Hello all !
I have an issue after trying to change ROM : I had "icon stretching" issue with Source, something apparently no one had seen before. I wanted to switch to Maximus, so I flashed it apparently with no problems. But then the phone only booted to Recovery (TWRP)...
So I tried to flash firmware again, the one given in tuto for Maximus but same problem.
And this is when it got tricky, not only I couldn't boot to Android, but I couldn't mount USB either, the option just disappeared ! Along with issue to mount "/cache" apparently. I tried factory reset, then full wipe, no result.
I tried a more recent version of TWRP than the one given with Maximus' tutorial, with it I have the option to mount USB but only for a 48 MB space ! Again, factory reset and full wipe don't help.
I'm not too worried since I can access bootloader and Recovery fine but I feel out of options... I flashed 2.16 firmware like a dozen times (is it any point in doing that ?), I wonder just how I can get a recovery that can mount USB, or a stock ROM to start fresh.
Anybody to help me ? Thanks in advance !
stick on TWRP 2.3.3.0 which (iirc) is the last one that allows you to mount storage. slap another rom across, flash, fastboot boot.img, get the rom up and running and then consider changing recoveries.

I flashed this version but I can only mount a 48MB storage . I didn't reflash the firmware, should I downgrade to 2.15 ?
i've got no experience with 2.16 firmware so i can't really help you as far as it goes with the best recovery on that... all i can say is the last version of TWRP that allows sd mount is 2.3.3.0. have you tried the latest CWM that works with kitkat roms? that might be worth a shot.

Maybe you should use recovery from maximus thread, it can be specialized for hboot 2.16. AFAIK 2.16 hboot has a different partition system, that could be the problem. Try to wipe your sd with op recomended recovery. You should flash the firmware twice by the way(second one when got the error), i think you already know that

YYYYYEEEEEESSSS !
Finally ! That was the right way to do it, downgrade to Hboot 2.15 with help of http://forum.xda-developers.com/showthread.php?t=2322755 but the recovery wasn't booting, just reflashing TWRP offered in http://forum.xda-developers.com/showthread.php?t=2528113 did the trick.
And then I was able to mount my SD card and follow the steps to flash Maximus.
What a relief. Thank you all for your help ! I'm just surprised not many people seem to have had the same issue... But maybe they manage to resolve it by themselves !

Since yesterday I have the strange problem. Searched the forum but can't find any solution thats right for me.
I testet a sense 3.5 rom (Insertcoin) and then wanted to restore an backup. It went wrong. So I wanted to wipe everything and flash my old rom new.
No I get the error messege when I want to wipe data/factory reset?
What could I do to solve this problem?
I also tried superwipe from the revolution rom, but this doesn't solve my problem :-(
I just want to wipe everything and then flash a new fresh rom.
What I tried so far:
Installed the original Vodafone RUU. Then installed clockworkmod (latest version) then installed root.
Error still occurs.
I got the same problem!

This just happened to me after flashing stock RUU again and installing CWM and CM7.
Should i worry or is it irrelevant?
Also Now in the bootloader it says
LOCKED but s-off on hboot 1.18 should i run Revolutionary again? I tried but it would just close a second after. I did manage to flash CM7

I wiped my System partition, and data partitions and was going to flash the latest AOKP (i had some issues with rohan's kernel so i needed to start fresh) but i can't mount my SD in TWRP and I'm pretty concerned that my built in storage is screwed. Any help ??

go here: http://forum.xda-developers.com/showthread.php?t=1671237 and read before you do anything else. Then.. find your device specific RUU and run it. You'll have to relock your bootloader and reflash TWRP.
Which AOKP were you looking to flash?
Which hboot do you have?
and do you have the international or US version?

I was hoping to avoid doing an RUU, don't RUUs wipe USB Storage?
I have Hboot 1.09
CID 11111111
I had flashed AOKP Milestone 1 JB earlier.
I flashed a few 4.2 JB apps and upon reboot my phone would stick at the boot animation.
So I wiped my data partition and system to reflash Milestone 1 but now i can't flash anything.
Can you get into TWRP? If so, you can adb push.

I did yes. Everything is wiped currently so im sort of in a soft brick. i can boot in and out of recovery but i cannot flash anything. my SD storage wont mount in TWRP
I figured out what i did. i accidentally hit factory reset from Bootloader. and then stopped the reset. had to flash stock recovery and flash twrp again. lost all of my stuff. pretty upset about it. my fault though i guess.

Does windows ask you if you want to format when you mount usb in twrp?
If so format fat32, transfer files over and go.
I don't know why everyone are telling people to RUU when they dont have to
because formatting in windows doesn't always fix it.
newbie mistake
So after stumbling to this thread I realized what happened with my attempt at flashing a rom. I selected Viper 2.6.0 for my newly rooted At&t one x on firmware 2.20. It was stuck on bootloop. I had backed up using TWRP 2.3.1, but the restore was stuck on a bootloop as well.
I realize now that this was because I was flashing an international rom. Is this recoverable or is this the last mistake I can make on this phone?
Any advice or links to the proper thread would be very much appreciated.

I'm having the "E: Can't mount /sdcard/ problem in CWM recovery v5.8.3.1. I followed the guide to install but I find it misleading. I came from Leedroid so I had to "factory reset" but this has caused the problem. Any advice to a noob?

i can access only to bootloader and recovery. Try several times operations with change recovery, clear storage and hard reset.
The problem is that i see SD-EXT in recovery ( TWRP v.2.6.3.0 ) but only like a folder, no size, no nothing. Last time the problem was mount... and i resolve it but now i can't. Only see internal SD (in recovery) with 50 MB memory. The phone is ville S4 16GB, S-OFF, Unlocked, H-boot 2.16.
Lost over a day with try many solutions of my problem but didn't made it. Please HELP me with some ideas or better - solution of my problem.
p.s. Sorry for my bad english.
Edit step by step:
1 fastboot stock recovery... or i think its stock because i try with several recovery witch i think is stock (and may be here is my problem)
2 when i "clear storage" phone is reset in TWRP 2.6.3.0 don't know why...(after that step if i go in bootloader and try go enter again in recovery i get red triangle)
3 i go factory reset and reflash new recovery and can't mount SD-Ext
i go in CWM go wipe dalvic cache and i take again E:unknown volume for path [ext-sd]...
Please tell me where i go wrong, if some of u have stock recovery please give me link ot it.

Go on hboot 2.15 and follow step for installing roms on 2.16 hboots i'm sure you'll solve the issue! Try connecting the phone to your pc and formatting the sdcard if this does not work try the first thing i said.

but i think they are for Ville/C2. After i try hboot 2.15 internal storgage go 10GB and again cant mount sd-ext (i uploud ROM in internal memory and try to install it but get only Error) and i go again at hboot 2.16.
If u can give me hboot2.15 for Ville (S4) because cant find another.

EDIT3: now i have problems with SIM - after few calls my conversations stops and phone show message with NO SIM but have signal.. after few try to recall i have call again and after 1-2 min call stops and have again this message for NO SIM
The Question is: what i need to reflash to eliminate this problem, radio may be?

With HDMaximus you should only use TWRP 2.6.1 which is mentioned in the OP.
If u would bck to stock from hboot 2.16 u must flash any ICS RUU in fastboot for ur CID.
Then u can follow the installation for HDMaximus again, but its mandatory that ur S-Off

Im with Super CID (111...) S-OFF Hboot2.15 and CWM v.6....
I dont wana go back to stock... i wana go Kit Kat but now havent time because have tons of work from the last days and i think will try to do something on my phone this weekend.

I'm hoping that someone can help me out. My nephew who's a wanna-be geek was going to update my ROM (which didn't to be done really, but he wanted to show me what he was able to do). He went and downloaded the latest CM 11, rebooted and went into TWRP 2.7.0.1, did a factory reset, an advanced wipe, and did the Format Data. When he did that, it got stuck in a loop. Nothing happened. He tried to install the ROM he downloaded, and NOTHING! Long story short, by him doing the Format Data, he wiped out my partitions. TWRP, no matter what I try to do tells me it's unable to mount 'data' or 'media' etc. etc. Which is leading me to believe I have no partitions left. I've tried installing the latest update from Asus, and no dice. I've tried restoring backups that I have on my micro SD, and that's not working.
Can someone please help me and tell me how to fix this? Can I get my partitions back? I'm really hoping that I don't have to send this tablet off to Asus in Dallas, TX to have it repaired.
I'm using a loaner tablet from a rental store, hoping I can get my TF 300 fixed.
Any help is deeply and sincerely appreciated.

I was about to suggest you look in the correct forum, but I see you posted there as well. Anyway, your partitions are probably okay, they have just been wiped out, that doesn't mean you lost them. Have you tried reflashing the recovery? Were you able to flash anything with said recovery before that? Wiping the partitions in the recovery shouldn't have any effect on the flashing functionality, no matter which partitions were selected (except for sdcard, since that's where the ZIPs are stored), so there might be a problem with the recovery itself. Are you sure you have the correct one for your device?

Thanks for your reply. I have tried reflashing the recovery like you suggested. And after that tried to reinstall CM11.Afterwards reflashing the recovery it says “Unable to mount data." I don't know if I need to flash a more recent version of TWRP, if that would even help.
Before all of this happened I could flash almost any ROM of my choice as long as it was KitKat supported. All of my backups and restores are kept on my micro SD so they aren't accidentally deleted. As far as I know it's the correct version of TWRP. It's worked for me so far, ever since I updated to KitKat.

You could try flashing a newer recovery first, I think 2.7.1.0 is available for your device. Also try doing a hard reset by cold booting - here's a how-to: https://www.youtube.com/watch?v=oTU38Ewh8qo (wipe the cache afterwards)
If that doesn't help, reverting to stock is probably your best option. Try this: http://forum.xda-developers.com/showthread.php?t=1757077 (be sure to grab the correct firmware and back up your files)
Have you tried installing any other custom ROM? Does it give you the same error when you try to restore your backup?
Here's what I've come across so far. I did the reset, and that did nothing.
Tried to flash the stock version, after a few seconds, TWRP popped up with FAILED.
I've tried flashing Omni ROM. It will flash, load up, and gets stuck at the boot screen or boot animation. This happens with just about every ROM I try to flash.
Been looking for TWRP 2.7.1.0. I'm not seeing it listed on TWRPs site. Where can I get a copy? Thanks!

Hello, i think i ran into a major problem with my one sv(k2_ul android 4.2.2).
Well it all started when i was in the recovery(twrp 2.7.0.1), i got distracted by someone at school and tapped around on the screen.
Apparantly i wiped it.
Then at home i tried to restore my backup i made after i rooted it, but it ´didnt, work because:
a.) twrp was unable to mount the sdcard
b.) twrp was unable to mount int_sd
after some research i found out that while wiping, the filesystem might got screwed up. I tried several things to fix it like mounting the phone as an usb devices and reformatting it to fat32 --> didnt work, because it wouldnt even recognise the phone.
Also tried wiping it with adb shell, didnt work either.
Tried flashing different recoverys, but twrp 2.7.0.1 seems to be the only one working for me(all other recoverys i tried didnt even start)
Well thats my problem, as mentioned above i have a backup of data, boot and system partitions and also have a stock rom.

EDIT: After some research i found out that i cant go s off with my device in the current state

It wont even mount the internal storage or sd so wiping is impossible
