[Q] Nexus 5 Not allowing to install OS, cannot mount to PC, Only bootloader or TWRP - Nexus 5 Q&A, Help & Troubleshooting

I have a Nexus 5 bought In Israel. It came with 4.4.1 and I updated to 4.4.2 and I wanted a change so I installed PA ROM beta 4.
Now from here all hell broke loss...
For some reason my vibration motor stopped working completely and from time to time it gave me this weird vibration for maybe 2 hours and then disappeared again... I'm not blaming this on the ROM.
Yesterday I got an update that PA ROM beta 5 was out so I installed it through their OTA app.After the update I had no keyboard, my volume buttons did not respond well so I thought maybe the update was a bit off, So I did a Factory reset. after the reset the OS was still shaky and laggy so I wanted to go back to stock... I found a Stock odexed ROM, got into TWRP wiped everything except sd, and went on to install the new ROM.
(I did not make a backup).
Now my problem Is that I could not install the stock ROM, every time i tried to flash it it would stop and not install...
Now i'm stuck in TWRP without the ability to mount my phone to try and change the ROM... when I reboot it says I have no OS...
I tried working with the Nexus 5 toolkit and install stock from there and also nothing helps...
I tried installing the Stock from Google manually with fastboot commands and still no answer from the phone...
Please does anyone have an idea for me?

Then you have to flash the stock rom and start over, here is u tutorial http://forum.xda-developers.com/showthread.php?t=2513701 to flash the rom. Google rom`s https://developers.google.com/android/nexus/images?hl=nl.

I think you have to format data and the try flashing any rom, this has happened to me 2 times already. Twrp is the culprit i believe

[Q] Verizon S3 locks up every reboot on rooted 4.1.2 > 4.3

I am having a really weird issue I'm hoping someone can help me out with. I was on 4.1.2 Clean ROM which was working great. I wanted to move to 4.3 Clean ROM (8.1 Core Edition). I did the upgrade and then flashed my modem to ML1 both via TWRP.
The ROM setup fine and ran great. However the first time I did a reboot, the phone gets stuck at the Samsung Galaxy S3 logo and never goes anywhere. I left it sit there for 30 minutes and it was hard locked. Only a battery pull would reset it. Through numerous reboots/pulls, the same thing would happen. I tried wiping Dalvik and Cache in TWRP and fixing permissions, to no avail. Reinstalling Clean ROM 8.1 via TWRP without formatting data did not fix the issue either. The only way to get my phone back up and running was a complete wipe and reinstalling from scratch. Then as soon as I reboot the first time, the issue comes back. I've tested this on a clean install - letting the phone go through the initial setup process but not installing or updating any apps - and it still happens.
So I moved on to try AllianceROM build 22, also a 4.3 build and wiped to do a clean install. No matter what options I choose in the setup, the same problem happens. Interestingly in AllianceROM I always get a acore process has failed error on the first boot.
Then I went to download the Knox-free rooted version of stock 4.3 available from Scott's website. I have the same issue with this. Also a wipe with clean install.
It's obvious there is something my phone doesn't like about moving from 4.1.2 to 4.3. In total, I have probably installed three different ROM's about 20 times now after the past couple days.
I've searched about this problem extensively and the only thing I found is a thread for the AT&T S3 that talks about this exact issue, and using Odin to go to stock 4.3 and loading a custom ROM to fix the issue. But I am under the impression that a 100% stock 4.3 will lock my bootloader.
What I plan to do next is use Odin to flash a stock 4.1.2, then go through the process of moving to a custom 4.3 ROM again.
If anyone knows of a tar'd version of 4.3 that won't lock my bootloader, I would be grateful!
If anyone knows of a different way to fix my problem, I would be grateful for that too!
Well I think I can declare victory. My phone is running Clean Rom 8.1 CE and has survived a reboot after every app install for a total of 15 so far.
As it turns out, I think being on TWRP 2.5 and trying to upgrade to Android 4.3 was the issue. Before I went the route of flashing a stock 4.1.2 release, I thought to check if my version of TWRP was the latest, and it was not. I flashed TWRP 2.7, and did a complete wipe and clean install of Clean Rom 8.1 CE, and it's running like a champ.
I know, I know...the install guide on most every ROM says to use the latest version of your recovery - until now I thought that was a "recommendation" or "best practice" and not a hard requirement. Turns out, at least for me, that it is very much a hard requirement. In reading the release notes for TWRP 2.6 and 2.7, they both appear to implement things specifically for Android 4.3 or 4.4.
Hopefully this info helps someone who may be having the same issue.

Having real trouble updating to Lollipop (recovery help?)

Alright so I have a 2013 Moto G, and basically in the past while on KitKat I've rooted my phone, given it custom recovery (TWRP), and messed around with the ROM (changed it to CM, PA and a few others and back). Now I've come to update to Lollipop and of course I need stock recovery to update my phone via OTA, and when it first prompted me to update, I had TWRP and I didn't really think, I just updated, this sent my phone into a bootloop, rebooting straight into TWRP every time I turned my phone on. Luckily I had a backup and I just restored to that and that fixed it. Recently I've been trying to fix the correct stock recovery for my phone so I can update, and I found some MotoTool AIO thing, and that changed my recovery back to KitKat stock (or so it says), now when I go into recovery it LOOKS like stock, can do everything you could normally do with stock recovery, but when I came to update it, about a quarter of the way though the update process, it errors and reboots my phone. Luckily this time my phone recognizes it has errored and just cancels the update instead of sending me into a boorloop. Any help would be greatly appreciated.

Try putting twrp back on and flashing stock Lollipop Rom. http://forum.xda-developers.com/moto-g/development/rom-stock-motorola-lollipop-rom-t3017510 You can also try aosp, cm12.1 or any other lollipop ROM.
The Lollipop OTA is very picky, I believe any modifications to the stock firmware will cause the update to fail. You will need to have a completely stock system for the OTA to install successfully. Find the correct KitKat firmware for your phone and flash it then try upgrading.

Is there a stock firmware "file map" so that we can check manually what is wrong and erase/add needed files ?
Maybe this could be done by checking a flashable stock rom ?

Update to 5.1 success..... erm, well it's all relative
Eventually managed to update my XT1039 to 5.1 via ota.
Started with a fully charged phone and flashed 4.4.4 stock firmware (http://forum.xda-developers.com/moto-g/4g-development/xt1039-moto-g-4g-stock-firmware-kxb21-t2805619).
Once completed, powered up phone and signed into google play then updated the motorola update services app.
DO NOT install a custom recovery or root yet or the update will fail!
Start the update by going into settings/about phone/system updates.
After first boot into 5.1, do a factory reset.
To root once the upgrade is completed, download cf autoroot (http://forum.xda-developers.com/moto-g/4g-development/root-xt1039-cf-auto-root-peregrine-t2942406) and unzip to a folder on your pc. Open a command prompt and change directory to this folder. Powerup handset in bootloader mode (hold vol down while switching on), connect USB and run 'root-windows.bat' from the command prompt.
So far I'm hugely disappointed in the update as I use WiFi tethering alot and it appears to be a massive fail. I know it wasn't 100% in 4.4.4 or CM12 5.1 with both reqiuring an occasional reboot, but in 5.1, something's just not right and it's basically unusable (could it be updated radio or something?). Fortunately, USB tethering appears flawless.
Another obsrvation seems to be abscence of a + appearing in the HSPA signal indicator, I seem to be getting just the H appearing regardless.
Also no more bluetooth visibilty timeout setting.
After installing CWM 6.0.5.0, I got gliches with the display in recovery mode but it works.
Now after I have it rooted, ad free and de-bloated, fixed the power on logo and installed CWM 6.0.5.0, I'll stick with it for a while to just to see how the battery lasts.
In hindsight I should've stuck with CM12 5.1.

Unable to restore/mount/flash in TWRP and unable to achieve dl mode in LG Flashtool

Here's to hoping someone can help me out. My phone details before I messed up: LG G3 D850, rooted, TWRP 3.0, running Fulmics 5.2 for about 1 1/2 weeks
I noticed that I was not seeing the LTE indicator on my phone. I checked the HUB and saw the BootD850-30012016 data patch and figured that I just needed to install. So I flashed it and my phone wouldn't start. I was able to boot into recovery, so I figured I would do a clean install of the ROM so I did a full wipe, accidentally wiping EVERYTHING except for my external SD.
Result was that I can access TWRP but am unable to flash any of my backups or zips from my external SD, or OTG. I can see the files, but don't have the option to "swipe". I also am unable to 'mount' or 'install'.
At this point, I noticed that AT&T has finally released the OTA for Marshmallow, so I figured I would just use LG FlashTool and totally un-root and give the update a try. (BTW, I have used the flash tool many times with no problems/issues on both my phone and my son's D855).
Unfortunately, I am getting the dreaded "unable to change to download mode" after 15 seconds. I have uninstalled and re-installed Flash Tool, the LG Drivers and all files and used every port on both of my computers (Windows 10 btw) but can't get past the download error.
So...after many hours of digging, I took a leap of faith and was able to sideload Fulmics 5.1 successfully and install the 5.2 update from the HUB. I made backup of the ROM but TWRP still won't allow me to "swipe" and install/restore. I reinstalled TWRP using Flashify and it installed version 2.8 but still no go on flashing from recovery.
I realize that I'm lucky to have been able to figure out how to get the phone back using the sideload function in TWRP, but I really do want to go back to stock and it bugs me that I am unable to use my backups.
Sorry for the novel, but I wanted to be sure to include everything I did....if anyone can help me out I would be much obliged. My 11 year old is anxiously waiting on me to figure this out before she gets this phone (I'm moving up to the HTC 10 in a couple of weeks!). She still thinks I'm 'SuperMommy' so if I can fix this I get to keep my swagger!
I was also running fulmics 5.1 and updated to 5.2 in my phone but now i was having pretty good laggs and i finally realised that the update provided by the phone was corruped so i went on the official site and downloaded the same 5.2 OTAs from there and flashed it into TWRP recovery to finally get the latest version 5.3..know that is it always advisable to flash zip file when in recovery mode using TWRP recovery...The big mistake was cleaning also the internal memory of your phone.this caused the malfunction of twrp.
The issue is that most often i have seen cases of lagg in those updating to fulmics 5.2 from the Hub .sometimes in twrp recovery the update files will not successfully wipe and then reboot system with 5.2 installed but with many lagg the file downloaded there may be corrupted and not give you permission to swipe..if you really like fulmics and still update to the latest..Try this it may help you solve your problem:1 reinstall the fulmics 5.1 go to xdadevelopers.com/fulmics follow the link given by xpirit to www.fulmics.com and download the OTA 5.2(file size maybe 46 mb).. 2.reboot your phone now in twrp recovery it should now be able swipe and install..(if not then install Twrp 3.0 and try) 3-search the files in your internal memory or wherever you have downloaded it and flash it ..4.reboot system.That should be done!! In case you want to come back to stock and cant install from Twrp then the problem is with Twrp just download the latest version that goes with your phone.
Thanks guys for the suggestions. I finally figured it out after a couple days of sweating and swearing. Yaneddik, you were spot on about updating from the Hub. I ended up installing TWRP 2.8.6 and re-installed 5.1 from the Fulmics website along with the updates. Everything has been working great (except for losing LTE connectivity, but that's for another thread!). I'm not inclined to update TWRP to 3.0 unless absolutely necessary, 2.8.6 works just fine for me.

Hey there....you can access the Hub after you flash the Fulmics ROM. It's where you can get your tweaks, downloads, mods, etc. You'll find it in your app drawer.

Every Custom ROM crashes at system setup wizard in Verizon Galaxy S3

I have a Verizon Galaxy SCH-I535, it had Android 4.3, but I downgraded it to 4.1.2 creating a custom-stock ROM.
Now I rooted it, unlocked the bootloader, installed TWRP 3.0.0 and tested other recoveries as well, and also flashed several ROMs, all of them correctly but when the ROMs starts, just about to show the FIRST SCREEN of the Setup Wizard, the image crashes and shows blinking colored dots.
At the end the only ROM I can have booted is the 4.1.2 that I saved.
Can anyone tell me how can I do to have a different ROM installed, and thta is correctly booted,thanks.

I have my old S3 running CleanRom and I've for the most part never had an issue with the setup wizard on that device. On others have. Are you clearing the cache and the dalvik after flashing the rom?

But, anyways, that is past now, I lost my mind and flashed the stock 4.3, it hardbricked my phone, I used the unbrick method for 4.4.2 and I don't know how I ended up with the wrong kernel so there is no going back now, I flashed stock 4.4.2, so no downgrade possible now.
I have read about JTAG solution to downgrade, is it possible?
I bought the Octoplus Dongle for Samsung Lite, but I don't think it works for this type of problem.

Did you update the radio/baseband to NE1? No need to utilize a jtag as your flashing custom roms so the device is already bootloader unlocked
Edit: nevermind you updated to the latest stock so as you already know your bootloader is locked

Note 4 Dreadful unusable performance!

Hopefully this is in the right section
I have a Note 4 SM-N910G and its just turned absolutely useless since I factory flashed it. I have rooted it before and bricked it and un bricked it before so have some experience with it. I recently wiped my phone and did a firmware restore using Samsung Kies and since I did the phone is absolutely useless. It started out fine for an hour or two and now it lags by like a minute with any action, even trying to bring up the keyboard when typing. It will reset itself, lock itself, crash, just un useable. I've tried reflashing it again stock a couple times and still the same everytime. It went from perfectly fine to dreadful with the original mentioned restore. I did just recently get an error code of "sbl1 partition doesn't exist" in Boot recovery mode.
Does anyone know if this is fixable and not a hardware issue?
Should I try and just root it again and flash a non-oem rom? (I havent had the time to do it recently)

Hi Guys,
Also I should add that when I was rooted the phone was fine but when i went to unroot through super user the phone was still functioning as though rooted and I couldnt download software updates because my device was "modified".
So thats when i just flashed stock firmware and again now flashing through odin aswell it proceeds fine then the device will say installing updates and then give a no command error or seandroid not enforced. Everytime is basically a different message.
How can I just wipe everything and get back to stock?

So to add to this is there any relationship between the firmware running and the bootloader? I just realised that before i tried to flash stock firmware i was rooted and didnt un root via superuser because in the past that did jack all.
I flashed dr. ketan rom this morning and the phone was operating as normal very snappy but as soon as i downloaded a new app from the play store (reddit and chrome) again it turned to absolute crap.
Is there a disconnect here that i missing that can be fixed?
