I usually add a few more steps, and I've never had a bad flash.
I'll spell it out, in case you don't know:
open rom manager > reboot into recovery > wipe data/factory reset > back > wipe cache partition > back > advanced > wipe dalvik cache > back > wipe battery stats > back > back > install zip from sd card > choose zip from sd card > select your rom > install!
Hope that helps.
If you do a search for the full wipe zip in the development area you will find this to be the most useful tool to wipe your phone. All you do is put it on the root of your SD card and then when you install a new rom you use the full wipe zip and it works quite well to prep your phone for the new rom, something that worked well with my old inspire and something I will use with my new Red Inspire when I get it.

It does a good job, be sure before you flash this you have the next ROM ready to flash. It wipes everything.
I found the link to give credit where it's due.

agree with ashishv
but a clean install is good
1. install recovery
2.boot into recovery
3. do any options with "wipe"
4. select & update your new zip file.

Start over again!
Follow the below steps:
1. Flash stock FTF (11.3.A.2.23)
2. Start the device & setup everything
3. Turn off device
4. Extract the kernel that is provided in CM11 Alpha 5 & flash it.
5. Turn on device
6. Go to recovery,
> Wipe Data / Factory Reset
> Wipe Cache
> Wipe Dalvik Cache
> Format System
> Format Cache
> Format Data
> Mount System
> Mount Cache
> Mount Data
7. Install ZIP from SD Card (select CM11 zip)
8. Wipe Cache
9. Install ZIP from SD Card (select GAPPS zip)
10. Reboot device, setup everything! Done!

[Completed] Partition Cache vs Dalvik Cache vs Storage Settings Cahce

I searched but could not find a clear answer.
Q1) What is the difference between A) Wipe Cache parition from recovery B) Wipe Dalvik cache in recovery and C) Go to Settings -> Storage -> Internal Storage -> Cached Data -> Delete.
Q2) Is it good to wipe which cache regularly?
Q3) I can't wipe my parition cache using TWRP as my phone is encrypted. Is using the Storage Settings wipe good enough?
