I'm trying to identify this calculator app (pic attached), and where I can obtain it from.. please help!!

That's "my calculator" from the Samsung gear app store. I use it all the time.... Atleast that's what it looks like

Oh dear....perhaps you should ask the owner of that forum where the Phone Manager apk is.

its strange to me. at least a month ago i tried but it wasnt working as expected.

may I ask, did you config any specific options (before cloning) or leave them all default?

you can use Aurora Store Or F-Droid instead
