My phone is Samsung Galaxy Note10 Lite (Android 12, One UI 4.1).
The device was working normally before I put it in my suitcase with many other phones without protection. After a while I took the Note10 Lite out and saw the wallpaper had changed to default like when I activated the phone the first time. A "Safe mode" text also appeared in the lower left corner of the screen.
I unlocked the device using password and it took me to the process looked exactly like a factory reset with Language/Region options and privacy settings to choose. I followed all of these steps.
After the "factory reset alike" was done, my 3rd apps were all gone, only system apps were kept. I opened Settings>Apps and no longer saw 3rd apps in apps list. I moved through the internal storage and noticed 15gb of data was loss. The strange thing is my media files (musics, videos,...) and documents are still intact, nothing has been deleted.
I tried to restart the system, also press the "turn off Safe mode" notification but it always show an error message when the phone boot up:
"Phone started in Safe mode. There's a problem that's preventing your phone from starting normally. You'll need to factory reset your phone to exit Safe mode. you can use Safe mode to back up your data to a computer or SD card before resetting your phone."
As far as I know, phone which turns into Safe mode isn't supposed to do a factory reset automatically. Does anyone know what problem am I facing with? I don't want to do a factory reset as the message said, because there are apps contain important data which will be lost if I reinstall them.

I tried restarting the phone, but then Google Play and all my apps were gone, it's as if the device is partly in Ultra Stamina Mode with other functions still working. Google Play services keeps crashing also...
I think maybe I pressed "activate/deactivate" Ultra Stamina too many times as it was not exiting out of the mode at all...

Ouch!
I just did a factory reset and everything is fine now. Unfortunately I didn't back anything up and once this glitch occurs you can't! (you can't even view your IMEI number)
Sony gave me this reply but it wasn't helpful as I already did the factory reset:
1. If the issue is caused by a glitch on the software, you can try to force the device to restart, connecting the device to the wall charger and then press and hold down the volume up and the power key together at least 5 seconds, or until the device vibrates 3 times, then power on the device.
2. If that does not work you will need to find the OFF key that is next to the SIM card, is a red/yellow button, and please press and hold down the OFF button for 120 seconds.
3. If none of the steps stated before works, you will need to perform a factory data reset, please remember to perform a backup of the information because this process will reset the device into factory settings.
Factory data reset.
From your Home screen, tap the Application screen icon.
Find and tap Settings > Backup & reset > Factory data reset.
If you also want to erase data such as pictures and music, which is saved to your device's internal storage, mark the Erase internal storage checkbox (does not affect the SD card).
Tap Reset phone/tablet or Erase everything.
If required, draw your screen unlock pattern or enter your screen unlock password or PIN to continue.
To confirm, tap Erase everything.
If none of the steps previously taken solved the issue, in this case I will recommend you to perform a software repair to your phone, here are the steps on how to do it, we have an extended guide that should help you fix the Pattern Lock issue on your phone, please follow the instructions carefully:

Thanks for the information. I read something about the step 1, but i tried and no luck..I did a backup before resetting my sony z5p, but the backup could not be loaded because it is saying that the Android OS is old (I guess Safe-mode). Luckily the Backup & Restore inside the xperia phone backed up almost everything, just some apps' chat history is gone.
I'm very afraid of using the Ultra Stamina mode, even know i am certain of the error that i (might have) made.

Apparently quite a few Z3/Z2/Z1 users have also encountered the same problem - appears to be a Lollipop-related bug. I couldn't even backup as the only option under "Backup & Reset" was Factory Reset
try to enter Ultra Stamina again, and then quit it

I have the LG G4 (Verizon) which was upgraded to the stock Android 6.0 a few weeks ago. It is not rooted or modded. Last night I was sending some text messages when the battery died and the phone shut off. No problem, I'll just plug it in. However, when I did that the phone booted and said "updating app 1 of 1", which I thought was weird because it only does that when I upgrade Android versions.
So it boots into the home screen. I start playing around and here's what I notice:
--Wallpaper is different (stock)
--"Home" and "Show all apps" soft-buttons do not work. "Back" button works. (This is the biggest problem)
--Sound/haptic settings back to stock
--The top-bar swipe-down menu only contains current tasks, no buttons for location, airplane mode, hotspot, usb, etc
--Settings menu does not include option to connect to USB via different modes (Media, charge-only, etc). Connecting to a PC does not let me access to phone from the PC.
--My default text app was no longer the text app, although all my old texts are there
--Android version showed 6.0, and "last factory reset" was 3/6/16 at 10pm, which is weird because that is 36 hours in the future
I figured something got corrupted and I needed to do a factory reset. Upon trying to execute a factory reset from the settings menu, it displays the warnings and a list of my accounts that will be affected, but the "Reset Phone" button on the bottom is greyed out and will not let me click it. So I tried the other method....turned the phone off, held down power and vol down buttons, cycled power button when LG logo comes up. This causes it to hang there, then the Verizon logo comes up, then LG logo again, and will keep doing this back and forth as long as I cycle the power button on the LG logo. I never see a factory reset screen.
I backed up everything and need to nuke the phone or reinstall android somehow.....how do I do this without the ability to factory reset and without PC connectivity?? Or anything else I should try?? I don't want to take it to the Verizon store....

I've posted a similar message on another forum but we haven't been able to get close to solving this yet. A family member has an unrooted S5 which has had issues with Android updates in the past which were solved by using Kies. Since that time there have been no major issues. There has never been any physical damage to the phone. It is out of warranty but slightly half way through the 2 year contract with our carrier.
We put the phone on Ultra Power Save Mode 3 weeks ago Right away it shut down and kept restarting in Ultra Power Save Mode. There is very limited functionality (grey screen with most apps not available) in Ultra Power Save Mode and no way to remove this setting. The shutdown cycle is less than a minute after it starts.
It is important for me to try to protect the data on the phone so I have not done a factory reset yet. I've tried Safe Mode, Recovery Mode, clearing the cache, a soft reset, removing the battery, removing the SIM card, charging the battery fully, fully discharging the battery and none of this makes any difference. It was recommended that I reflash with the stock firmware and I did that through ODIN. The firmware installed, apps optimized, and then the problem started again. At no point has a computer recognized the phone so Kies and SmartSwitch have not worked. I now just keep the battery out so that the phone powers down and doesn't keep cycling.
Forums, including this site, have suggested various solutions. One suggestion was to reboot to a bootloader and type "am enable com.sec.android.emergencylauncher" but I can't find a way to do this. Another was to drag the Google app to the top and click "More" and "Delete updates". There was some suggestiong involving Google Now that I couldn't understand. I just have TouchWiz installed. There were other similar suggested solutions but I can't do any of those as I don't think I have installed the apps that were suggested.
Eventually I think this will lead to a hard brick but I'd like to get the data off before it happens. Thanks for any suggestions.
Try installing a custom recovery make a nandroid, then factory reset. For sure something is corrupted. There are too many apps to restore files after a factory reset.

So a customer brought a Note 7 from Verizon to me with demanding Google Account in order to continue setup. After the usual searches and clicks from forums to forums I was unable to solve the problem. I even booted the phone into download mode and tried to reset using Z3X Box (I choose SM-N920A) but still nothing. So, in disappointment, as I held down both Power and Volume Down buttons in order to reboot phone, something happened. I came across a menu I am kind of vaguely familiar with - the Maintenance Boot Menu.
Not only interesting is the fact that it has useful options like Safe Mode and Factory Reset, but the most interesting factor was the last option, option number 5
USB Debug Mode: Turn on USB debugging option under Developer Options.
I used the Volume buttons to navigate up and down and the Home Button to choose option 5 and the phone rebooted. I re-plugged the phone back to computer, waited for ADB drivers to install, fired up Samsung ToolPRO once more and choose SM-N920V (again!). I navigated to the Unlock tab and clicked Reset FRP. I said okay to the questions asked and waited for it to load. I still clicked OK despite the warning about wrong model number and just like that factory reset was done (By Samsung ToolPRO of course). I checked by phone and nothing happened!! What the- I took a deep breath and clicked Reset FRP, again. And just like that, the setup screen vanished. I went into menu and did a factory reset of the phone and now all is good. Cool huh?
NB: I know not everybody has Z3X Box for servicing phones but the essential is that ADB can be activated and that counts for something right?

The second to last day of my vacation at Walt Disney World (first time!), my S8+ started acting wonky. I tried to take pictures and the application wouldn't work right.
So, I rebooted.
Now, I'm staring at a screen with a textbox at the bottom of the screen and some text near it (I don't recall it verbatim) "Enter your emergency password".
What? What's an emergency password? Typing in the textbox, it was obvious that it wasn't a numeric-only textbox (for PINs) but it was alpha-numeric. I simply don't recall registering anything but a pin and my fingerprints.
Freaking out, I kept trying to restart and worked with the power and volume buttons.
All I continued to see was the Samsung logo.
Suddenly, I got a black screen with text telling me things were being erased. I then see a blue screen with an android bot telling me stuff was being erased.
After a bit, I was back at the language selection.
Gone. Pictures. Data. SSD... entire phone... fully erased. (Thankfully, a majority of my pictures were immediately put into Instagram... the lost pictures I used the phone's camera app because it works better than the Instagram camera).
Don't have my laptop... so I don't have my password database, so I cannot get into ANY applications.
I know there are requirements for Microsoft Exchange, and other applications that require special security for being a device administrator... I'm not aware of Exchange requiring a password for the phone but... who knows? I didn't enter the password wrong too many times.... even as hot as it was I still used my print to unlock the phone.
Man, that is horrible. My condolences. If you can boot the phone into recovery mode (Hold down volume up, then the Bixby button & then the power button...all at once). Once in recovery, do a factory reset and you shouldn't have to enter that 'Emergency Password'. Unfortunately, everything is lost unless you ran a backup into the cloud or on your 'puter.
Sorry for the post if you've already reset the phone as it sounds like you might have since you mentioned the language screen.

Yeah I didn't have the choice -- the phone decided to reset to a fresh install point. Once I got back home I restored to a previous backup after trying everything I could to undelete stuff on my SDCARD.... but none of the software I tried would work... kudos Samsung... your deletion of my SDCARD was VERY complete...
Did you have the SDCard stand alone or merged with the internal storage? I'm surprised that got wiped also.

The only reason I see for this to happen is if on the "lock Screen and Security", inside the "Secure Lock Settings" have enabled the "Auto factory reset".
Biometrics won't trigger a device wipe, they will only force the pin/password field on too many attempts.
You say Exchange. Is this a corporate account? Is there a possibility that someone remotely wiped your device? Log into OWA (the web interface of your Exchange), navigate to Options -> See All Options, then click the Phone tab and see if a wipe was sent to it. Because what you described sounds a lot like a remote wipe. (Though I've never seen the emergency password field.)
Also make sure that you're not violating your corporate policies. Granting admin access to the Exchange app gives your company full control over your device. They can see you accessing your email with it, and if you're doing something that you shouldn't be, they will wipe your device.

No way of knowing but I don't recall ever setting that (I wouldn't).
something corrupted your ROM it sounds like, somehow!
if it was the red box that says like "enter password" I believe it is actually "default_password"
