Used all the suggested methods. Using Windows 7 and tried like a million times.
1. Restored to V21E stock Rom.
2. Downloaded the B2CApp .. The software update tool doesnt go beyond Analysis. Says Problem Connecting Phone and restarts till it allows the exit button.
3. Used the method where I enter the IMEI number, here the tool goes beyond the download phases and crashes at the update phase.
4. Tried using the Offline method but the tool doesnt go beyond the connection phase in this too.
5. Tried using direct update method on the UpTestEx tool either by CDMA or 3G but the tool crashes at extracting KDZ.
Current Stage :
1. MSXML installed
2. B2CApp Tool Installed.
3. All Drivers Installed.
Please guide me towards successfully updating the V21Y baseband with 2.3.4

Basically on a windows machine you download kdz updater and uncompress it as it is a zip file.
Download from here.
This is what I did on an xp laptop.
I had to down load the drivers from the official lg site. when here you enter the code that is on the box the phone came in or under the battery.
Chose the your county and laguage from the drop down box at the top right of the dialogue box. Then from the middle you can choose the drivers. For some reason I had to find the files and click on them to install the driver.
Ok once you decompress kdz updater there are three seperate files to work with. 1. windows enabler. 2. msxml.msi 3.KDZ_FW_UPD.exe
You will also need to download the right file to restore with,You can find it with google.

Install msxml now.
Now you have to boot the phone into rescue mode. On Lg Optimus One turn phone off even if it means taking out the battery.
Press power - volume up - back (this button is next to the home button).
Connect to usb now.Computer found new hardware and installed four drivers.
Then open device manager. Left click my computer - properties - device manager. You need to disable the lg modem driver. Left click on it and disable.
Now launch kdz updater. There will be a drop box on left set to 3gspt or similiar. on the right drop box set to diag. The long box just above the info box with a file at the side lets you browse to find restore file. e.g. V10a.00.kdz.
Double click the button below and let it do it's thing. Took 15min. My phone shut down and came up with an error at the last minute. Anyway I rebooted and all was good.
Everything was the way I left it. Just had to re-root.
My only real problem was getting the kdz installer to see the phone.
After installing the drivers restart pc. If it finishes quickly and just before saying finished it says phone not found try removing all phone related drivers including nokia pc suite and the lg suite and remove and reinstall the drivers.

i need help stuck on emergency mode and computer wont recognize phone it did a at the beginning but when connecting I got partition error table on LGMDP, also got an error on KDZ saying someting with error code 1002 ... please help don't wanna take out the battery cause i'm afraid i would have officially bricked it as in it wouldn't start back up! please willing to try anything at the moment!
Just follow the steps which I mentioned above and everything will be alright.

No, i suggest you do not use kdz updater, it had high chances of bricking your phone.

It once bricked my phone too when I removed battery in emergency mode. But after that I used it many times and it worked flawlessly.
You can definitely use kdz, when you know what are you doing.

I saw a thread like this before but I can't solve the problem.
I accidently flashed O2x stock rom to my G2x.After that I installed rom manager to this version.
Updated recovery to 5.0.2.0 and selected Reboot into Recovery.Flashed P999-G2x Stock Rom V10f.Tried to reboot but couldn't pass second LG screen.

The only difference between this instructions with which i did is APX folder doesn't contain usbpcdriver folder.Anyway windows identified nvidia usb xxx.

Tried @Shmoopty 's advice.I installed ADB and all drivers.
When I plug device (while pushing vol - button) program started to update.Uptade stopped and finished.It says try to reach LG Customer Service.
(I selected USA,English before use LG Update Tool)

Following instructions from the link above and you will be fine.
I know of a few that did what you did and was fixed in no time. Dont panic. It is just a soft brick that can be corrected.
ps: Make sure you download the LG drivers. and install on your computer
Once you have the V2E installed, you will have the latest T mobile ROM.
Then re-install the CWM (oneclick NV Flash) and then flash your favorite rooted rom

I started to update using LG Software but after 12mins still at %4 and says"running LG Mobile Software Update" is it updating or what i don't understand.

Edit:Tried with win7 compatibility.I selected kdz file, program started but my device seems like p990.Cause of that program updated it like p990.

It works on Windows 7. Not a problem. Make sure you install the LG drivers.

I don't really know i have proper drivers or not.I'm using
LG_T-Mobile_Android_USB_Driver_v2.2 but when I plug phone to pc windows can't find some of the drivers.
I also got so many drivers like:
LGUnitedModemDriver_WHQL_ML_Ver_4.9.7_All_Win7_LGEAll - ~46MB
LGUnitedMobileDriver_S4981MAN32AP22_ML_WHQL_Ver_3.2 ~11MB
Yeah I followed it but I can only open device in recovery mode now.
When I try these steps (after pressing vol + -) it stucks while updating %4.
(in recovery mode)smt cutting the connection between phone and pc
Look in the dev section for the nvflash restore, think jboxer authored it. This will properly recover the partitions and firmware... you would only want to use the kdz tool for baseband upgrade/downgrade/repair.

I have the same problem. But I applied the solution suggested by OP (http://forum.xda-developers.com/showthread.php?t=1248644) and it works... half-part. I recovered the OS but I'm still without baseband (so, no signal or battery icon).

I was trying to get this brick back to life over 4 entire days.
I just picked up a used LG O3D P925 that has no connection to any network (well wifi works). When I tried to update using the LG mobile update tool. I had a no success detecting automatically so I clicked recovery and manually entered the IMEI and S/N. Soon I got a failure error. Prior to this I read a thread about OTA firmware updates but I have no button for OTA update.
I tried rooting using megatron (windows version) and installed the LG drivers but windows keeps detecting new hardware and wants me to install drivers. When I click run it just says "daemon started" and I wait for a couple of minutes and .... nothing. When I check the apns either none are listed or I get a message about "reorganizing internal processes. please try again later " ...
I contacted someone who had their phone replaced by carrier for same issue. Possibly the carrier has blocked the phone from network? ...O I hope not

ok I have succeeded getting root using Unlock Root. Now when I open getRIL and select a RIL to DL the "ok" button is "greyed out" or not available, just the cancel button. So after I cancel and select another RIL to DL & install I get the superuser permisson granted message followed by a message that says "The system partition needs to be writable.... and back to the greyed out ok and cancel choice.

I see imei could have been damaged uhm try updating to V20C see if it helps

I seem to get hung up on the step where you check the device manager for current active "COM" ports. When I connect the USB (holding vol up + power) I get OMAP4430 appearing for a second under ports and I get the "windows has detected new hardware" flashing on and of simultaneously. The phone doesn't seem to want to boot into download mode and it doesn't vibrate after 3 seconds.

Delete All Lg Drivers and Software Run CCleaner and clean registry then reinstall LG PC Suite IV

On The Phone Goto Settings/Applications/Debugging/ Make sure ADB is ticked and then try flash it again try a diff usb port also make sure its not plugged into a usb hub. if it fails again i think the problem is with pc not the phone
Try connect device without battery and with holding vol up. Then after device is connected (wait 20 sec wile holding vol up) reinsert battery and start update.

if it keeps connecting and the disconneting try this. attach usb cable to PHONE only then take your battery out, then put battery back in and quickly press volume up button and then attach usb cable to PC. keep hold of volume up button all time dont let go and install all drivers omap, usb driver etc.

I have managed to make things worse ... :crying:
Now my phone is stuck on the AT&T boot screen. The only way I can power off is to remove the battery.Yes I tried flashing the v20P kdz after I thought I could do much worse.
I no longer have a problem getting my computer to recognize the drivers. I'm not sure if I flashed the v20C kdz without a problem or not. I thought I was able to root and things were ok except that I still had no network connectivity. I don't recall exactly what I tried next but I think I lost root somehow and some of the installed apps began misbehaving. I also noticed that superuser was not installed. I tried to root again but couldn't. I treid to flash a kdz again but again not very successfully. I also tried repairing the IMEI using this guide http://forum.xda-developers.com/showthread.php?t=1734330
I flashed the CP ? but didn't manage to restore the imei.
Now I when I power on I get the AT&T animation ... I tried to flash back to the 20C kdz without and luck. Now I have no idea where to go from here and would be grateful for some input.

If you were stuck on the art boot screen after flashing v20p you should have don't a hard reset and wiped all data. I had the same issue when going from v10k to v20p. As far as the no signal goes I'm not sure but it could be a bad radio or it could have been stolen and deactivated by AT&t

I have seen cases where the USB cable itself is the cause of the issue. For sh*ts and giggles, have you tried using another cable?
Unable to find a solution to my problem. Searched and read many threads on the subject and think I've about tried everything with no luck.
Have a T-Mobile D801 phone. Took the OTA update and successfully upgraded to kitkat. Did not like kitkat so I downgraded back to 4.2.2 using the LG Mobile tool and the D801110C.kdz file. This downgrade was successful and I was able to root also with success.
So I have a stock rooted 4.2.2 phone with stock recovery.
Used the phone several weeks before playing with the build.prop file. After a modification, restarted the phone and the phone now will not boot past the LG splash screen.
The phone will boot to stock recovery as well as fully boot to download mode.
Installed newest LG mobile tools as well as the drivers, and reinstalled several times.
I have tried using the standard LG mobile tools to revert to stock. When using this method, the program identifies the phone correctly as a D801 but does not list a software version and tells me that there is no upgrade available. When click on upgrade recovery, it tells me that there is nothing to fix. So, I tried clicking on upgrade recovery without phone attached and enter the imei number and connect the phone. The software downloads the update successfully but when it attempts to install, stops at 2-3% and tells me to connect the phone. Restart the phone in download mode, reconnect and attempt again with same failure. Have tried this with swapping the downloaded file with110C, 110G, and kitkat KDZ files and all fail equally at 2-3%.
Have changed and tried com ports 41, 7, and 4 as well as disable LG modem in device manager and all fail.
Have tried this method on a windows 8 64 machine, windows 7 64 machine, and windows xp 32 machine and all still yield equal results. Tried several different USB cables and ports.
Have attempted using the KDZ with the L&D method using the 110C/D and kitkat files and the 110C/D fail at 49% and the kitkat will not even start, just errors right away. Tried this method with the different computers as well as cables and com ports.
I have also tried using the LG flash tool with the D801 TOT file and dll. Will not start and gives me error that the model numbers do not match even though it says it is a D801 at the top of the program.
At a loss here for what to try next. Any help on this would be much appreciated.
FIXED
Fixed the problem. Couldn't get any of the above methods to work. Downloaded the LG Firmware Extractor and extracted the .DLL from the TOT file and used that with LGFlashTool. That worked perfectly. Up and running again.
