Just like the title says. When I receive a call from an unknown caller, the phone keeps ringing, even when answered. I can't take the call and have to reboot and call them back. Any ideas?

I just recently upgraded my phone to Gingerbread, since then i have been having problem to receive an Incoming calls
when I have an incoming call, the phone does not ring or, vibrate or even show the incoming call screen. the only thing I can see is the little icon next to 3G icon on the top of the screen showing there is an incoming call and the screen will be blanck. non either call accept or reject page will show instead it will show blank page and the network bar will be lost.
so I can get messages. I can dial out without a problem.

I just got a Motorola Droid Ultra XT1080. When it was first set up my phone rang with each incoming call. Then all of the sudden (something I did, I am sure) it started announcing when I got an income text, a feature I didn't want. I played around with it and somehow I got it to stop doing that! HOWEVER, now my incoming calls don't ring at all. I get audible notification when I get a voicemail message, receive texts, facebook and email notifications, etc. I have checked my settings and the volume is turned up, ringtone selected and such. Also, if I call my number from another phone it goes straight to voicemail and (as the caller) there is no ringing - just voicemail immediately.
Can anyone give me advice/input on why my phone no longer rings with an incoming call and why every call goes straight to voicemail???

I've got a problem when answering the call. Most of the time when I take the call the other participant cannot hear me unless 3-5 secs pass. It is as though I put an incoming call on hold and after 3-5 secs pass I am able to continue conversation normally. During that mute period I keep saying 'hello, hello" but the caller can't hear me. I've got stock rom, no modification, regular updates but this problem is persistent. This also happens from time to time when I make an outgoing call but it is more frequent in incoming calls. Please help. Thanx!
When I was CM11 while I was on phone, when apps get notifications, phone vibrated. Now with marshmallow on Nexus 5 doesn't vibrate. This function is useful to me while I'm on call, because it allows me to see if I'm getting another call when busy as I receive sms alert. How can I do to reactivate this?

I'm on GS7, MM 6.0.1, rooted/xposed.
Whenever someone calls me, even if that call is blocked and sent straight to voicemail, vibrate will turn off.
I've looked through the settings and cannot figure out why this is happening.
Any ideas?
EDIT: After some more testing, it seems that vibrate turns off whenever a call is ended, regardless of how. If I get a call that's automatically rejected/blocked, that 'ends' the call and vibrate turns off. If I call out to someone else and the call ends, vibrate turns off. This is REALLY annoying.
