I have never experienced this in 3 different 4.1.2 Stock ROMs. Try to see if you have some quiet mode on or something that triggers on a specified time.
u can try repartition with pit file via odin...
I re enabled google play music and samsung music hub and just hid their icons instead. Haven't had it happen since. Maybe that was it, or maybe just a fluke that it hasn't gone quiet on me lately. Stock rooted and not much installed, only a few things disabled that I didn't use.

Hello Everyone,
I tried researching this topic but haven't found any with that seem to deal with my issue.
I'm having a very interesting situation going on. I upgraded my phone to ICS, and have done a Factory reset. I have noticed that my Default Notification sound will be set to Piezo Alarm, and my ringtone will to Ta Da. No matter what I change them to, my phone will revert back to these within 24 hours. Has anyone else heard of this? or have any suggestions on what I can try?
Thanks

Glad im not the only one out there, i have this same issue and would like to know if there is a fix for this as well.
I have gone back and tried FLASHED the ICS update via ODIN and still have the exact same issue. Tried multiple Factory Resets and still it reverts. I did a search from my PC and found "piezo.ogg" in system\media\audio\alarms and renamed it to "piezo.old" to see if that makes any difference. I'll let you know if this has any effect.
I've run into this issue with multiple roms (Sky ICS and CM9) although not to every 24 hours, but it just randomly seemed to forget what ringtone I chose and went back to the default one (it would be listed as "Unknown Ringtone). I think maybe it doesn't like that I'm using an MP3 as a ringtone.
I've rooted my Samsung Galaxy Skyrocket and inserted ArmyVet AOKP ROMand everything was fine until today for some odd reason whenever I use the single button headset instead of changing, pausing or playing it increases the volume. I cannot find the reason nor the app that is causing it. Don't want to go through the long process of testing each app. anyone experienced this before or knows the app or solution?

This is a kernel issue. Are yo on AOKP JB or ICS? In any case, get a proper kernel and you should be fine.
help
Aokp ICS, can you direct me to the correct kernel? I believe I'm still using the stock kernel. I never installed a new one or was given one to install.
If I am playing music then every time I press something on my screen (such as an app or an app button - basically any time a press is detected and activates an action) the music will lag/stutter for about a second. Scrolling does not cause lag, nor does pressing hardware buttons (power, volume, home) or the menu/back buttons under the screen. For example, the music will stutter when I open the Gmail app and also when I "click" (press) on an email to open it.
I am currently rooted on the latest rootbox (3.9.1) although this problem has occurred with several other ROMs including AOKP and CM. Also occurs on all music playing apps I've tried (Google music, Android music, etc.). I believe the problem occurs with MP3 files but not FLAC files (although I have not tested this extensively).
I believe this is different from the buffer problem whose solution is to download a music player with the option to increase the buffer size. The problems I am having seem to be different. Any help would be greatly appreciated and thank you for your time.

Never had this problem on stock. Only since rooting has this occurred.

Ahhh that would actually make a lot of sense. An app I was making would make the glitch sound thing with .mp3 but not .wav
Sent from GS3 runnung CM10.1 and blacked out theme.

My line of thinking was that since you did not have this problem on stock, it would be probably a AOSP/AOKP/CM Rom issue. I would of likely thought kernel, maybe your phone did not like a certain kernel version 3.0 vs 3.5 or kernel from a certain kernel developer or the scheduler/cpu governor combination. I am uncertain now since you said that only since rooting which I assume you mean you were on stock rom and you just rooted it instead of flashing a different rom with root already included.

I am currently on Rootbox. I flashed several other ROMS including AOKP and CM immediately after unlocking the bootloader and rooting, all which have had this problem. It is probably most definitely a ROM issue but I have found no other information on this specific problem. Would using a different kernel help considering flashing several other ROMS hasn't solved this issue? Not sure if they all used the same/similar kernels. I don't extensively understand the importance of kernels.
Still have yet to find a solution to this. Any help would be appreciated.
i saw in the deviant xone thread that it stops if you disable touch sounds and screen lock sound, it should fix the issue.

disabling touch sounds and screen lock sound fixed the issue for me .... Thank you
Hey guys, I've been having this issue for weeks and it seems that it doesn't matter what ROM I'm on.
All sound will inexplicably drop out. I mean system sounds, the ringer, notifications, music, in-call, podcasts - everything. Nothing in particular seems to trigger it and sometimes it eventually just comes back on it's own. It seems that sometimes I can get it to come back on by turning the volume up and down repeatedly (may just be a coincidence though). This occurs multiple times a day.
I've tried numerous 4.3, 4.4, and even 4.2.2 sense ROMs, and it still occurs. Pretty annoying because what good is a phone that never rings or notifies you, and you can't even talk to people when you dial them.
I can't confirm this, but this may have all begun when I updated my hboot to 2.15 for the new 3.4 kernel based ROMs. Can't quite remember because it's been like this for quite awhile now.
Anyways, if anyone has any ideas it would be greatly appreciated. I'll try to post a logcat the next time it happens.

No idea? How come I seem to be the only person with this issue? Haha
I'm currently using Dark Jelly S 28.1 (the last version with the 3.0 kernel). Seems to be a bit better. Anyone have any links or zips of other 3.0 based ROMs? The lionks in all the threads are dead. Guess they took down the zips when everyone started switching to 3.4 and 4.3/4.4.

Well I tried some other 3.0 roms thanks to some very helpful people in another thread I created http://forum.xda-developers.com/showthread.php?p=48029123#post48029123 but I am still having sound issues.
I tried reflashing the 2.15 firmware and the sound was back until I received a call. After I hung up, I lost all sound.
I'm not very knowledgeable when it comes to this stuff, but I ran a logcat and noticed a view errors relating to sound (SoundPool: error creating audio track, Audiotrack: could not get audio output for stream type 1, AudioPolicyManagerALSA: getDeviceForStrategy() speaker device not found for STRATEGY_SONIFICATION, etc). Not sure what this means or how I can go about fixing it. Looking at running an RUU to see if that will help but I'm on Linux so it's a bit more complicated than just running the .exe and I'll have to reroot anyways so hopefully someone can shed some light on this for me. Attached is a log showing the errors.
I am having the exact same problem with my HTC one S. For no apparant reason all sounds stop (speaker, music, notifications, can't make or recieve phone calls)
My problem started when I was running stock software. It got so bad that I had no choice but to S-OFF, Root and install a custom rom to see if that solved the problem. I currently am running MaximusHD and I love it. But the problem is back.
After searching throught the logs there seem to be two main errors "failed to enable the mixer controls for the speaker" and "AudioPolicyManagerBase(261)".
Does anyone have any idea of how i can disable or reinstall the service that is tied to this?
Bump... I also seem to having this problem. I'm running stock rom, rooted. S-On.
Randomly, my audio stops working. Sometimes a reboot fixes it, sometimes not. Sometimes a hard reboot (holding power down) works, sometimes not. More often than not, rebooting to recovery, then booting sytem fixes it.
I know it's not hardware because during this occurrence, some software (e.g, google voice search) gives an error but runs fine when things are normal.
I tried looking at logcat (I'm new at this) but nothing jumped out. Would love some help getting to the bottom of the problem as it's really annoying to randomly not be able to answer calls.
So this problem is happening more and more to my phone. Since my post earlier this month, I've tried Stock rom, liberty rom, and CM11, and the no-audio problem happens randomly on all of them.
This is really frustrating, as it makes the phone unusable. I'm very close to giving up and just getting a different phone.
Interesting find... I removed Lookout Security, and this appears to have solved 80-90% of the problem.
Hello!!
I have an issue with my Mate8, I really don't know if this is in all phones or just in mine....
It's when a song or a video is playing, and when the power button is pressed to lock the phone, the audio breaks for like half a second, but if you press the button again to unlock, it do it again, and everytime you press the lock button.... it kinda annoys me a lot.
The rare is that this only happens when the audio is coming from the speakers; when i have anything plugged in (headphones or whatever) it dissapear, and with bluetooth headphones dissapears too.
It happens with all apps, spotify, native app, soundcloud, vlc, poweramp...etc
Thanks a lot!!
Best regards, ElĂas.

I did have this problem previously after flashing a pre-rooted rom of version B320, but it went away after I flashed the stock rom back.
Since you haven't mentioned anything about custom roms and/or rooting, I'm going to assume you're on a stock rom at the moment, in which case I'd suggest a factory reset (backup data 1st of course)
If you're running a customized rom I'd suggest discussing it in whichever thread you got that rom from as the issue may be specific to that rom

No problem, please leave feedback after if it worked or not, may just help someone else down the line...
I'm on B320 and this happens to me too! Please come back and tell us if it disappeared after a factory reset because I might do one as well.

I'm wondering if that might be a CPU usage issue, try go to settings - advanced - battery manager - power plan, and set it to performance mode to see if the problem still occurs

SOLVED!
It appears to be some setting in Nova launcher that caused this, i have to install/uninstall a lot to get into this, but 100% sure is some setting in nova, just did a clean reset of the phone and everything superb! reinstalled nova but this time, trying not to modify a lot like before and it's working perfectly.
