Stuck in recovery after failed update via TWRP - ONE Q&A, Help & Troubleshooting

yesterday I bought a used oneplus one.
It was showing me a notification that I can do the incremental update 12.1-yog4pas2ql. So I tried it. But after the restart it booted into TWRP (2.8.5.1) recovery.
Then I tried to flash the update (cm-bacon-7be76f9310-to-49d90510ef-signed.zip) with TWRP. But, as I now know, I haven't set in developer options the option not to update the recovery.
The result is that at startup it now boots automatically into to stock cyanogen recovery. When I try to "apply update" I don't see my cm-bacon-7be76f9310-to-49d90510ef-signed.zip in the folder. I only can find a cm-12.1.zip which seems to be from the previous owner.
When I select "Reboot to bootloader" it shows "fastboot mode" and is stuck in that screen. I can't do anything but to turn it off.
I have rooted a Moto G before and I know I can do it when I know what to do. I'm completely unsure: should I select "factory reset" in the stock cyanogen recovery? I just need a starting point. I don't need TWRP anyway, maybe it's best to get rid of it?

[Q] I think I may have unlocked my bootloader twice...

I tried to use the search and google but didn't really find anything helpful. Almost 2 years ago, I unlocked the bootloader back on honeycomb and did custom recovery, root, and custom rom. Then a little while after ICS came out, I did the OTA update.
Today, I reset the xoom back to stock ICS and tried to unlock the bootloader again...thinking nothing of it. The process was taking a few minutes so I decided to get a snack and when I came back it was rebooting.
I assumed that it worked and continued to try and fastboot a recovery image. The recovery image installed and I did another reboot but was greeted with the android welcome screen. Anyways I went through the menus and then tried to enter recovery and it gave me an android laying on its back with and exclamation point above his open chest.
I hard rebooted and can still get into my device, but adb cannot seem to find my xoom anymore. I have tried it on a 64bit desktop and laptop with no luck.
Have I messed up? Or is there a simple fix that I am missing? Thanks in advance for any help!
Can you get into fastboot mode? The second time you unlocked did you have to go through the questions on your xoom again?

Yes, I can get into fastboot by holding the volume down key and the power button. That's fastboot, right?
Yes, I had to go through the menus the second time I unlocked.
sounds like the recovery you try to fastboot did not take or the rom was corrupt.. Which recovery was it? Do you want to be rooted and run a custom rom or completely stock? what xoom do you have(wifi, 3g, 4g)

Download your rom choice, gapps and put them on the root of your sd card. If they are already there skip this step

open a cmd prompt in that o that folder
type fastboot flash recovery TWRPv2.4.1.0.img
Once you have verification on your device that this has been flashed, reboot your device by holding volume up + power.
Then 3 seconds after you see the Motorola logo, hit volume down. You should now see "Android Recovery" in the top left corner. Hit volume up to boot into recovery.
You should be in twrp 2.4
select backup/restore and backup your current system
select install and find your rom select asnd then swipe to flash
when done then select home
select install again and find the gapps package and install
when done select wipe dalvick/cache
when done select reboot
select system
you should be good to go.
Well I got through all of the steps, but when I try to boot I get stuck in a loop of the roms boot logo. Is it okay to flash a wingray rom over a stingray device?

I think it is fine just the 3g/4g won't work.
I can get you going again but you are going to be starting with a locked honeycomb device. You will then have to either let it update to ICS or JB or unlock, root, install custom recovery and flash a rom. What xoom do you have 3g or 4g?

Okay I restored it again but this time i did a factory reset and wipe dalvick before flashing the rom and gapps. It works! Thank you for your help!
However, could I ask how to root this thing?
since you have a custom recovery on it already and are unlocked.
Download this file Universal xoom root place it on the root of your sd card and flash it. Then reboot and try a root required app such as root checker

[Q] locked screen

Morning. Was wondering if anyone can help me. I have a verizon HTC One M8 running 4.4.3 rooted. I was loading Xposed apps last night, selected them in the framer and attempted a reboot. Now the phone locks at the verizon screen. Do not know how to get past it. Any suggestions?
There is a zip file automatically created by xposed that can be flashed in recovery to disable it in these very situations. I don't know its name or location but I'm sure you can Google it. Go to "install a zip" from your custom recovery and drill down to that folder to find the file, flash it, and you should be able to boot up and disable whatever went wrong in xposed.
If you don't already know, you can get into recovery during a bootloop by forcing a reboot with power+volume up and as soon as it starts to reboot switch to power+volume down. That gets you into bootloader where you can select recovery.
That is what I was looking for in an answer. My only problem right now is that when I try a hard reset it is just going through the normal boot and locking on the Verizon screen.
I believe that the FastBoot may have been enabled. I think that is my problem. But I don't know how to get by it.
I was finally able to get it to reset. But I couldn't find the zip file option. I just TWRP reset. It's working.

I can't root my OS12 !

And I have no idea why. Before I downloaded the newest update I upgraded my TWRP to the latest version. After the update I tried first to install SuperSU via google store but didnt work so i downloaded the installable zip and tried through TWRP. All I get is : E: failed to verify whole-file signature; E: signature verification failed.
Tried the old SuperSu that always worked (2.02) but I get the same bug.
I think the problem is that instead of TWRP when i start my phone with start button + vol down it gets into CYANOGEN recovery. I just installed TWRP ver 2.8.5.1 but its not working =/ dont know what to do, help please

You need to resinstall TWRP, just after you have done so, reboot back to TWRP instead of rebooting the whole system. Once back in TWRP you can flash SuperSU again, then reboot and you should be fine. Also make sure you have the cyanogen recovery option unticked under developer settings.

How do i reinstall TWRP? im pretty sure ive done it already but still when i turn on the phone I have the cyanogen recovery on, tried to do it with TWRP manager (at first i couldnt start it because but later it worked. However, when i click on "Reboot to Recovery" it does not work) I have recovery installed in TWRP manager (ver 2.8.6.0) maybe i can do sth with this ?
Edit: I was able to start TWRP manager, now I'm not again because it says "root is required for this app"

Once it flashes, are you rebooting back into recovery first (without switching off, fully rebooting, anything else)? You should boot into TWRP and then you can reboot fully, it should be fine. If you just reboot out of TWRP after flashing the it gets overwritten by the stock cyanogen recovery.
After completing the TWRP flash in ABD run the following command.

Im doing what Im told, so - After the phone has booted back up, turn it off. Now to enter your newly installed custom recovery, hold volume down + power. The phone should boot into recovery mode.
Wish I wasnt so braindead on this field =/ and the cyanogen recovery welcomes me every time

Downloaded new drivers for pc and new fastboot, tried like 15times to flash twrp on my phone and seemingly it worked but when I rebooted, turned it off and on again with volume - it's always cyanogen recovery there, never twrp. I downloaded twrp manager but it won't work because it requires root and I can't root because manager won't work. Saw on oneplus forum that some people have exact same problem but there S been no solution so far and I have no idea what to do anymore. The only thing I haven't tried yet is to hold start+volume- keys after I flash twrp and reboot the phone using the code, so instead of having to turn it off and on again maybe I ll be able to get into the twrp instantly and it won't get overwritten by cyanogen recovery.

OK so i am experiencing the EXACT same issues. When I go into Fastboot Mode all I get is a small cyanogen head forllowed by the words 'FASTBOOT MODE' I get no menu to navigate or anything. TWRP appears to install but I get Cyanogen recover every time. If I use the toolkit and go to fastboot mode it sees it as an unlocked fastboot device but will not allow me to root the device.

After flashboot reboot do not let phone turn on again but use the start + volume- combination, it will take you to twrp. Found the solution finally but I still have some issues with graphics and Google account =/

Yeah, that's not working for me, still goes straight to Cyanogen Recovery.

Cyanogen Recovery problem

Really hope someone can help me on this. I clicked on update and now it shows 12.0-YNG1TAS17L
And when I boot the phone into recovery, it goes to a new screen of Cyanogen Recovery, it is totally different than TWRP that I have in on my phone.
I already make sure the phone "update Cyanogen Recovery" is unchecked. But no matter now many times I reboot, reset the phone. It goes to that new recovery screen.
Now I can't root my phone at all. I tried android-sdk-windows.....etc. It will not work.
Tried install it from recovery, it saids install failed.
IS there a way I can use TWRP instead?

Yes, there are many ways. Check custom recovery installation.

Use fastboot or a toolkit to reinstall TWRP, after it installs reboot back into TWRP, dont reboot the whole system and TWRP will stick. Otherwise TWRP will be overwritten with the stock recovery.

All previous steps I followed without error.

You haven't followed the instructions there, from what I can see you've issued the "fastboot reboot" command after flashing the recovery, which you're not meant to do, you're meant to force a reboot directly into recovery with the power + volume down combo.
Hi Heisenberg, I got the problem solved by using the toolkit. Now I got my TWRP back and rooted. Thanks for the info.

"My phone stuck into bootloop" I want to unlock bootloader in order to install previous stock rom

I have motorola fusion plus Its stucked into bootloop I have tried to flash it using latest version of firmware zip but it still not starting...
So I want to flash old version firmware but in order to do that I need to unlock bootloader
But I have not eneabled oemunlock from developer settings..
What should I do now?

To re-flash phone's Stock ROM there is no need bootloader gets unlocked before.

Tried Re-flashing the same version but its not helping me and its still stucked into bootloop
A bootloop means endless booting and shutdown loop. To end this you may
1. Boot phone into Recovery mode.
2. Once you are in Android Recovery mode select the option "wipe data/factory reset" and confirm the action in the next step.
3. Now go to the Recovery mode main menu and select the option "reboot system now" and your device will be restarted and will hopefully boot up successfully.
