First, welcome in the forum.
We have a quest answer forum , please look before make a posting
You need to do a full wipe in recovery ( factory reset ) , nothing more , you can save your apps and app settings with titanium backup and can restore them after flash cm. Make sure you don't restore system apps , only user apps + data are no problem.

Do it the exact same way you used to put Cataclysm on your device.
Welcome to XDA. I ain't a pro, but probs a factory wipe from your recovery and a flash of Cyanogen mod will do. Backup all ur apps with Titanium Backup beforehand and all that stuff, coz it is a factory reset so everything will be wiped except the stuff on ur internal storage. Remember to make a full backup of ur phone on TWRP beforehand, coz something can still go wrong. Side note, do NOT use any toolboxes, they are sh*t. Good luck

Download the cm 11 ROM for ur nexus 5 and copy it to ur internal storage. Boot into the custom recovery and perform a factory reset and flash the cm11 zip and the gapps package
Before that make sure u have a back of the existing rom.

Why u wanna use Cyanogen mod anyway? Cataclysm's tonnes better. What can Cyanogen do that Cataclysm can't? Personally I've been using Cataclysm since I got the phone, and I ain't looking back. Good luck!

I recently rooted my phone by using chainfire's CF-AUTO-ROOT and it has stock recovery on it. Now, I want to install a custom recovery, specifically cwm recovery. How will be able to do this? Do I have to unroot then root again? a step by step info will be appreciated. thanks
First of all find a cwm file for ur device specific to the model u have on google. Flash it with Odin n u will have the recovery.

done finding cwm file that's compatible with my tab
Flashing? not really that well with android terms yet. after clicking "pda" then selecting the cwm file, should I click start? is that what you meant by flashing?
I appreciate your help.
Yeah first go to download mode holding power and volume down button and select the file in the PDA section n just click on start. And wait till pass. appears on the Odin screen. That's it.

I also would like to install cm 10.2. is it stable? I tried searching and I found pac rom and if i'm not mistaken 3 custom roms are installed into 1 rom.
I also would like to ask if it is really necessary to wipe all data before installing a new rom? and if so, would apps like titanium backup pro would be able to backup every single app or information that is in my tab/
Yeah u have to wipe cache and dalvik cache. 10.2 nighties r stable. No issues with me.

Your thread is changing direction but I can't resist commenting anyway. If your tablet is rooted, stock I don't think you will see big benefits to a custom ROM. For other tablets I use like the Kindle, Nook and N7 custom roms can make a huge difference. I'm running CM 10.? Or variants of that on those devices. My most used tablet is this 2.7 with stock ROM. If you must try another ROM, I'd suggest a sick based ROM that is deodexed with sound or visual enhancements. Also, changing to a noon, Touchwiz launcher will make big differences. GOOD LUCK!
P.S. quess what a ROMaholic might be.

don't format system after flashing ddlf2.
directly install 10.2 or any custom rom over it.
procedure =
1) flash ddlf2
2) root it
3) go to recovery and directly install 10.2 or any rom you like.
4) wipe data/reset. (optional)
(don't format anything or else it will give you
that error)
hope this helps you.
thanks
very thanks people, i did it exactly as you recomended, but still i got the same error its my first phone what i can not flash...very sad But thanks for your advice.
Are the Roms that you're trying to flash based on 4.3?
If yes in CWM recovery go to "mounts and storage" and mount all that is unmounted.
Than flash the Rom.
It worked for me.
You can even try to redownload the Rom file, it can be corrupted.
Cheers
Lol, people still use this phone?

Amazing! worked perfectly thanks for quick reply !! woot!
just so you know as well... root is independent of ROMs. you can have custom ROMs without root.
And also its not rooting that wipes a device, its unlocking the bootloader

find a rooted stock rom in the android development section and flash it(bigxie makes a good stock/root rom). or you can just flash the factory img from google via fastboot.
Theres another stock ROM linked in the resources section of the sticky thread in my signature

Hi
Stock 5.1.1 LMY48B
rooted with TWRP installed.
Can I flash to CM12.1 just by flashing the zip from TWRP ?
will I lose any data ? Do I need to worry about bootloader or radio versions ?
If I want to go back is it as simple as flashing my nandroid backup ?
Thanks

As I know going from rom to rom you need to make a factory reset.
That will remove all what is stored on /data
All apps, cache and dalvik cache.
All other data insidr /storage/sdcard will be on safe
As I know stock rom cannot be restored, but I just take this device before 1 day. I don't know for that, because on mine previous phone that was impossible

CM nightlies include a radio and bootloader so you don't have to worry about that.

No they don't.
But since he's upgraded to stock 5.1.x already, everything is up to date and there's nothing to worry about. Besides, Google's bootloaders usually have good forward- and backward-compatibility.

From Jira:
Me => CM (AOSP) is the only rom which has a bootloader and a radio into its zip. Why? Could you remove them?
I was on the newest bootloader HHz12f and a radio from KRT16M. Every time I flash CM nightlies I have to update my bootloader and flash an older radio. Very annoying.
Thank you.
CM Team => Sorry, we will continue to ship both. It saves us more headaches in the long run. Thanks for your understanding.
It's as simple as that.

Custom ROMs have a check into their update-script for the bootloader version. If you don't have the good one then you can't flash it.
