The message Unfortunately system ui has stopped keeps popping up and I am unable to do anything except press ok on messages. Need help fixing this so I can get back to using device. Deleting dalvik cache should work, I assume you don't have TWRP recovery or root access?
As for why, it may be because you reset the device while using a UI module from Xposed or magisk or something similar.

Device has been factory reset and everything is erased including settings. So how am I am to solve the system ui stopped
I think your only option is to flash your stock ROM or a custom ROM through fastboot.
Maybe one click root apps like kingoroot can root your device since it's old (4.2.2) but I highly doubt since you don't have USB debugging on.
If you do manage to root or install custom recovery then update here. Preferably custom recovery.

My Nexus 5 has been working pretty well until now. Earlier today I decided to disable lightflow and try the Xposed LED Control module. I installed and rebooted and it seemed to be fine. But then while using the phone shortly after unlocking it it completely froze then rebooted itself. Since then it's been stuck in a bootloop where it gets as far as the boot animation but then freezes for a few seconds and returns to the Google screen again.
I tried power off and back on and tried loading into recovery to see if wiping dalvik + cache might get me as far a boot but when it loads TWRP it asks me for a password (I've never set one nor enabled encryption of any kind) so my only option is to cancel but then it can't mount the partitions to wipe the cache.
I suspect it's the Xposed module I installed that caused this but I can't be sure it's not just coincidental. I haven't done anything else unusual on the phone today.
Is there anything I can do to fix this that won't require a complete factory reset? Is there any way I can at least backup or keep my data? Unfortunately I'm not at a computer where I can run or install ADB but if that would let me save my current setup or data I can do so at home tonight.

I know clearing the cache asks for a password, but try to see if you can flash something. The something is the Xposed disabler zip file in the root of your scard.

Maybe this..., the "Xposed-Disabler-Recovery.zip" should be at the root of /sdcard (at least for me it was the case).
You could try re-flashing recovery in fastboot mode to see if it fixes the password issue or also I believe philz touch recovery can read twrp backups if you want to switch to a different recovery.
The original bootloop seems like /data corruption to me (freeze + fsync perhaps), then the twrp password further evidences that but.... hmmmm

I think I was able to flash this. I keep getting errors about unable to open "/data/app-lib/com.h3r3t1c.onna" - rest off screen, then "Not a directory" and "unable to mount '/persist'", but it seemed to mount /system and flash successfully.
Unfortunately, this doesn't fix the bootloop.
I guess I'll try flashing the cache.img as suggested tonight and see if that works. If not, I guess I'll have to try and wipe.
If I do a factory wipe from TWRP, will I lose photos, videos I've taken? Restoring everything else will be a pain but it's really the photos that I haven't backed up yet I'm more concerned about.

I just installed that module today and its working fine. I'm on the stock ROM. But doesnt Xposed put an Xposed disabler flashable zip file on your sdcard? Or is that something that maybe I just transferred over and forgot that I did. Because I dont remember transferring it to my phone or downloading it but I have it on there. If you have it, maybe try flashing that to disable Xposed and see if it boots.

Wiping or flashing cache didn't help either.
I tried a factory wipe, which didn't help.
Then I formatted data from TWRP and was able to boot but there were strange issues including a complete inability to connect to the cell network.
Also, TWRP kept complaining that it couldn't mount /persist
So finally I ran flash-all.bat from the factory image download to try and completely return to a factory image. It seemed to work but then I got into a bootloop where it reboots after upgrading android then apps starting.
I tried to do it again but now fastboot keeps giving me this error: FAILED (remote: flash write failure)
or if I try to wipe: FAILED (remote: failed to erase partition)
I'm starting to thing maybe it's a hardware problem with the internal flash... Is that likely or is there still something I can do?
Will I be able to RMA it in this state?

First of all: I made a thread about bricking my Moto G earlier, but since this is a new issue (and a new Android version) I thought it would be better to make a new thread
So as some of you might have noticed yesterday there was the first release of CM13 for the Moto G and if you had nightlies enabled (like me) you'd just get it as a normal update, except for having to install the new Gapps of course. The problem is I didn't notice that it was CM13 until it was too late... First thing that happened was of course that my Google Play Services we're not working anymore. I tired flashing Gapps for 5.1 but it said that I used the Gapps for the wrong Android version. So I looked in the settings and noticed that I had Android 6.0 . Now I tried flashing OpenGapps for Android 6.0, which worked, but as soon as I booted Android it told me something like "Setup Assistant stopped working" over and over again, so I tried reverting to my latest CM12.1 backup I had, which was a bad idea... My phone was stuck in a bootloop and everytime I tried wiping my phone (except for internal storage) it rebooted when wiping data. Reflashing TWRP didn't help, so I tried flashing stock Android with mfastboot and it got to the point where it was optimizing apps but after that it just did the same thing again (classic boot loop).
Here's my question: How can I get TWRP working again and install CM13 (again if possible without wiping my internal storage or at least being able to back some files up) ?

I know but if I only wipe dalvik, cache and system it didn't work.
Now I tried booting into TWRP and it shows the "no command" screen :'(

Guys i have a similar problem here, i got the latest update downloaded (cm-13.0-20151207-NIGHTLY-peregrine.zip) and when i clicked on update, the phone went black and nothing work now, i cannot get into fastboot or anything else, it only show "qhsusb_bulk" when I plugged in to the computer, any help??? i would really appreciate it .

I thought that it must be the problem of the custom ROM and i try to flash other ROM but it seems that i can't proceed to the procedure of wipe data/factory reset because it kept restarting after i checked that option . wiping dalvik cache is fine though . I also did format/data on my solo attempt to fix it but still it won't work.After reading some forum I decided to flash stock firmware but it kept failing on Odin . anyone know what might be the problem of this ? Did I just hardbricked my tab to the point of can't be repaired ? thanks in advance
Try to delete everything in data manually through twrp file manager (Advanced->File Manager)

I can't change my rom, wipe / format doesn't work. Nothing changed after flashing with Odin.
16 GB Tab 2 have a known faulty EMMC (MAG2GA). It can happen, that your EMMC get “read only”, so you can’t perform any write actions (also you can’t format) anymore.
From the EMMC data sheet:
5.1.7 End of Life Management:
The end of device life time is defined when there is no more available reserved block for bad block management in the device. When the device reaches to end of its life time, device shall change its state to permanent write protection state. In this case, write operation is not allowed any more but read operation are still allowed.
But, reliability of the operation can not be guaranteed after end of life.
On a faulty EMMC firmware it happens a lot faster.
What are the symptoms?
Uninstalled apps and deleted files are back after a rebootFormating the storage doesn’t work (after reboot the old rom and apps are back)You can flash whatever you like via Odin - it showes “passed” but after reboot nothing changed (e.g. still no StockRom or still the old Recovery you had previously installed)… there might be some more, but only to mention some …
In most cases, the issue can be fixed by a EMMC firmware update, but you need someone who’s able to do it (possible via ISP, nothing you can do esylie at home yourself). In some bad cases you need to find someone who can replace your EMMC.

Hopefully you will be able to help me with this problem as I am currently without a phone until I can resolve this.
To run you through the process, I flashed Nitrogen OS, rebooted to recovery, flashed gapps and all was well. I decided to attempt to encrypt the device once I had setup my Google account. The battery was 100% and plugged into the charger. Shortly after the process started, the phone went into a bootloop. Since, I have attempted to wipe system , cache, dalvik and data, then reflashed Nitrogen OS, but it is still in a bootloop.
When I wipe system , cache, dalvik and data, I also get the error "Unable to mount storage" in TWRP.
I have even attempted to restore the backup I took before flashing Nitrogen OS, with no luck.
Any ideas?

If anyone else has this problem and looking for a solution, you need to completely reflash stock firmware via fastboot. PM me if you need any help or info regarding this.

i have yureka plus facing same problem.I was using aex5.2 rom .Once all the apps kept starting and so i rebooted. After that message came showing that your data is encrypted and you need to factory reset.I tried factory resetting through twrp but did not help.Error shows unable to mount system storage and cache.Tried all possible things through twrp bt no use.Did not use fastboot mode as my pcb is damaged.(only charging occurs, no data transfer.).Willing to use fastboot. please help.
Guys my D6653 is rooted and on eXistenZ Marshmallow firmware.
Dont know what went wrong (may be a wrong xposed module ro some app dont know) now whenever i reboot phone it keeps rebooting even after lockscreen appears. The only way to stop is immediately swipe on lockscreen to gain entry on to desktop.
Anyways theres lot of app clutter now in phone so would like to factory reset it.
But will just factory reset solve the issue or do i need to wipe dalvik and cache via twrp for clean rom??
I know I'm late on this but I feel you should factory reset your phone and install the rom all over again.
