Was it from a app? Did you maybe happen to back it up to the cloud?

OK, so, i know the topic sounds confusing. But my wife's mytouch 4g's battery died the other day. so, we charge it up then turn it on only to find out that every widget, every app, etc is gone from the screen. all that is left is the live wallpaper. apps and everything are still on the phone but all the links to the apps and various widgets are gone. just blank live wall paper screens. so, she manually puts everything back like it used to be, rebooted the phone and now everything is gone again. WTF? anyone know why it is doing this and how to make it not happen? she is against installing any custom ROMs (i'm working on bringing her to the dark side of android). thanks for the help.

The first thing you should try before you do anything else is a factory reset. This sounds like a pretty weird software bug. If it continues, call T-mobile and have them send a replacement.
Is this her first Android? Is she restoring data and settings from another phone through the use of MyBackUp or similar programs? Sometimes using these programs to restore data can cause strange behavior like this, especially if that data is coming from a different device.
On first boot after the factory reset let the phone sit for a solid 5 minutes before setting up. There is first boot "thinking" going on...
EDIT - if you lucked out and got the "good" screen and don't want to warranty replace it you could always root and use the stock nandroid restore found somewhere in the mytouch 4g development section. I'm fairly certain that a factory reset will solve your problems though.
that happen to me.... power the handheld down and let it sit a good few minutes and then boot it back up... all the homes screens should be back to normal.
idk whats up maybe its a bad batch of MyTouchs T-mo recently recieved but i had to send mine back as it kept rebooting itself multiple times a day
I'm not sure if this is a recent problem or if I'm just doing something wrong, but for some reason every time I've recently tried to select a picture on my device for use in another app it will only reset the app after selecting the picture. This has happened on both TweetCaster while trying to attach a photo and on Handcent while trying to Take a new picture which it acts like it does then as soon as I hit okay it resets the app and I guess deletes the picture because I can't find it, this also happens while trying to attach an already saved image and also trying to just set an image on my phone as a background on the conversation window.
Like I said I'm not sure what's going on, but if this can be fixed or anyone knows what's going on please let me know. My phone is not rooted because I'm in the process of contacting Google to replace it so I rather not root it ATM in case any one thinks that a rooting might be the source of the problem.

Usually I put my phone into aeroplane mode before I go to sleep, but last night I fell asleep before doing that.
When I woke up this morning and picked up my phone, all of the widgets were gone from my home screens apart from Google Newsstand. My wallpaper had reset back to the default Nexus 5 wallpaper. I use Apex Launcher, so at first I thought that maybe the phone had switched back to the default launcher somehow, which would explain the empty home screens but that wasn't the case - I was still in Apex. When I went into Settings, under Accounts there was nothing listed apart from my main Google account. eBay, SoundCloud, OneDrive, Dropbox etc. were all gone.
I rebooted the phone, hoping that that would somehow fix things, but it started up in Safe Mode and of course, everything was still missing. I turned it off again and turned it back on and this time it booted into normal mode, but it was still messed up. I ended up doing a factory reset and reinstalling everything.
Any idea what might have caused this?

I am constantly losing the weather info from the At A Glance widget and on the lockscreen. I never lose data and it's happening on both WiFi and mobile data (twice as often on mobile). I have reset the phone to factory default and had nothing installed on the phone so it's completely stock out of the box. Turning location off and back on doesn't fix it. The only way I can get it back is to add another copy of the At A Glance widget to the home screen which immediately brings it back, then delete that widget. Rinse and repeat a day later. Is anyone else having this issue and any idea how to fix it (if that's even possible)?
Yep, I have been facing that issue with the widget too. The weather simply disappears for no reason. But I had that issue with my previous Android One device too(Nokia 7.1). So I somehow feel the issue is with the widget itself and not much to do with the device.
A quick fix for this is adding the 'at a glance' widget to a blank space on your home screen, usually it brings the weather data back for me, but this is not permanent, it will disappear again some time.
This is going to sound silly but I gave this old phone to my mom who's been using it for just whatsapp and the odd call here and there and because her phone was getting full of photos, she decided to go ahead and delete some stuff to make room for more photos. I don't know how the software let her but she deleted the system keyboard
Now she can't get past the password lock screen since the keyboard nolonger pops up for her to enter her password. First thing I tried was an OTG cable but only a mouse works, a pc keybaord doesn't work and I've tried a few.
What is the best way to resolve this without having to do a full system reset?
Maybe you can try to go through safe mode? Im not to sure!
