[Completed] Can't perform a factory reset even in recovery - XDA Assist

I'm using the Moto X (1st Generation) and running on CM12.1 on Android 5.1.1.
I wanted to change the rom to Dirty Unicorn. So I followed the normally process of changing roms by first going into recovery and performing a factory reset, wiping dalvik and etc... But when performing the factory reset., this error comes up. .
Everytime I perform a factory reset the apps are still around when I reboot. Any idea what the problem is.
Please help!!

[Completed][Q] Mistaked while installing CM11

Hey XDA Assist,
I just treid to install a new ROM for the first time. I'm using TWRP Recovery and I have the GSM HTC One Mini.
I made a mistake on the step where u usually have to clear dalvik and cache, i deleted /system too. so... Now I'm pretty much stuck without a ROM to boot

I wanted to but my CM11 zip file was also wiped from the phone. And now when I try to put it back my PC gives an error

[Completed] Boot loop on Android 5 - Android 4 is fine

Hi there,
I tried installing http://forum.xda-developers.com/nexus-4/development/rom-simpleaosp-t2932090 but my Nexus 4 is now in a boot loop. Before that I had a CM12 alpha installed and I'm afraid it did something to /system and /data since TWRP can't repair the partitions.
Lollipop factory image also does not work. Other flashed ROMs (with Android 4) work just fine.
I already checked the md5s.
What can I do to fix this?

I don't think this is a issue related to this ROM but to Lollipop in general. Nevertheless, I will post there as well.

[Completed] connection problem on MOTO X 2013 with new rom

i installed a new rom on my moto x ..,but after connecting with wifi the window is stuck at checking connection..this can take a while from a long time what to do ??
Hi, thank you for using XDA Assist. I would first try flashing the Rom again. If that doesn't work then return it to stock. [Completed] All the apps are getting optimized whenever i restart my phone

Please help me out, whenever i restart my phone all the apps are getting optimized automatically which takes very long time to boot up.
I rooted my phone. I am using Moto G X1068 with android lollipop 5.1.1. Cyanogenmod 12.1.

I rooted my htc desire 820 successfully using twrp but unable to get ota updates.I saw this issue persists for many rooted users.So how can I revert back to stock recovery.I tried to do factory data reset but it does not work .Where can I find stock recovery files so I can reflash it
