I know there is another very similar thread that was just opened(http://forum.xda-developers.com/oneplus-one/help/completely-stock-oneplus-one-stuck-t3396207), but his solution didn't help me so I'm making this thread.
I really need to save my internal data, so only as a last resort will I wipe userdata to save my phone.
First of all my phone is completely stock. Locked bootloader, unrooted, stock recovery, and stock rom. Last thursday(June 9th), I got the CM13.1 OTA update which I installed and everything went normal. It finished installing, restarted and booted up, optimized apps, rebooted, but then optimized apps again and again. I thought it might help to just reboot my phone, but it didn't. So I immediately started looking around for solutions.
I can boot into recovery and fastboot mode and since I can boot into recovery I started with wiping cache(Stock Recovery>>Factory reset>>Wipe cache partition). It took ~20mins which seemed like an unusually long time, then I rebooted and it still got stuck in the "optimizing apps/reboot loop". Next, I set up USB drivers, ADB, and fastboot since it seems to be very important to have those set up. I used a few guides to get them properly set up and I checked that they were correct by checking that the cmds "adb devices" and "fastboot devices" returned my device, which they did. I saw someone saying he saved his friends OPO by just ADB sideloading a CM zip file, so it looked hopeful in my case. I downloaded cm 13.0 and 13.1 zip files and tried sideloading them with the cmd "adb sideload cm13.zip". I tried both and they both got to around 50% complete and the install failed. Some of the red text said, "Package expects build fingerprint of oneplus/bacon/A0001:5.1:6.0.1..... & E:Failed to install /sideload/packages.zip ". Next I tried installing TWRP, which looks like it requires an unlocked bootloader to flash it(even to to just boot into it). From all my research it looks like there's no way to unlock the bootloader without wiping data. Even the OPO toolkit says you need to wipe data to unlock your bootloader.
One thing I haven't looked too much into is using fastboot to save my device. This guide http://www.technobuzz.net/fix-bricked-dead-oneplus-one-easily/ shows to flash userdata userdata_64g.img, which I assume wipes userdata.
Maybe I'm just overlooking some simple fix, but if anyone can help me I would be eternally grateful.
Edit: I need my phone working and I'm tired of waiting for help, so I just did a factory rest. Also I've been getting tons of conflicting advice. Thanks everyone for the help anyways

download and install the 15 second adb/fastboot installer (http://forum.xda-developers.com/showthread.php?t=2588979)
It'll create a folder: C:/adb, In that folder you can open a cmd prompt window to send cmds to your device through adb/fastboot. that folder is also where you place files that you want to flash or boot into.
Also for the USB drivers just download the oneplus one toolkit, or use this guide: http://www.technobuzz.net/install-oneplus-one-usb-drivers-windows/
the opo toolkit is in the 2nd method
Still looking for help. Phone status: After sideloading cm 12.1 it boots up but I get tons of errors saying com.android.phone failed/ android.system.acore failed which make my phone inoperable, also my phone usually reboots after ~10 seconds after booting up and getting errors.
when you flashed the stock zip via sideloading did you use the full zip or the incremental zip?

You will have to flash the latest version as performing a downgrade without wiping data will cause issues like the ones you are seeing

I can't really find a full cm13.1 zip. On CyanogenMod I only see up to 13.0. I also don't know how to tell the difference between an incremental and full zip D:

I just got around to sideloading the cm 13.1 zip. I'm still getting a bootloop /:
Edit: My phone reboots and optimizes apps after getting hung up on the cyanogen boot screen for a while.
I'm in the same situation.
It seems like a cache cleaning is needed but I have no idea whether it possible without a custom recovery and without a full adb support.

You can wipe cache partition in stock recovery also. To boot into recovery mode first power off your device. Press Volume down + Power button together until you see oneplus logo. When device is in recovery mode press volume down and select 'wipe cache partition' and press power button to confirm the action. Reboot after process completes.

The same like woodhaus I have locked bootloader and there is no USB debug over adb.
I don't have a 'wipe cache partition' in the recovery.
The only things I can use are adb sideload and fastboot.

Alright, so, my tablet decided to one day just cease stop working. The recovery for some reason can't flash anything now, is used to be able to flash a backup, but not anymore. It used to be able to boot past the animation, but not anymore. I cannot flash a ROM from recovery because it says that the file is bad (and please, don't tell me to check to see if the md5's are correct, they are, I've triple checked.) I've flashed CWM recovery and TWRP recovery, and TWRP cannot fix permissions, but CWM recovery can, but neither can flash a single ROM. I've tried sideloading a rom but i get this error from the CMD window

You have flashed the latest TWRP via fastboot and it still wont flash roms? Also, what bootloader are you on?

What tablet do you have?
From where did you download which recovery for which Asus tablet.
Sounds like the recovery version doesn't match the device.

Asus Tf700 32 GB. I got the recovery from the TWRP website. In answer to the boot loader question, I have no idea how to check. All I know is that I can choose between RCK, Cold Boot, Fast boot and Wipe Data.

Have you ever unlocked the bootloader? (If not: do so by searching on Asus' site for the Unlock Bootloader Tool v7) It's a simple apk which you install and then check and let it unlock.
Only then, you can start flashing!
If you already have unlocked, maybe you sent it in (?) Or flashed an official Asus rom (?), you might want to unlock it again.
Let us know if any of these answers above helped

If you have the 4 icons that indicates JB.
Sounds like the custom recovery is not installed if your on the RCK icon and press the up volume rocker and nothing happens.

Unlocking it would require an OS to be installed, but none is installed because I was wiping to install a different ROM. I have unlocked it. In response to the boot loader, yes, it has 4 icons. So I guess it is the JB boot loader. When I go to RCK it does boot recovery but from there on its unable to open any zips besides any backups I have. When I reload a backup it doesn't install the OS, but it restores everything else.

Can you get adb shell access to your recovery?
Which ZIP are you actually trying to flash? What is the exact error message from the recovery? Try running md5sum (inside the adb shell) on the file you want to flash. Check the output of "mount". Check if /cache/recovery/log contains interesting info.

Alright, I'll have to try that when I get home in about 3-4 hours. I'll get back to you on that. Also, I noticed last night that when I was on fast boot, it would stay stuck as waiting for device on the cmd window. I'm not sure if it was my computer that didn't detect it or a new symptom. O hope its just a fluke!

OK, I assumed that you're not using data2sd. 1. Make sure your microsdcard is good (reformat and recopy your backup or rom if needed),2. Boot into twrp and wipe cache, dalvik cache, factory reset, wipe system, wipe internal then finally the most important part, FORMAT SDCARD. Now reflash a new rom or try to restore from backup. I would try step#2 first. Good luck.
EDIT: format sdcard instead of wipe sdcard.

I'm trying to flash an update to PAC Rom, but I can't get my phone to reboot to recovery (or even shut down).
When I use the power menu, no matter what I choose, the phone does a hot boot. When I hold power and volume down it will go to bootloader then I can access TWRP, but I can't clear dalvik or cache as it says that it can't mount the partitions. The ROM runs fine however and I can access all my data so the cache can't be corrupt.
What can I do here? Thanks to those who help!
Sounds like somethings wrong with your recovery. Get into the bootloader and reflash TWRP using "fastboot flash recovery FileNameOfTWRP.img"

Yea that should work too, but if not, fastboot is the way to go.
EDIT : Very very strange. Flashing TWRP again via Flashify got it to work. It seems something in PAC Rom is corrupting the recovery. I think if I get this all worked out then I'm going to have to go back to DK until it's all worked out. I wish there was another ROMs with all the same features as these two but got updates (whereas DK doesn't any more) and is stable enough to DD (lots of FC with PAC).
EDIT 2: Just tried to reboot into recovery again. After booting teh ROM just for a few minutes the recovery was corrupted. I can't access internal memory from recovery any more, even if I get it to boot. It doesn't make any sense as the ROM has full access to it all.
To be clear, TWRP says Internal Storage - 0

I actually used to go around the Q&A forum telling people to do just that when they were having issues with wiping on early TWRP versions. Surprised I didn't think to try it. All back on DK now and hunting for a new ROM. :victory:

Sounds like you solved it, but I think you could've also done an adb reboot recovery command. Not sure if that works though since I usually only use fastboot.

After flashing teh recovery again, I could get INTO it, but the data was not showing up so I couldn't actually do anything from within TWRP. It read I had 0mb internal storage.
Hey everyone,
Yesterday i tried flashing the latest CM12 nightly straight onto my Moto G over stock KitKat. After i flashed the ROM, i tried booting and noticed it was taking a lot longer than normal to boot. Ten minutes had gone past so i tried rebooting it again and the same thing happened, just got stuck booting for ages.
So i decided to boot into recovery and do a factory reset to see if this would make any difference. But even after doing this the phone still wouldn't boot, so i decided to just flash the stock KitKat ROM back onto the phone and forget about all the Lollipop stuff.
The KitKat ROM wasn't on my phones storage so i downloaded it from the internet and flashed it via fastboot using and included Windows batch file. I've used this method numerous times to get me out of situations like this and it's always worked, but this time no!
Tried booting after flashing the new stock ROM and now it just keeps going to the 'Warning, bootloader unlocked' screen and then booting back in a loop.
When i boot into ClockworkMod recovery it gives me these errors:
Can't mount /cache/recovery/log
Can't open /cache/recovery/log
Can't mount /cache/recovery/last_log
Can't open /cache/recovery/last_log
Can't mount /cache/recovery/last_install
Can't open /cache/recovery/last_install
And if i attempt to format /data, it says this:
Error mounting /data
Error formatting /data
I hate asking for help like this but unfortunately i don't have much option this time, no idea what to do despite searching forums for many hours.
Please help, any advice would be greatly appreciated!

You can try getting into fastboot and run commands fastboot erase userdata and fastboot erase cache from your computer to pretty much factory reset the phone.

Thanks for the response. I tried entering the commands you suggested but the problem is still the same, phone stuck in the 'Warning, bootloader unlocked' loop. Does anyone have any other ideas? Really need some help with this, my little Moto has been dead for days!

Yeah it sucks having a pretty much lifeless Moto, kinda scary as well not knowing if i'll be able to get it working again! But in response to your questions, i can turn it off and get into fastboot, i'm also able to boot into my recovery which is Philz Touch (ClockworkMod). If you take a quick look at the original post, i updated it a short while ago to show more details about the errors which are displayed.

If you can get into fastboot you can flash the stock firmware (not backup) back onto your phone that way. I have used fastboot many times to restore my xt1028 Moto G many times after seemingly bricking it. You will need the stock firmware for your phone to do this.

Thanks for taking the time to respond to my request for help. I have already tried flashing the stock ROM via fastboot mode but it makes no difference to the situation, even though the new ROM flashes perfectly without error, when i try to boot it just gets stuck in the reboot loop on the 'Warning, bootloader unlocked' screen. I have no idea what to do now!

I would suggest you try to install a custom recovery through fastboot (I use TWRP 2.7.1.0)
Just replace the stock recovery with TWRP in the fastboot file and use the command "flash recovery recovery.img" then reboot into recovery(hold power and volume down button down together for about ten seconds, release and you should see the fastboot screen. Then press recovery and then wipe>advanced>wipe ONLY cache and dalvik and then reboot)
See if that does the trick.

Thanks very much for the responses. Unfortunately i already had Philz ClockwordMod recovery installed on the phone and had attempted the various wipe options a number of times without any success. However, i thought that as i'd only tried flashing the UK ROM to the device, i would attempt flashing a different ROM just to ensure there wasn't an issue here somewhere. I downloaded the stock US ROM and after extracting the archive i ran the Windows batch file to flash the files onto the phone. After the process had completed i rebooted the phone (Didn't bother going into recovery and wiping/formatting cache) and to my surprise the phone booted up completely fine! So weirdly it seems that just flashing a different ROM did the trick. But anyway, my Moto is alive again now, i'm very pleased to say the least!

yes another bootloop,
current state: power on device>starts up>starts to "optimize apps"> finishes and wants to boot to system but restarts entire process

but like i said it was completely stock so i didnt have USB debugging enabled on the phone so i think thats why windows device manager isnt recognising my phone as a adb bridge, and i cant connect to it
i think thats the problem
windows file explorer also isnt showing anything
i tested fastboot and i can command the phone that way but im not sure what to do next
i want to keep my data intact however
is there any possible solution to this?

After that you must configure you device again, but the data on you internal storage would not be deleted.

I'm pretty sure factory reset deletes your data. Isn't that what it's supposed to do anyways? Maybe he's thinking of a custom recovery? If you go into factory reset you can wipe your cache without messing with your data.

Yeah I already tried that and it had no effect
So was it as i did it yesterday. All my music files, my eBooks, my fotos were not deleted. When you have synchronized your contacts, fotos or whatever

Hey, I just tried to sideload that file, It didn't fail but it just abruptly stopped and went to the stock recovery's main menu at like 50%. Is it supposed to say it finished?

Currently my phone boots up but I get an error saying com.android.phone & android.process.acore failed and my phone just restarts. So It boots, but I cant use it because the errors constantly pop up..
EDIT:Also, just for some more info, I sideloaded cm 13.1 and that's when I get into the bootloop(optimizing apps/reboot) but then I tried sideloading cm 12.1 which allowed my phone to boot up and I was able to open my lock screen but then I get all those error pop ups, which makes it impossible to click anything. when I click an app it just restarts my phone, so i'm pretty sure it's some sort of incompatibility with cm versions. maybe I should sideload cm 13? maybe It would help to get a link to the correct sideload-able zip of cm 13 and I'll try that.

Ah well I didn't know there was a difference.. The zip I downloaded was this (http://builds.cyngn.com/cyanogen-os...be76f9310/cm-12.1-YOG4PAS1N0-bacon-signed.zip)
cm-12.1-YOG4PAS1N0-bacon-signed.zip - so cm Im guessing is cyanogenmod. So for a cyanogenmod zip I need to sideload a cm12.1 compatible gapps? and as for clearing contact data, I would be able to but the errors kinda prevent me from using my phone at all.

Just tap ok when message appear and quick and patient step by step navigate to settings /apps and clear contact data.

Ok so I've been trying to do that, but it's really not possible. The errors pop up endlessly and the phone reboots after 10-15 seconds of turning on.

I'm in the same exact situation as the OP except that my phone WAS rooted with TWRP recovery and ADB debugging was enabled before it entered a bootloop. I cannot manually enter TWRP using hardware keys (it just shows a black screen), but I can do it using "fastboot boot recovery.img". I can also obviously enter fastboot mode. I was intitially trying to upgrade my phone to the latest COS 13.1 from COS 12.1 (skipping COS13.0). I was told that the problem might have happened because I allowed TWRP to install SU after flashing COS 13.1, but I don't see how that could have been the cause because I have done it with every update in the past without a problem.
I just remembered that I did a nandroid backup last February so I flashed it and have now been waiting for 3 minutes for my phone to leave the boot screen (Cyanogen letters w/ black background), but it doesn't seem like it's going to leave the screen anytime soon.
Please help.

Hello, I've been trying to install Cyanogenmod 12 but every time Rom Manager does nothing when I flash installation or with other options I can't go into recovery mode since it just reboots every time!
I have Sony Xperia T with Android 4.3 and I need newer version to install Pokemon Go!
I have tried many guides so pls don't post me links but just write me step by step what to do or tell me what I do wrong!
1. I have unlocked bootloader on my phone, my camera still works despite what the guide said. When I go into SERVICE in phone it says it's unlocked.
2. At this point I tried installing TWRP but to my finding I need to root my phone! Wtf there was nothing about this in the guides... But okay, I look in Internet and install KingRoot that automatically unlocks the root. Now I can install apps that require root.
3. I install Twrp, it ask me if I wanna install Bombox or something like that but I press no, I go into backup and whenever I try to do it, it does nothing. I restart the app and this time I try installation without any backup. I select the zip file of CM for my phone, select to wipe the data etc and start. But it does nothing and just freezes :/
4. I try Rom Manager, I install it try to do backup but it tells me to install something but later it says my device is unsupported?! I try installing just the Rom zip but again nothing...
Whenever I try to go into recovery mode through any apps it just restarts my phone!
Pls help I don't know what to do...

Then I have option to cancel or to reboot now but if I press it then it freezes and I can only close the app.

There are plenty of occurrences in TWRP where it will show that recovery was not successfully installed. Once TWRP Manager shows you that message, close the app and shut down your device and then try to boot to recovery using the correct button sequence. I believe for your device that the correct button sequence is as follows:
Power off your device.
Boot in*recovery mode*by pressing and holding Volume Up and Power*Button*at the same time.
Release all buttons when the Sony Xperia T screen flickers and in a few seconds the Recovery screen will appear.
Give this a try.

Well, I suspect that if TWRP Manager cannot install recovery on your device then you will have to download the correct TWRP from www.twrp.me and fastboot flash recovery on your device.

Just to make sure, you can still boot the device normally into 4.3 JellyBean, right?
Okay, have you ever used ADB or fastboot?
Have you installed a program on your PC like
Minimal ADB & Fastboot or 15 Second ADB installer or the Android SDK?
To verify that your ADB & fastboot is installed and working correctly follow these steps:
Boot your device normally.
Go to Developer Options and verify you have OEM Unlock enabled and USB Debugging enabled.
Connect the device to your PC with the USB cable.
Once connected to the PC you should see a pop-up message on the device asking for authorization. If so, check the box to "always allow".
Open the Device Manager on your PC and look to see if your device is recognized. (if you see a yellow triangle or question mark the correct drivers need to be installed)
So, if your device is recognized and the correct drivers are installed, open a command prompt window and type this:
adb devices
If your ADB is setup and working and your device is properly recognized the command prompt will return with a list of attached devices and the serial number of the device.
This is where we need to start that way we know the ADB and fastboot environment is setup and working correctly.
Also, I was looking around the XDA Xperia forums and there are some good guides on how to do this just in case you want to read up on the subject.

To unlock bootloader I was using some fastboot program which was in a guide and I had to go to cmd and just type fastboot.exe oem unlock boot 0xKEY or something like that and then I just installed app that rooted my phone and that's it. Nothing more. I didn't install any other things since I have problems with Internet :/
Also, I was looking around the XDA Xperia forums and there are some good guides on how to do this just in case you want to read up on the subject.

Quick question. Which recovery did you install? Philz or TWRP?
Good job on getting into recovery. First, I would take a backup. That way if anything goes wrong you can boot into recovery and restore your backup and have a working device again. After your backup, when you install a ROM+GAPPS you will need to perform a full wipe. This includes:
Dalvik Cache
System
Data
Cache
Do not select your internal storage unless you have your ROM + GAPPS saved on a micro SD Card or USB OTG drive. If you have your ROM+ GAPPS saved to your internal storage and you wipe it you will not have an OS to install. Just be careful when selecting partitions to wipe. Let me know if you need any help.

I have successfully installed Cyanogenmode which Im grateful!
But I Have a problem, I have installed gapps but when i go into phone i can only see Cyanogenmode apps and none from google! I'm running gapps installation again, maybe it will help.
After everything is okay I can wipe whole system with internal storage right? I think it switched to TWRP after I have booted into cyanogenmode successfully.

I would dirty flash the ROM + GAPPS again without wiping anything and then boot normally and see if you have your Google Apps. Also, don't wipe your system unless you have another ROM ready to install. Also, don't wipe your internal storage and your system at the same time unless you have a ROM saved on a micro SD Card or USB OTG drive. The only time you should wipe the internal storage is when you are doing a completely clean installation and you want your device to be in a fully stock new state.

1. I have installed CM 12 after i wiped everything except internal storage.
2. I powered on the device and I had only one msg that 'Settings' stopped which i just press okay and everything is working.
3. then I tried installing many different versions like aroma, full, mini but every time after successful installation whenever i power on my phone i get all those pop up messages and I cant get rid of them and do anything...
Dont know what to do anymore

If you are having a lot of apps force closing I suggest finding a different ROM to install. Maybe a stock style ROM that has Gapps built in. Try several ROM's. You may find one you like better.
