Running TWRP 2.6.3.2 I have not been able to successfully flash while others are reporting no issues on their end. I am booting into recovery, checking rm -rf as suggested to fix the long wipe times, wiping data/cache/dalvik/system, and then attempting to flash Minimus v1. Everything goes fine but when I try to flash the ROM it just hangs for about 5 minutes and eventually fails. Backup and restore work fine. It was also suggested that I boot into recovery, then to bootloader and back to recovery to fix the issue. I did that followed by the same steps listed earlier and it still fails. Any help would be appreciated.

I successfully unlocked my phone and installed TWRP recovery, but that's as far as I can get. I tried to flash the latest CM10 nightly but hit an error when I run the zip, saying that the MD5 file is not found. I also tried to separately flash the boot.img from the CM10 zip, but it didn't help.
I've cleared my cache and factory wiped (through recovery), but I'm still stuck in the boot loop and can't get to the OS. Please help!!
I think the problem might be that I never actually rooted the phone, despite having unlocked it. Is there a way to root my phone via recovery, since I can't actually boot into it?
I tried to install RebelMod, but i hit an error saying "Unable to mount /data" -- I'm assuming that's because it's unrooted?

update: i managed to flash RebelMod successfully (according to TWRC) by running the zip file, but I'm still stuck in the boot loop even after doing a Factory Reset thru TWRC.

wait a sec -- I got it working after I flashed the boot.img with fastboot.
Using ROM Manager and the latest version of CWM I tried installing today's (12-08) nightly build of CM11 to try it out. After doing a complete backup and then wipe data/cache the install failed. I'm not sure why but it said it was aborted and to check log and report the error to ROM Manager.
After that it asked if I wanted to reboot and I wanted to make sure I restored from my backup so I backed out of the screen but then it booted up on its own. It did not get past the S3 loading screen with the CM Android/alien guy it just turned off, back to that screen again and then back into recovery.
At this point I went to recover from my backup file and it said MD5 mismatch and won't load. I've since tried wiping data/cache/dalvik cache all manually and then boot up, or wipe everything and try to install the rom again... every time it just loops back to recovery mode. The last time I tried installing the rom again it told me I might have lost root access and asked if I wanted to repair it... I'm not sure what that means.
Any suggestions? I am lost at this point. I have no experience with ADB (I'm not sure if that's my only option) but I've used ODIN mode a few times. Easiest solution would be best.

I had the exact same issue when trying to flash that same CM11 nightly build but with TWRP...
I just pulled the battery took my SD card out and reinserted it and I just kept booting into recovery and turning the phone off until it let me reflash a rom or backup. Obviously this isnt the most legitimate solution but that could work for you! It did for me!...
This could have been caused by needing an update of CWM or TWRP to better support 4.4 perhaps?

I tried that to no avail. I unfortunately had to revert to stock unrooted via ODIN mode and it got me to boot back up. I can just reroot now. Luckily I made a backup with Titanium and uploaded it to my Google Drive.
Guys I'm freaking out, I installed TWRP while on 4.4.2 and then later tried to install the OTA Lollipop update. My phone rebooted into TWRP, and now it won't stop. Even normal reboot option continues to reboot it into TWRP.
I tried flashing another ROM, I tried factory resetting from the TWRP menu, I tried rebooting to recovery then rebooting normally, nothing is working.
Please, what can I do?

So your completely formatting system and then flashing a custom rom? That should fix it if not just follow my back to stock guide.

I found your back to stock guide, you're sure it will work? I'm trembling in anticipation here because the .TOT file is going to take 5-6 hours to download, and I've got work really early in the morning. If it doesn't work I have no idea what I'm going to do.

What worked for me was from the TWRP terminal command was to type
dd if=/dev/zero of=/dev/block/platform/msm_sdcc.1/by-name/fota
then hit enter
type
dd if=/dev/zero of=/dev/block/platform/msm_sdcc.1/by-name/misc
then hit enter and reboot.
I was able to fix the phone a few nights ago; I was on the road and never had a decent internet connection to update.
The return to stock method ended up working. Although, the process failed about six times before finally going through.

Recently I updated my Nexus 5 from Lollipop to Marshmallow 6.0.1 (MMB29Q) - latest build.
everything stock works fine but, once I flash a customer recovery, in my case TWRP it stops working...I get either a startup loop or TWRP gets erased by stock recovery.
I tried various steps ...
flashed TWRP 2.8.7.1 - rebooted to boot loader was able to get into TWRP but, once I reboot system, everything works but TWRP is gone/erased and replaced by stock recovery?
Flash TWRP 2.8.7.1, then flash latest TWRP 3.0, reboot to boot loader and can get into TWRP but once i reboot into system, I am stuck on the initial boot loop.
I have to start from scratch of flashing system and boot etc to get past the loop and it still replaces TWRP with stock recovery.
I can always get into Fastboot but when I go to recovery it seems its being replaced by stock recovery.
anyone experiencing this issue? thanks for your help.

If you give more detail on the steps you are taking it will be easier for someone to help. Also, have you tried doing a factory reset when it bootloops?

I had Lollipop with TWRP 2.8.7.1
yesterday, I downloaded latest build from android site...extracted all files....booted into bootloader and flashed all files..
rebooted - Marshmallow works perfectly...offcourse it not rooted yet or its locedback due to this flash
Scenario:1
So downloaded TWRP 2.8.7.1 and flashed it as recovery, no errors nothing...i rebooted to bootloader and went to recovery to confirm TWRP is there
I rebooted the system - TWRP gives option to root which I said yes so its rooted. I reboot system, this goes into bootloop - I wait for say 5-10 min in this loop mode and I see that adb can recognize the device but nothing else....its stuck in loop
when I force shut down by holding power button for few sec - and go back bootloader/recovery - TWRP is still there
***note*** I did factory wipe in TWRP but that did not fix above condition.
Scenario2:
exact same steps but after I flashed twrp2.8.7.1, I rebooted bootloader and flashed twrp 3.0 as well confirmed twrp 3.0 is flashed and rebooted system - phone start normally no issues - when I restart to bootloader - no TWRP exists, its replaced by stock recovery
Scenario 2, I tried couple of other ways and still the same thing happens - twrp 3.0 get erased.
not sure what I am doing wrong but TWRP doesn't stick
since this is my active device - i flash system.img and boot.img to get the phone to working condition

That did the trick.....thank you very much. I was using older version of supersu thinking it was latest.....I downloaded V2.65 and installed it and that fixed all my troubles..

I'm not a total noob, but I do this so infrequently that I can't commit it to memory. Would really appreciate some help here.
I followed the official LineageOS Guide for Herolte, over here.
I had done this previously on my Nexus 7 so I was fairly confident.
Everything worked well, I got TWRP installed using Heimdall from Linux, and successfully pushed the latest LineageOS and open_Gapps images to the phone.
Rebooted into TWRP, and followed the recommendation to wipe System, Cache and data partitions. This failed with red text stating that ti was unable to mount data.
I googled some, and found a suggestion to reformat the partitions, first to a different file system and then back to their original file systems again. I tried formatting /data to exFat, and it took forever, eventually erroring out with an error message. After this I can't get into TWRP anymore. When I try the phone boots to the TWRP startup screen, and just freezes there.
I have tried reflashing TWRP using Heimdall and this does nothing. Flash succeeds, nut still freezes at TWRP splash screen.
I googled some more and found a suggestion to flash stock Samsung firmware, and then try again, this time formatting data system and cache without trying to first wipe them. Problem is I can't successfully flash stock firmware.
I tried using this method in Heimdall, but it complains "Local and device PIT files don't match and repartition wasn't specified!" and fails.
I've also tried booting into Windows and using Odin, but it never works. The screen turns grey, and no progress indicators ever start. I assumed this was because I had the incorrect drivers for the phone, and tried using zadiag.exe to replace them, but the driver install fails.
I'd greatly appreciate any help. I can't seem to force the phone to shut off, it just keeps rebooting and getting stuck on either the TWRP spash screen or on the download mode screen. I know this is solveable, but unless I figure it out fast, I'm worried I am going to burn in the screen.
Any suggestions appreciated!
Never mind. I must just have had a bad stock image.
I downloaded a different one, and retried heimdall and it flashed properly.
I am now able to re-flash and boot into TWRP again.
Hopefully I won't have the same problem again when I try to wipe the partitions...

So I solved this issue by (once I reflashed stock, and then reflashed TWRP again) formatting the data partition twice, first to ext3, and then back to ext4. This looks to have cleared the encryption issues that were causing thsi problem ,and allowed everything to continue working as expected.
