When I select to reboot to recovery the phone attempts to reboot and never shows the google screen. I have to enter the bootloader and enter recovery from there. After i install what I need to install and click "reboot to system" in the recovery, the phone does the same thing. It just goes to a black screen. I have to re-enter the bootloader again and select "Start" to boot into the ROM.
What could be the issue here? Do I need to reflash the bootloader?
What rom are you on. Stock doesn't give you an option to boot into recovery. I'd try and re-flash the rom

PSX. This started to happen after I flashed CWM so I switched back to TWRP but it seems like clockwork might of screwed something up. Might have to flash the factory img.

I would try flashing the recovery image in fastboot. That should fix up the recovery issue. Cwm seems to have some issues right now.

That did not work. Still cant turn on the phone unless i go into the bootloader when attached to AC adapter and click start from there.

I would try flashing the entire factory image then. But i think you have something else going on if you need to have it plugged in to start it up
I just got a broken Nexus5 with a boot-loop issue. As soon as the Google logo appears it reboots. I can enter booloader normally but if I select recovery the same happens, reboot. I unlocked the bootloader and flashed TWRP in, same thing. Tried to flash full stock 5.1.1, same thing.
Any ideas?

Try flashing stock google image 5.1 with or without fastboot.

I did already as I mentioned. But I can't even enter TWRP, as soon I select recovery it reboots. Faulty board/storage?

You said 5.1.1 that is not out yet only 5.1. Also you do not need TWRP to flash google stock image, if you can get into bootloader as you said, you can flash the image with your PC. or am i missing something ?

Sorry I meant 5.1.0 from https://developers.google.com/android/nexus/images
I first flashed TWRP with the idea to install CM from there, but then I can't even enter it (all commands through fastboot work just fine), which is a pretty bad sign I guess. Then I flashed stock 5.1.0, and nothing changed.

I don't think so, phone stays in bootloader indefinitely until I reboot or try to enter recovery.
I have long been a user of CM11 on my OPO, but today I wanted to install a custom ROM. As the guide says here I was required to update to CM12.1 beforehand. The updates seem to have overwritten TWRP with Cyanogen Recovery, but whenever I try to flash TWRP back onto my device it always boots back to CM recovery when I boot recovery.
I have followed this guide to attempt to re-flash custom recovery and re-root my device but I seem to have hit a brick wall.
I used the following commands:
*fastboot flash recovery recovery.img
*fastboot reboot

I had this problem when I first tried rooting the phone. Some said that you had to reboot directly to recovery after you had flashed TWRP and then it should presist, but that didn't work for me. The solution that worked (at least for me) was flashing it with a toolkit.

If I power down the phone, after flashing recovery, and boot it with power+volume it stays on the OPO logo, screen goes black and reboots again - and if I do not hold the volume button it boots into the normal OS. If i keep holding the volume button the OPO logo keeps appearing and it keeps going to black and reboots again and again.

Thank you for all your responses. I made a thread over on /r/cyanogenmod too and found a solution.
I run the following commands instead:
fastboot flash recovery twrp-2.7.6.0-bacon.img
fastboot boot twrp-2.7.6.0-bacon.img

This booted into TWRP. Reboot prompted me to install SuperSU. Recovery now boots to TWRP. I'd link the thread but can't as a new user. Hopefully this method works for others too.

Did you try using the fastboot boot command instead like I did above?
I'll give it a shot next time I get. Hope it works.

Try doing what the poster directly above you did.
I finally managed to get it working by downloading the latest version of TWRP (2.8.3.0) and (if it helped) using the command 'format cache'.
It doesnt stick, but atleast it works.
Yesterday my phone died for no reason, and i've been trying everything i can to restore it...
What heleped me a little was the QFIL software, and downloading with it the Color OS.... the problem is that i can't access the fastboot, and the recovery boot only lets me wipe cache and data...
anyone knows how to acces the fast boot??? or is there a way to install CM12 and TWRP with QFIL??
I really need to preserve some data already in the phone, so i havent risked the chance to wipe everything...

If you're able to boot into recovery,why not in fastboot(vol+ + power)? That's strange.Also it is really a matter of doubt that you'll be able to save your data if you're not able to boot into fastboot mode.
Did you tried to flash another twrp recovery with Twrp?

i installed a mix of color OS + CM13 and getting into recovery and then advanced lets me reboot into fastboot, which is good... i installed TWRP, but the section with the "slide to confirm" in my phone is damaged (screen not working there) so.... i need to see how to flash everything via fastboot that it's what's been working for me...
any help witht his? tutorials everywhere list only how to install TWRPand then installing OS from there, which is impossible for my device

Are you using original USB cable? Try it a few more times by rebooting into fastboot mode again.

I'm glad it worked.
But I didn't thought that It was a driver issue as you were able to do other commands just fine but anyways the good thing is ,it worked.:fingers-crossed:

Cos13.1.x already has Gapps mate.Btw,im wondering how you use bacon with broken touch? Screen mirroring to PC?
Just FYI,lcd+digitizer replacement doesn't cost much and even you can replace it at home.I did myself a few months ago.

its not all the screen, just the bottom inch....
and i use it rotating it 180º, which had worked fine for over half a year now... i bought and replaced a screen once, but it only lasted 11 days...
anyway... if cos13.1.x has gapps, why does it stays in the boot logo forever? :/

You mean it didn't booted? FYI,Gapps has nothing to do with rom booting,they are optional.
First boot can usually take 20-30 mins.

Good day I have a problem with my idol 3 5.5 a few days ago I turned off and it took a long time to turn it on again, I did a formatting and it continued the same, it only vibrates when trying to turn it on and if I connect it to the charger or to the pc it turns on the LED but there not passed . I have tried to install several roms but when I finish installing it I get the reboot, I reboot and the cell phone goes off again. It is rare that the cell phone enters recovery mode. It comes with factory recovery.
If someone knows a solution or a rom that works very well that is to install directly from the recovery would thank you.

yes, recovery alcatel xDDD
I only give you the power button and volume + and enter recovery mode I give you install I select the folder where the ROM is installed I reboot it and the cell phone stays frozen.
I do not know if it's the rom euphoria that does not work or if in the end the cell phone is no longer useful, or if you have to have another recovery.

Last probable cause, Not having TWRP. You really need that on top of that, Unlocked bootloader.

That's to Unlock Bootloader and flash TWRP. Then install ROM.
