If this is your first time rooting I would double and triple check you're rooted and good to go, and also I would do a test boot to recovery mode to make sure that's all good.
For me, the easiest way has been to flash through the recovery mode. Its basically like a factory reset though, so it will erase everything if you're going from, say touchwiz to CyanogenMod. Make sure you have a D2VZW (Verizon) ROM zip and the necessary GAPPs (Google apps like gmail, etc). Those apps can't be packaged in the ROM zip for licensing reasons. Also, you may need a radio to flash (not absolutely necessary in most cases though).
Once you have all of those, place them on your external SD card. Boot into recovery mode (you can use ROM Manager app from play store if you're having issues with that, its tricky to get the hang of the first few times). From here, you'll be wiping/formatting your /system, /cache, /sdcard (INTERNAL SD), there may be more. I always do basically everything EXCEPT your external card. From here, you'll go back to the main menu and press "install zip from SD". Then theres another option to install from external SD. Choose external SD. From here, find the radio first and install (again, not necessary). Next would be your ROM zip file with the same process, then your GAPPs file with the same process, then reboot. Don't interrupt ANYTHING its doing, or you may brick it. First switch from touchwiz to cyanogenmod took a very very long time to install the OS, probably all the formatting it had to do. Subsequent flashes take all of 15 seconds.

I still for the life of me can not figure out why people are having to do this. As habbit I do steps 11-15 every time I flash a rom and have no issues at all.
I do thank you guys for putting it out there ...definitely helps those that do run into the issue

something definitely is weird with the clock work
i beleive even though its the same almost everything
some miscommunication happens with the skyrocket
it wont fix permissions right
and you run into issues where dalvik cache wont clear
or simply put it wont fully clear a partition thats been flash over and over and overand has become corrupt
never had clockwork act like this
i just ran it on my atrix and it took seriously 2 minutes when i chose fix permissions
and on our sky rocket. no matter what ,, its always instant
Vote to pinned this post! amazing post very helpful.
pinnd agreed
just did it worked flawless
I agree this should be pinned.
I just followed these instructions after having a ton of weird FC's (google services framework, market, etc) even though I hadn't made any changes to a stable setup.
Now everything's running smoothly and haven't seen an FC.
Does the odin have bootloaders?
Will this method trigger flash counter?

I have noticed that if you are running a t-mobile rom the fix permissions takes signifigantly longer...just an fyi
Awesome instructions and helped me in fixing some weird issues I had with my phone.
BTW, if you using OSX or Linux (as I do), you can do this with windows running inside vmware if you use the SGS Drivers from Softpedia.
I used this one 1.4.8 sgs drivers
So it's necessary to format the external sd too? Our would just internal be sufficient?
I did this last night, just the internal SD card. Took my 16GB microsd out when I did it. The ROM runs nearly perfect now

Hello guys,
I am trying to update to ICS on my WiFi Xoom MZ604.
Did a full wipe and cache wipe on clockworkmod, but it won't find my sdcard so I can flash it with ICS, instead it will show me the content of /sdcard and won't find anything on any directories, since I wiped it.
What should I do??
Thank you.
I'm assuming you are using the Tiamat recovery. Idk if it supports external SD or not. However, rogue recovery 1.3 supports both SD cards and adds a few other features. Not to mention it looks a lot cleaner.
I would suggest putting the rogue recovery on your SD card and flashing it.

thanks 4 the info
How would I flash it from the SD card?
yeah, still don't know what to do :-/
Plug you Xoom into your computer. Some kernels are sometimes problematic so put you CWM flash file on both the external card and internal flash. These can be distinguished in my computer. Be sure you have the latest drivers or you may have trouble finding your Xoom. http://www.motorola.com/Support/US-...tware_and_Drivers/USB-and-PC-Charging-Drivers
After that you go to install zip from sd and choose accordingly. Might I also recommend rebooting into recovery from the CWM app as I know in my case, I cannot access the recovery menu from the boot screen.

I've already flashed a custom recovery image of ClockworkMod Recovery 5.8.3.1 and have rebooted into recovery, which works fine.
I have Downloaded the latest version of CyanogenMod and gapps-ics and placed on the root of the sd card in my xoom. I have also tried this by placing this in the root on the internal storage but I am having the same issue here.
I can wipe data/factory reset fine, as well as wipe cache no issues, but then when I go into install zip from SD card, then choose zip from sd card I get the line E:Can't mount /sd/card/ so can't install the zips cm-9.0.0-rc1-wingray or gapps-ics
Any thoughts on what might be the issue here, and why I cant get to the zips. As I said I have tried this both ways, having the zips on the internal storage and on the actual SD card.
HELP! So so fed up with v3.2 I need CM9 on this!
I don't know if the message is caused by the launcher. I would disable the signature check and try again.

I had the same problem, did a few google searches which indicated there was a problem with that version of ClockworkModRecovery, i ended up reflashing with ClockworkMod 3.2.0.0 (R4c) and all was fine after that.
Hope that helps,
K
Thanks will give that a go and post back later for if it helps others.

So Im starting my gamut of backups before i begin unlocking/rooting this thing and Ive run in to a little roadblock. Im currently trying to backup my IMEI/EFS per this thread (http://forum.xda-developers.com/showthread.php?t=1852255). Step one check. Step two no check: While trying to run the synergy zip, I am unable to mount the sdcard in stock recovery to "install the package". Apply update from external storage>E:failed to mount /sdcard (invalid argument). I have a 64GB scandisc mini that I have watched some movies off of ect. I didnt format it just used it out of the box. Any thoughts? Ill post if i figure out my user error...
Coming from droid charge mostly at rootzwiki, I would have posted this question to the thread linked above but XDA sayz I noob no can post developer thread X/
You have to be rooted to run the zip and on a custom recovery.
Edit: I'm not sure if you must be unlocked but I know you need to be rooted to run the Synergy zip. Also, custom recoveries only see your external sd card if its formatted to Fat32.

yea thats what i ended up doing, but it sounded like it could be done on a fully stock rom...

I have a SCH-R530U US Cellular GS3 running on JB 4.2.2 with AOKP's mr1 build 5.
Kernel: 3.0.67-aokp-00038-g5a02406
I'm bored of this ROM and want to try Liquid Smooth for the features and hopeful battery life extension. I have been trying to get my phone back to stock EVERYTHING every couple weeks for the last few months now, and have not found any stock rom/kernel files that do anything other then brick my device into a hard boot loop. I have a NANDROID backup, so all is not lost, but I'm just so sick of not being able to find ONE file that will give me stock kernel and stock ROM so I can try a new ROM with new features and speed. I would flash LS over the AOKP build, I just haven't found any info on whether or not the ROM is compatible with my kernel, or if that's even an factor and Iv freaked out too many times over thinking my phone was F$*!ed to be that ballsy in trying something without having exact instructions or some sort of literature/video. So Id rather be safe then sorry and start completely fresh.
Also to get rid of all the non-sense i have on my phone from flashing tons of files to get rid of AOKP, and not knowing how to delete them.
The other issue. My SD card isn't recognized by anything. I cant move apps to my SD card with any app out there, the file is emulated and what i think happened was somehow, I ended up with two SD card files, and they have mixed files in them. So being as newb as I am, idk what files i need to keep in order for proper functioning of my phone and which I can delete, so i can move apps to the SD card and pictures and all that stuff to free up space on my phone. Im more then willing to delete everything on the card, but that wont fix my unorganized internal file clusterf$!*#.
Iv seriously being looking for months, downloaded ADB toolkit to try and get stock back that way, but just another boot loop.
TL;DR
1. Need stock ROM/kernel files for my version that will actually work and not bootloop me.
2. Can any ROM be flashed over any kernel, or are there restrictions with specific ones?
3. How can I fix my SD card problem (not recognizable by any App2SD app; literally none -says my SD card is emulated), with messed up internal files?
PLEASE HELP!!!!
Through your recovery do a full wipe (system, data, cache, dalvik) Sounds like you want to wipe your internsl sd too? Depending on where you keep your info you can wipe your internal SD but it's not necessary.
When you flash a ROM it will come with it's own kernel so you don't have to worry if a ROM is compatible with your kernel. Just make sure you flash a ROM that is compatible with US Cellular.

Thank you for your reply, honestly.
I vaguely remember doing a full wipe (system, cache, delvik, and data) and it not fixing the issue. But, I will try again. I'm more concerned about my SD card issue and all the messed up files. Will wiping all this fix that? Using ES explorer, I have about six /0 files... Idk if you know much about that, but from what I've learned, within that ONE /0 file, there should be storage for my SD card (data/media) and of course, there's nothing there. So, do you think or do you know that doing a full system wipe will remedy the problems? No tone intended, not trying to be a jerk, simple curious for the future.
Just format the SD card. Wipe everything off if your comfortable with losing the info. You can save important stuff top your CPU if necessary.
Sent from my SCH-I535 using xda app-developers app
Won't formatting the SD card just erase everything anyway? This would fix my "corrupt" file issue though, correct? The multiple files and unoraginzation?
Sent from my SCH-R530U using xda app-developers app orginized
Yes formatting it will completely wipe it.
Sent from my SCH-I535 using xda app-developers app
And that's something I can do through my recovery, yes?
Sent from my SCH-R530U using xda app-developers app
I'm using TWRP and it can be done though recovery.. wipe > advanced > check sd card
Just backup anything you want to save
