted yet (and still with s-on).
What I did to get OTA update:
- I removed my SIM card
- I removed my microSD card
- Shutdown the phone.
Then I charged my phone and removed battery for few seconds when it was full, and inserted the battery and microSD card back (also I moved everything off the card and formatted on my computer before I inserted it back).
- Entered hboot menu (volume down + power button)
- Selected Factory Reset
After phone rebooted and started up it asked me to setup gmail account- I wanted to follow this "setup wizard", but without SIM card there was no data connection and it seems impossible to finish it without logging-in to gmail first (? it seems that way?) so I just cancel it and I setup my home wi-fi connection, and once I saw wi-fi working I restarted the phone.
"Setup wizard" started again after reboot and this time I finish setting up my gmail account.
- Then I had notification about HTC update available. I run it, and after it rebooted itself 2 times (maybe 3 times, I didn't stare at it all the time) it wiped out everything (I think) because it started with "setup wizard" again, so I knew it must have updated the ROM. I went to my phone's About Phone -> Software number and sure it is version "1.63.531.2.710RD" now
I have hard reset it again ("factory reset") and am using it since morning.
There are few changes I have noticed so far.
edit: Main difference I have noticed so far: "Visual Voicemail" somewhat partially work with my provider (Wind does NOt have visual voicemail). By partially I mean that now when I have new voicemail I also have another notification as if it came from the person who left the voicemail, saying for example "(number) I have left you a voicemail (length of the message). Press 00021 to listen to the message or go to voicemail" or something like this. When I touched the clickable "00021" it went straight to this message in my voicemail - nice! - even though Wind Mobile doesn't have Visual Voicemail, so I guess it is some general GSM networks option I didn't knew about. Anyways, with previous software I never had such message or notification, phone simply showed usual Voicemail icon and notification only.

If you are "unlocked", you could easily flash CWM Recovery and other ROMS, it just requires an extra step since you are still S-On (flashing the boot.img through fastboot). Also, being unlocked means you can also easily root with just a simple flash. So, you could flash the MikTouch ROM and enjoy the same 1.63.531.2 level of service but in a more optimized and tweaked manner, that is also rooted.
But, if you're happy the way you are, then, "if it isn't broken, don't fix it".
Yes the MikTouch seems like the one I would love to try. I always prefer "minimal ROMs", because if I like the ROM I usually keep it forever and I don't want any obsolete app's "junk bytes" taking space inside the ROM image.
The stock 1.45.x and 1.65.x ROMs seem actually usable, but even though T-Mobile have crapped them with much less garbage (than what I usually saw in their Windows Mobile ROMs), there are still many "apps" I would like to get rid of. So yes, I would want to root my phone at least for "debloating" purpose.

Since you are S-On Unlocked, you could flash CWM Recovery (version 5.0.2.7). That gives you much more capability, like doing a nandroid backup/restore. You would do that using flashboot. Obtain the correct recovery.img, do fastboot flash recovery recovery.img and then you're ready. From then on you can boot into HBOOT, choose Recovery and then flash ROMs or other packages, including the SU (for rooting). The Miktouch ROM is already rooted, so all you would need is to flash CWM, then the ROM. Since it is still Sense, as soon as I signed in with Google, it automatically synced all my apps and contacts, which is a nice bonus.
I got this phone a month ago and flashed various stock based ROMs with no trouble. I then tried AOSP (cm10.1, eclipse) and everything seemed to be ok although, I couldn't make phone calls because I have no service where my house is and was never able to test (I remember that in settings->about phone->system status it said it wasn't activated and it didn't display my phone number, but when I went back to stock it did display my number). Then I went back to a stock ROM and my phone would not make or receive calls. It would send and receive text and was able to received data via 4g but when trying to make a call, it would say "dialing" and then eventually just stop without making any type of connection. when calling the phone it would go straight to voice mail as if it was off. I used odin to go entirely stock and had the same problem. I ended up returning the phone and getting a new one and I am running Clean rom 5.5.1 and everything is working fine but I'd really like to try other roms but I don't want to have the same problem again. anyone know if it was just a bad phone or if I did something wrong? if so, what? any ideas would be greatly appreciated! Thanks!
You should have consulted xda before returning the phone. With your original phone, you needed to reprovision your sim to properly have your data return since you had Odin flashed. That code only works on ICS though.
The loss of data on your prior phone was from flashing CM10.1, you just needed to follow this post to resolve your issue without Odin flashing a stock rom.
Edit: The sim reprovision code suggestion is still valid BUT disregard the data loss part...
So one day, my Nexus S randomly decided not to have a data connection with my carrier (Wind Mobile) anymore.
Wifi works, mobile data does not.
Steps I did to try and fix this:
1) make sure APN address is correct, which it is.
2) turn airplane mode on and off
3) toggle carrier connection on and off
4) factory wipe phone and reinstall ROM (slimkat)
5) factory wipe phone and install a different ROM (AOSP)
6) flash new radio (KB3 and KB1)
Other things I tried:
1) tried my SIM card with another phone (Blackberry Curve), data connection seems to work fine on that phone.
2) Called Wind Mobile and asked them to reset their towers to my phone... all they did was remove the Blackberry security thingy
that I had with them when I was still using a Blackberry. Rebooted my Nexus S, tried again... still no data.
So... I'm at my wit's end, and I have no clue how to fix this. Any ideas? I've found a couple similar threads for this problem... non of which had a solution to mine.
I remembered I made a backup nandroid a little while ago... and I restored it.
Voila, everything works again. But as soon as I tried reflashing to my preferred ROM (Slimkat), the problems started up again.
I also forgot to add that before I started encountering problems, my phone froze and I had reset by doing a battery pull because it wouldn't respond to any key presses. I'm guessing it corrupted something, though I'm not sure what. I've diagnosed that this is not a hardware issue, and that it's purely software... I'm not sure why the problem comes back when I reflash to another ROM. I'll keep you guys posted.
One other thing worth noting is that when I have Slimkat installed, it doesn't seem to read off my SIM card properly.
The APN doesn't get named "Wind DATA" like it usually does when I install a different ROM on there; it just gets named "Internet".
I've been trying to fix my sisters SGS2 Skyrocket all day today to no avail. Ive searched high and low for a solution to my problem.
Basically my problem is that my sisters cell phone will not connect to the ATT mobile network.
Current phone status:

Service State: Out of Service

When I try to make phone calls it states "Emergency Calls Only"
I can sometimes recieve SMS messages around 45 minutes after they have been sent (which is pretty weird that I get them at all)
It is not a SIM card error, I even went today and got a new SIM card from ATT just to be sure.
I tried calling ATT they did some stuff to it, not sure what. At one point after a fresh restart I was able to make a phone call
and it went threw, hanged up waited a few seconds and tried again and it would say "Dialing" but would not reach an outside line, and the call would end before ever connecting. the ATT tech person said they would put my phone on some kind of list for their tech pros to look at and would get back to me within 5 days. I do not have much faith in that.
I have tried changing the APN settings, tried factory resetting, tried installing CWM recovery and flashing a new modem.
When WIFI is off, the only icons I see are the signal bars with 1 or 2 bars in it, No data connection. Earlier when they were messing with it
4gLTE popped up, and appeared to have a connection but the browser did not work, and I could not make a phone call/send texts.
If anyone has any new suggestions that I can try, im all ears! Thanks for reading!

remove sim(eliminates sim card issues)
remove external sdcard
backup internal sdcard then format(fat32) it
then perform a full wipe: read q.26
copy ucmc1 radio onto sdcard and flash it twice w/out cable plugged in
then use odin to flash jb stock firmware x2.
if still having issues after doing this then its some kind of hw failure, what hw failure idk, could be most likely emmc (memory) failure
This also ended in failure.

most likely but not definitive - either sim card tray or antennae problem, search with those key words for possible fixes
I received an OTA Update in early to mid May. I am now forgetting which version I received at that time. ( I forget as I have done so many downgrades/upgrades/factory resets in my troubleshooting the last month!) Up until these issues, I've never rooted or flashed a ROM, or unlocked boot loader or anything. My N5 had always been completely stock.
First Issue
It all started with just the sim card. I get "no sim card" message, even though it was in there. This was my original SIM card and I had never ejected it since I bought it new. I found I could turn it off, pop it out and back in, and boot it again. Sometimes it will see the sim card again. It may take 20 times, but finally work.
I naturally got a new sim card, as well as confirmed said sim cards worked in other phones. My wifi and bluetooth were working fine at first. Working with google support, they recommended a factory reset to fix the sim card issue. Immediately after the factory reset, my bluetooth would no longer turn on nor show a MAC address. At this point, my wifi was fine.
Second Issue
In addition to the sim card slot issue, now my Bluetooth no longer works! Around this time, I received another OTA update, this time it was 5.1.1 LMY48B. I had hoped it would fix the issues, but no dice! I start reading up on the XDA forums as I am determined to start rolling back to see if this will fix anything. I unlocked the boot loaded, rooted, etc following the threads on XDA
Third Issue
I do all sorts of flashing to hammerhead-krt16m_4.4 to hammerhead-ktu84p_4.4.4 to hammerhead-lrx21o_5.0 to hammerhead-lmy48b_5.1.1 . On the final flash back to 5.1.1 my wifi now no longer works!!!!!!!!!!! It is stuck at wifi turning on and no MAC address shows. It also seems my cell data is very very slow now!
After many calls to google, I think I finally succeeded in getting a “ticket routed up”. My phone is from the play store around 10/31/2013. They keep pushing I am out of warranty. I attempt to ask for a refurb N5 without luck so far. I am not very confident anything will come from google. This being my only handset now, it is just not an option to send this to LG. My current state is my voice/txt and cell data are working. Data seems a little slow but usable. Wifi and bluetooth won't turn on. I also still have the sim car slot issue upon rebooting. After a bunch of reboots it will eventually see sim card.
What else can I try? Is there a better way to attempt to repair/reload my radio drivers? Perhaps there is corruption of sorts? I have not tried a custom ROM or radio or kernel…… I did take to a local tech. They looked at everything literally under a microscope and did not see any issues that way. I am in the USA and have a lgd820. Although we are quick to assume a hardware issue, I am wondering if there is some sort of incompatibility with the first run of N5 and lollipop….. who knows!

This link was actually what i was going to suggest. It seems you've tried pretty much everything in the book except that.
I think its worth a try. Same as any other learning process, take your time and read the whole thing once or twice until you feel comfortable with the process. Then go for it.
Darn it, I should have downloaded those programs BEFORE rooting my phone again. Now I a struggloing to get my SIM card detected again to get data to DL them! whooops!
How would I go about attempting to read my files first to see if they are accessible and potentially write down my old MACs first?
EDIT: I found this but not sure how to get that working? I have Windows 7 and CMD prompt at this point. what program is that using?

OK, I went through those steps. everything appeared to go through correctly. Still no wifi nor bluetooth
EDIT: I rebooted a second time. WHen it first boot up, wifi was on and connected to my works AP. I quickly went in and snagged a screen shot of MAC address. Oddly enough, it did not match what I had input via hex editor. Within a few moments the wifi stopped working. It is back to the turning wifi on issue..... bluetooth still not working as well.

EDIT2: I get access denied when attempting to read either files within their corresponding folders under persist.
I was able to copy my stock files into my sdcard download folder and read them there without issue. Thus seeing my default addresses for both!
Perhaps it is normal, but I am never able to edit the old or new files under persist. I basically renamed original folders to bluetoothold and wifiold. I could delete them perhaps.
Note that I put my phone into airplane mode before doing anything noted in the steps.
Any other ideas? Given that it is showing my original MAC address the one time it worked, this makes me think that it is not reading the files .macaddr at all
I have the same problem. Funny thing is, the phone is just over 2 years old and the motherboard was replaced just 9 months ago (so, brand new one).
In my case, I received and Android update 2 days before I found the phone burning hot and restarting on it's own. Then the same chain of events you describe started to unleash. Now I can't be sure if this is software related, hardware related or both.
Can't seem to find a solution to the issue either.

I'm facing the same issues and I tried all the possible steps I could find on internet trying to repair the problem.
Actually, since the latest updates I was encountering some issues with the WiFi only: not turning on either off and not showing properly the MAC addres. So I decided to factory reset, even SDcard and since yesterday I have tried all the possibile guide I found to internet to fix the MAC address not showing and the WiFi not turning on/off but nothing worked. I tried even to flash again the old factory images, such as KitKat, Lollipop, etc., to lock the bootloader again and to dismantle the phone in order to clean the motherboard, but nothing worked. Phone still gives me MAC address 02:00:00:00:00:00 alternatively with the correct one, I can't figure out on what it depends because sometime it seems to work: it shows the correct MAC, it connects to wifi network; suddenly it doesn't!
I'm really going crazy to understand what's wrong.
You'll have to dig up my final posts on this , but in a nutshell, send into LG for repair, select cheapest option they offer. Receive mint phone. My cost was $45 and about one month... I picked up a cheap $20 phone to get me by.
In other news.... I now have a 6p!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!
