I always have my phone on vibrate, but for some reason, after some time, vibrate stops working and it's as if I only had my phone on silent. The vibrate icon is still there, but my phone no longer vibrates on notifications and calls. Is there some behaviour that I don't know about that brings this about? ... is there any way to always have vibrate on?

Do you have one of the power saving modes turned on? What is your battery % at when the vibrate turns off. It may be trying to save power.

No power saving active. It just happens randomly. My suspicion is I may be pressing the volume button up or down in my pocket accidentally, but I can't be sure or if pressing the volume button while locked would even matter.

My phone would vibrate randomly. Very short vibration, kind of like haptic feedback when typing on the keyboard. Not very often, but once or twice every few days (that I have noticed). There would be no notification or led blink, and I don't think I have turned off any sort of notification beside vibrating for any app...
Usually sound plays when my phone is dying, maybe you have it on vibrate so its a small vibrate to let you know that power saving has come on? I have no idea, just poking in the dark here.
I happen to have a similar issue. Don't know if it relates but mine happens to vibrate when pushing the volume buttons. Strange habit of mine, pushing the buttons of my phone when it is in my pocket.

If you press and hold the volume down button, the phone will silent itself and turn itself into "vibrate mode", but that's not the case here.

Maybe but I'm not sure. I remember a few days ago I simply had my phone laying flat on my desk when I was working. It vibrated so I picked it up to check for message, only to find that it vibrated for nothing. There was nothing on the notification bar (there would be a triangle with ! if power saving is on). Weird

Put me down for a "me too". I'm getting the random vibrations once or twice a day (that I notice, at least). I had the same thing happen, too, that you described -- my phone was on my desk at work and then randomly did a short "half-vibrate" -- but there were no notifications or any reason it should have vibrated. Must be a bug...

Then your vibrate is not random.
Is it in your pocket or in your hand when it vibrates? I have heard of this strange phenomenon where you get these phantom vibrations on your leg where your phone would be resting against your leg in your pocket, but when you check your phone, there wouldn't be any messages. It used to happen to me all the time after I got a phone that would vibrate with incoming calls/messages, lol. It sometimes still happens, but not as much anymore. The human mind is so fascinating!

It's weird... After I posted it here, I have never experienced any random vibrate anymore.
I've had it but only while using go launcher. Uninstalled it and never had it again.

Hrm, i'm just curious now if this is only me or some others are experiencing this too. Recently I flashed HyperSensation CM7 rom and I'm using FauxClock's kernel. Before my phone would not vibrate when I put it in silent or vibrate mode. All of a sudden yesterday after I rebooted a few times, the vibrate started to work in both vibrate and silent mode. Now it won't stop vibrating for no reason. It's a double vibrate, and sometimes I would receive at least 3-4 vibrates in consecutive times. I check my phone, my notification LED does not light up, I don't have any messages coming in, there are no Facebook notifcations, I don't have any e-mail's coming in.
Also whenever I reboot my phone and it finishes rebooting and shows me my lockscreen, it vibrates 2-3 times of the same doulbe vibrate rhythm. Does anyone know where I messed up while I was playing with my settings? My phone does vibrate when it's on loud when I receive messages or calls or other functions like my alarms. It just didn't vibrate before when I put it in silent/vibrate mode. Now it won't stop vibrating and this has been going on for 14 hours. I'm going nuts!
My notification LED only blinks after I have hit the power button on the phone(unlocking it)...which is kind of useless because obviously when I tap the power I am accessing my phone therefore going to see the notifications displayed. Why is it that when my phone is locked/on standby that the LED notification doesn't blink right away until I wake it up and then lock it again? It is as if the phone went into some sleep mode and even calls,texts, and everything does not wake it up automatically.

I've got the same issue at some (random) times, where it doesn't start notifying until I check it myself. It sure seems like a *small issue*, but since it's there to help us out... well, it should be helping us out.
No matter what I try, the ringtone keeps defaulting to silent mode. Whether using the volume rocker or through the settings menu, it may show a ringtone volume or vibrate for a second, but then it reverts back to silent mode. Same deal after restarting. Phone is unrooted. Any ideas how to fix it or whats going on?

Having same problem. If I use shortcut switches or apps to change the ringer volume, I can't seem to get silent - I have to go into settings and switch it to vibrate from there. Then vibrate will work awhile but after some time I'll automatically switch to silent (the icon in the notification bar changes with this).
Pretty sure it can't be a hardware issue, since it does vibrate when I set it to. But I can't figure out why it keeps reverting to silent mode on its own.
Any ideas? It's a new One S, unrooted. Haven't messed with it in any way besides choosing to use a Launcher. And it's been annoying not being able to get notifications when I need the phone on silent.

Sent HTC an email asking about it over the weekend, they replied this morning and suggested I boot into recovery and wipe the cache partition.
Looks like it worked! I can now toggle volume/silent/vibrate using the volume rocker, or toggle apps, without having to go into settings - plus, it doesn't seem to automatically switch back to silent mode now.
I'll probably keep an eye on it for a day or two just to be sure, cos the time periods between reverting to silent mode weren't consistent - but it looks promising

Anyway here are the instructions they sent:
1. Turn the phone off (hold the Power key on devices with sealed battery)
2. Press and hold the Power key and Volume Down key to boot into Bootloader mode.
3. Press Volume down to highlight Recovery, then press the Power key
4. When the Black recovery screen appears, press Volume Up and Power at the same time to show the menu.
5. Press Volume down to highlight Wipe cache partition, then press the Power key.
6. When complete, press the Power key to reboot the phone.
The partition cache collects a lot of data for apps as they are running and keeps this data in the memory to help boot the phone up faster as well as boot up apps faster, this can hold corrupted files created when a third party application has permissions that conflict with the update program and clearing this cache will allow the system boot without the conflict.

I have this bug with my OnePlus One. When I press volume up or down, the usual volume panel doesn't appear and requires a reboot to work again. This happens around two times a day. If I press both up and down buttons, I can turn the phone on vibrating mode and back to normal. Really annoys me to have to reboot phone to change volume level.
Anyone having this bug?

I'm having a similar problem with the sound. It doesn't make the beeping sound when I increase or decrease the volume, so it's difficult to know the volume level unless i'm on a call or open the settings. Also the haptic feedback has stopped working even though it's enabled. Still had the same problem after restarting.
I can´t figure out what´s wrong, obviously "do not disturb" is off, the slider is always on "ring" and I am missing a lot of calls due to this issue. It happens randomly, I can´t really pinpoint at anything that seems to be the culprit.
I am wondering whether the full cover is somehow silencing the phone or if an app is doing it. Are there any default gestures for vibrate only I am not aware of?
Are you accidentally hitting Power Button + Vol Up? This pops up a toast that says "Calls and notifications will vibrate."

Any way to turn that off? ... I think that could be it, the way the full cover is made it would very well explain what happens when I sit down. The hinge is just above the power button.
