I can't reproduce it. I have 4 devices with 1.8.6 & custom codec. Everything works here.
If you are using any patches, it may be the reason.

Usually Patching MX Player to remove ads or to bypass license validation of Pro version it may lead to the similar issue. MX Player will close just after few secs of playback.

I buyed every App i have on my phone, if it is not a free app, im using Xprivacy to stop some apps to talk home etc.
Just saved the settings of MX and trying with cleared settings now.
Edit: Even with cleared settings (over the app and app settings) and with and without codec pack, it skips the playback after 2 seconds with hardware decoding.
Clearing Xprivacy settings now and try again.
Edit2: It keeps skipping/stopping, i downgrade now to an older version, thanks for your help!

Actually HW decoder is just wrapper of android's mediaplayer API. MX Player have only very few controls over it. If the issue is only with HW decoder it might be system issue.
If the issue persists, just collect the bugreport from help menu immediately after reproducing the issue. Share here. I will check it. If it can't be resolved I will intimate the developer as well.

1. HW+ is missing since the update, SW works. (Setting up to use only SW for decoding has no effect, i must change decode mode while playback the video)
2. HTC M8
3. no system update since month, MX Pro got updated via PlayStore
Can you try that video in stock video player or gallery. It should close the same way as mx closes.
I have faced similar situations in one of my device particularly with a video. Basically it's a system bug. It will start the video at the last frame & close. Only first frame will be shown to the user.
Anyway AFAIK, new HW+ is in testing phase. If everything goes as expected, Soon there will be an update (may be within a week..??) which will enable HW+ on HTC devices & devices running with Nougat.

I have got the same effect with all players now (Stock, MX, ES File Explorer), as you predicted.
Which just worked before updating to 1.8.6, which is a bit confusing.
Waiting or seeking in SW mode and switching back, will also close the video 2 seconds after switching back to HW.
Edit: Clearing cache in recovery now, as mentiont as fix for playback problems with HTC.
Edit2: Clearing cache (also ART/Dalvik) didn't helped!

I have been using MX player on all my phones since my first phone.
Recently I decided to run Cyanogen 11 custom ROM.
The list of videos displays fine. The player plays a heavily pixelated videos for a few seconds. Then the phone reboots.
The same cycle occurs again if I try to replay.
At first, I doubted my ROM. So I installed all the other video players from playstore. Surprisingly ALL of them play without any issue. Only MX force reboots.
I am running it with default settings. I even changed to SW from HW. Still reboots.
Device : LG L90 D-410
It should be hardware video decoder, I think this is rom related, you should report it to the developer of that rom.

That's because MX Player uses custom android-internal modules that most other players don't; it's how MX's performance is superior. If a ROM builder had a compile problem, that's likely the cause.

It is hard to find reason with the log because only 3 lines are from MX Player.
But it seems that you used HW decoder. HW decoder is just wraper of android.media.MediaPlayer java class.
MX Player does not touch any system internal if HW decoder is used.
In this case, some MediaPlayer function calls -- which is not used by other video players you've tried -- may cause this issue.
I suspect subtitle related function calls, because 1) many video players doesnot use MediaPlayer subtitle functions and 2) there is an error from MediaPalyer; 10-30 23:53:36.478: E/MediaPlayer(6097): Should have subtitle controller already set
If my assumption is correct, SW decoder should work correctly. It would be help if you send log after trying SW decoder.

Enabled full SW decoding.

What puzzled me is the last statement in the log. "AudioFlinger", call the process ID of mx player. And the system reboots.

Would you try after turning off "audio fade" settings on Settings > Audio > Fade on start/Fade on seek?
SW and HW decoder both changes volume rapidly to implement this feature.
HW and software does work in the last MX Player 1.7.33 but not HW+. It was working in the version before.
I lose the audio boost because this, unless I use software decoding. Using Galaxy note 10.1 2014 with Android 4.4.2 installed by Samsung. Reinstalling the codecs doesn't solve it.
Any way to solve it? If not, is there any way to get the version 1.7.32?. I payed the pro version in Google play but even the free version here is only available the version that crashes 1.7.33:

I would be happy using the version with ads even I payed the pro as long it doesn't crash.
It seems to be that builds before this 33 were removed because of a license issue with ac3 which have to be removed from now on from mxplayer itself and will be just used with the custom codec.

I hope this problem can be solved in the future or I will be stuck in that version. I can not find any other player in android remotely as good as this one.

@karlpj Would you send system log after crash using 1.7.33?
Actually not so many codes are changed for HW+ on 1.7.33. It is hard to find the reason because it is not happening on most devices.

The program initially doesn't completely crash, it says: "unfortunately, MX Player Pro has stopped" and returns to the main menu of MX after a while displaying a black screen. If you try again then the program disappear of the screen and returns to the desktop.

It looks an audio problem. I have enable H/W+ video in preferences but forced SW audio and it works, obviously using software audio. If I enable H/W+ audio again it crashes.
Actually there is a mistake in beta download links.
Free version downloads the pro & pro version downloads the free... ? ? ?
Btw, if you are still in beta
Disable "H/W+ decoder"
Enable "H/W+ audio on S/W decoder"
Play the video in S/W and check whether it crashes or not.

I installed the pro version.
Using your instructions it doesn't crash.
But it does as soon I return to H/W video and audio.

I have just tested myself. HW+ audio will be used with S/W only when it fails to decode. Since you were testing videos with aac/mp3 audio, it won't use H/W+ audio decoder.
That's why it hasn't crashed for you even after enabling H/W+ audio on S/W decoder.
It will crash only if you play any video with ac3 or dts audio track.
Since H/W+ audio causing issues to some specific devices, I can't reproduce it in my device.
In earlier versions H/W+ was using S/W audio. Till the issue is fixed, you can use H/W+ decoder with S/W audio. So, there won't be any difference in H/W+ decoder.
As you know the developer is already aware of this issue, hope he will fix the issue soon.

Are you using latest firmware? I got an SM-P600 (US edition, though) but can't reproduce this issue throughout 3 firwmare updates from 4.3 to 4.4.2
I have the SM-P600 with 4.4.2 No other firmware is available in UK, at least using the official Samsung update here.

i recently did a clean install of lollipop on my nexus 10 but noticed when in settings hw+ and hw decoder is enabled as default and when i play some xvid avi files, they show as hw+ or hw decoder at first but like in kitkat it used to switch to sw when hw+ or hw didnt work but in lollipop it doesnt switch and causes the player to be very laggy and after sometime crash. If i goto settings and disable hw+ or hw as default then it plays with sw at first and works fine. The switching from hw+ or hw to sw doesnt work as intended.
one more thing i noticed is under android developer options if i force gpu rendering for 2d drawings then it seems to switch from hw+ or hw to sw just fine without a hang so i guess that will give u a clue on where the bug is

Nexus 10 5.0.2 looks like have some issues on media framework.
xvid avi is not the only format media framework crashes.
I hope newly released 5.1 firmware works better..
Please use HW decoder for the time being, at lease HW looks like work better than HW+ as of now.

i tried android 5.1 today, HW works but when i select that the seek lags a lot and HW+ still dead on a mkv file so i guess the player needs to be checked as well. Atleast now the decoder changes with message HW+ or HW not supported but still lags a lot and temporarily freezes the player
I just purchased MX Player Pro from the Google Play store and have v1.7.40 (neon)installed on my Nexus 6. Since I have a lot of video files with AC3 audio, I installed the custom AC3 codec found here on XDA (automatic restart applied). After that, I configured MX Player Pro to use HW+ decoding and failover to HW decoding.
I can't seem to get videos to play at all. I open a video and it sits there like it's on pause. I've tried multiple video files and get the same result. Most are x.264, one is x.265. I've tried changing it to HW decoding as well as SW decoding but get the same result. I've force stopped the app and tried again after clearing the app data and telling it NOT to use the custom codec but still get the same result. Am I doing something wrong? I really like MX Player Pro but I'm going to need to refund it if I can't get it to work.
EDIT: On some videos that I start, if I manually fast forward, I don't see any video at all. On the x.265 video, I can manually fast forward and see the video frame of where it's at in the timeline but it just never plays.

On a side note, after launching MX Player Pro for the first time, I was not prompted to install the codec package so I installed it manually.

Well, assuming by "codec package" you mean the ARMv7 Codec package on the Play Store, you don't need these codec packages by default. On the contrary, the fact that you installed the custom codec may be what's causing the playback issue.
The Custom Codec on XDA here is purely for AC3 and DTS audio compatibility, and generally shouldn't affect video playback.
To be certain, are you using the stock Nexus 6 ROM? If yes then which build in particular?
Also, if you could collect an error log after it freezes, that would help.

H/W+ may cause issues with certain videos. I don't recommend it to use it as default.
Just try to start the video in S/W decoder (log press the video & from menu > play using > SW decoder.)
Don't resume if it asks.
Let me know the results.

As you know Custom codec is same as the inbuilt ffmpeg which comes with mx player itself. But, ac3 & dts support is enabled. So, it may not be the issue.
Both H/W+ & S/W decoders use ffmpeg. If the issue is there in S/W it might be an issue related to ffmpeg itself.

I'm running stock Android 5.1.1 on a non-rooted device, latest version, screenshot attached.
My first attempt was without the codec that's is available on the Play store, but with the custom AC3 codec. When that failed, I installed the codec from the Play store and got the same result. Then I tried it again after removing the custom AC3 codec (leaving the codec available from the Play store in place) and, again, got the same result. In a past experience of playing videos in MX Player with AC3 audio without the custom codec, the video would play without audio which is what I was hoping to achieve as a troubleshooting step (to see if the custom codec was causing a problem).
Currently, I have MX Player Pro installed with the codec that is available on the Play store as well as the custom codec to play AC3 audio. This is the config which we're basing our tests on as of now. Please let me know if I should remove the codec that I downloaded from the Play store. Also, I deleted the x.265 video that I mentioned in my previous post and obtained another video encoded in x.264. Regardless of this, it's important to note that I get the same result with ANY video I attempt to play. I'm attaching my loss for your review as requested. In this test, I have HW+ fully disabled in settings and SW decoding fully enabled. I also choose to start the video from the beginning rather than resume. Again, thanks so much for the help. Please let me know if there is anything else I can provide to help troubleshoot this issue.

The issue is not with MX Player.
MX Player is not getting audio focus.
Looks like some other app is trying to keep the audio focus. That's why mx player is not playing any video.
Goto Settings > Player & Disable "Play Alone"
Then try again.
Note:
If you disable the option "Play Alone" MX Player will keeps on playing the video regardless of the status. For e.g it won't pause if you receive a call. So, best solution is that find the app that is stealing the audio focus & uninstall it or if there is any particular Settings in that app kindly adjust it.

That did the trick, or it could be that I'm not currently using the phone that did the trick.... or both. VIC works while I'm on the phone so I guess I was expecting MX Player to do the same.
I'm going to leave Play Alone disabled since I like the option of playing audio our video when I'm on the phone.
Would it be too much for me to request an feature (toggle-able) to have MX Player to pause when it loses audio focus?

As in, using your version shouldn't in and of itself cause playback issues! As in, the issue is likely not caused by the fact that you are using the custom codec, but probably something else!

Enabling "Play Alone" does precisely just that? As in, make MX stop playing when it loses audio focus if another app plays...

This is what the issue is.
Just find out the app that steals/keeps audio focus. Disable it or uninstall it.
You can now play even when "Play Alone" is enabled.
This is used to pause MXPlayer when you loose audio focus (like call or notification or alarm or Playback from another player). That's why it's enabled by default.
It's not the fault of MX Player.

Thanks be to you, I found the app that's keeping audio focus is the phone. I like the feature of Play Alone that pauses MX Player when it loses audio focus. However, I it would also be nice if MX Player allowed me to play audio our video) while I'm currently in a call without having to toggle Play Alone on/off all of the time.

Regarding HW+ crash, it needs logs to be collected from ADB as a system reboot will erase everything. I will ask the developer to check it out as I don't have a pixel device to test it out.
Regarding HW issue, are you playing a HEVC on MKV container..? If it's so, It's an issue which is to be solved by google. HW decoder is wrapper of Android Mediappayer API. On Nexus/Pixel devices Google doesn't implemented HEVC on mkv container. If you just remux it to mp4 (I mean without re-encoding), the video can be played in HW. So, MX Player has nothing to do with HW decoder.

So what is the solution for this?
I myself having a OnePlus 3T and having same bug as crashing MX player and rebooting the device by itself when I play HEVC on mkv container.
What should I do?

Try again after enabling Alternative HW+.
Based on another report on another thread, It's basically a firmware issue & user has mentioned that leaked Nougat build has fixed the issue. Just be patient till the next system update.
