You can't do a Nandroid backup of your SD card. A Nandroid backup is only for backing up the image on your phone.
Simply plug your phone into the PC, select Disk Drive and backup all your SD card data to your PC. Once complete right click the SD Card Drive, click Format. Copy data back to SD card.
EDIT: Sorry - misread the question. Same procedure really - just copy the Nandroid backup onto your PC and then copy it back afterwards. Usually the Nandroid backup and be found in the clockworkmod folder on the sdcard. The folder containing the nandroid backup is usually time stamped.
Does it mean i also have to reflash the radio?
Formatting (or replacing) the SD card will not require you to reflash any part of the phone. The ROM and the Radio are kept on the internal phone memory and never on the SD card.
Simply do as dr9722 has stated - take a copy of the nandroid backup onto your PC, then format the SD card and copy the backup back from the PC afterward and you'll be fine.

[Q] How do I perform a nandroid backup?

Hi there!
I've read a lot about performing a nandroid backup, but how I can I do it? There are several options in CWM, which one is the correct one?
Also, I believe a nandroid backup is a full backup to restore the tab to a previous condition and includes everything: so, after a full wipe (delelte EVERYTHING) I can restore a nandroid backup and all of my files, apps and its data are there, correct?
Last but not least: is it stored directly in the uSD card or in the TF's internal memory?
Thank you!
To do a nandroid:
Go to CWM > Backup and Restore > Backup
You can restore all your saved data from a previous nandroid backup.

Depends on the CWM version you're using.
The older version save only to external micro sd card but the latest version from Team Rouge save to internal and/or external sd card.
My version only saves to micro SD, was panicking when my backups weren't there after taking out my SD card.

[Q] Using TitaniumBackup on ICS

So I'm planning on ditching honeycomb and switching to an Ics rom... I backed up all my apps using TitaniumBackup but the rom requires a full wipe and cache wipes, question is, would performing the required wipes before flashing the rom also wipe the backups TitaniumBackup made on the internal storage?
No. All that is stored on a different partition...so you're safe. Just don't use the SuperWipeFULL version zip. THAT...will wipe all downloaded stuff on the internal SD storage.
Infact...to make it easy for you...just download and use the SuperWipe Lite script from the Revolution HD thread. That's what I do...when I need to wipe.
Also you could move the backup folder location to the micro sd card.
Just go into "preferences" and change it to micro sd card

Don't forget to change it back after you wipe and reinstall titanium, or it will just look like you lost all your backups since it defaults to internal storage.
After reinstalling TB, you can have it do a quick scan to pick up any folder that tb uses.
Also remember that TB stores its own settings on removable card if available on /Removable/MicroSD/data/com.keramidas.TitaniumBackup/

[Q] difference CWM backup to extern or intern SD

Hello,
is there any disadvantage if you make a backup with CWM on external than on internal SD card ?
Any risk ?
Is it possible to move external SD card to another phone (same type) and then restore it ?

I don't see a difference between internal and external backups. My current phone doesn't have an SD card, so all of my backups go to the internal, and it is just the same as my previous phone that had an SD card.
To answer your second question...yes. You can transfer the SD to another phone of the same type and restore a backup.

You probably have "filesystem" selected. You need to use "DocumentProvider storage" and should be able to navigate to/select your external sd card
