Anytime I install any of Cyan's roms and use a theme built for it, I cannot wipe without getting a Black SOD. I cleared my logcat (logcat -c) to clear away stuff that wouldn't be pertinent. I let logcat run for about 30 seconds.
Update: (further information)
I am NOT running Apps2SD and there are NO icons on my screen when flashing the theme. In addition, I have used the repair command to no avail.
Update:
This goes for xromhero theme, Ted's theme, or enochx theme so far. Will test others as time allows.
If you're having no issues with your theme, can you please do a wipe and see if the problem shows it's head? It's a simple fix, just wipe again and reapply the ROM.

what theme are you trying to install? what spl do you have?
Next time, use pastebin ><
Anyway, as already asked, which theme? Does this happen on the base Cyan build?
Wall of text crits you for OVER 9000!!!!!!!! you die

I have 4.1.11.1 with the enochx theme. No issues for me.
thanks :-/
the first obvious thing is the error that says that your led driver is missing.
dont know why people are asking about spl...
Library '/system/lib/hw/lights.trout.so' not found
push this file...
you will find this file in your update.zip
push it to the exact same location and then post the logcat if the problem persists...
have you tried removing all the icons off the screen then installing the theme?
it appears to be missing both:
copybit.trout.so
and
lights.trout.so
neither of these files are in the CM 4.1.11.1 build.
Is it possible one of the apps in the xromhero theme has these two files. Though I am confused why two different phones doing exactly the same steps would not have the same issue.
Though if my thinking is correct copybit.trout.so and lights.trout.so are the drivers for the hero and my touch phones. The dream requires the msm7k.so files dont they?
So logically thinking here, is it possible that when the CM 4.1.11.1 or the theme is being installed it misidentifies the phone as a hero/my touch?
they might be symlinks...

Have you wiped after installing the theme? Go ahead and give it a shot and let us know if it goes to a black screen of death
I had trouble with a theme so I cleaned out my davlik cache, and that fixed it. You could try that. Otherwise possibly a reformat of your ext3. Gparted works well for that.
on some themes dont you have to remove the apps and the clock on the homescreen before you install a theme because usually the themes clocks are 4x3 and the normal ones are 2x2.

Updated the first post to answer these possible resolutions
Ok so I stress tested this on my phone.
Could not reproduce with xhero theme
However, with Enoch
if you wipe > install cm 4.1.11.1 > reboot, log in > recover/wipe > flash enochx > reboot it will black screen
However if you wipe > flash cm 4.1.11.1 > reboot, log in > reboot/recovery > flash enochx > reboot it works
in other words.....do NOT wipe after you install cm 4.1.11.1 allow CM to load fully and sync then reboot to recover and just flash the theme.
EnochX seems to be black screening on the reg screen load on a freshly wiped phone, though interestingly enough if you load enochx with out the wipe allow the system to come all the way back, then reboot > recover > wipe > reboot it seems to work ok

I understand it will black screen if you wipe prior to installing the theme. However, have you wiped AFTER installing the theme? This is my issue. The other night I had to wipe while I was out. This left me without a phone since I had removed the ROM from my SD.
It seems like there is something missing (or conflicting) in the themes. This might be a workaround but it's not a resolution to the issue.
Yes I was able to get the theme running then wipe and reboot without issue. I will try it a couple more times tonight to see if it black screens.

Appears your method of wiping-->install rom-->boot/login-->install theme works.
I still think that there is something missing somewhere or at least would like to hear a reason why it must be done in this order. I'm no longer getting all the F/Cs I was before.
Thanks man
Hi All,
Anyone else have an issue with Bamf Remix 1.6.3 (had the problem with 1.6 as well, but not with 1.5) where the HTC clock widget disappears on reboot? This happens every time I reboot, I have to manually re-add the clock widget. I did a full wipe (data/cache/dalvik) prior to install, then restored apps (and app data, no system data) using Titanium Backup. Other widgets (including HTC ones) are not affected, and everything else seems to work great. Any suggestions for what to try to fix this? Thanks!
I would try deleting the widget apk and then adding it again from the original rom zip file.

would you mind going into a bit more detail? I'm open to trying different solutions, just not familiar with where exactly to find widget apk.
thanks!
dont know for sure if this is your issue, but it worked for me:
move the widget to phone if its on SDcard.
I have a problem. Maybe someone can help me. So I did a wipe and flashed a new ROM. Then I used TitaniumBackup to restore my apps and data. Only now, I can't change my wallpaper. The only things that change it are Live Wallpapers. I'll change it and it says "setting wallpaper" and "wallpaper set" but nothing changes. Help?
Fix permissions, delete cache, dalvik cache, and reboot your phone.

Nevermind, cleared the cache and it did the trick.
Ok so I just downloaded the Ktoonsez rom for the S3 and it's really awesome except I can't find any way to change the lockscreen even when I go into the lockscreen options. I click the reset wallpaper but I still can't change it. When i go into gallery to try to manually set a picture to a lockscreen, it will let me set it as a wallpaper but no option for lockscreen.
Any help please? I tried googling this. I fixed permissions and wiped the cache. Thanks.
After updating my Galaxy S6 (SM-G920F) to 5.1.1, the homescreen wallpaper always resets to the stock Samsung WP every after reboot. I use themes downloaded from the Theme Store. Everything else remains applied (lockscreen WP, icons, etc.) Anyone have an idea to fix it? Thanks.
it may seem weird, but just simply uninstall facebook or disable it, reboot then update it again.

Thanks for the hint, but i tried it and failed. It still went back to stock.
Solved!
I've solved the problem, although I don't know which step did solve it.
Things I did:
Uninstalled/disabled some apps and deleted some files to free up space (my memory was nearly full)
Switched to the default theme and uninstalled all other downloaded themes
Wiped cache in settings>storage
Rebooted, downloaded a new theme and applied it
Wiped cache again and reboot
Wallpaper from theme is still there Yey!
Special thanks to @Justinhopaolo for helping me out
