[Q] SoftBootloop(?) Recovery Problem - ONE Q&A, Help & Troubleshooting

Good Afternoon!
Here's the story:
I encrypted my phone.
I installed TWRP 2.8.6.1 Official.
I (tried) unsuccessfully to dirty-flash a ROM to fix a systems partition problem.
My phone gave me an encryption error and told me to format my phone.
I formatted my phone using TWRP and installed OxygenOS again.
My phone is now bootlooping into TWRP.
I've tried downloading OxygenOS again.
I tried formatting my phone again.
I tried a different OxygenOS Rom.
I tried CyanogenMod 12 Nightly.
Everything bootloops back to recovery.
I'm currently trying to find a Factory Image for the O+O and go back to CyanogenMod 11S to (try) and get it to just boot.
Is there anything I haven't tried? Any advice?

I flashed 11S via fastboot and then upgraded to a version of Cyanogenmod12.1 I found via the forums.
The stock CyanogenMod kept getting stuck in the middle. OxygenOS won't install, period.
My phone hates me.

[Q] Cannot get phone to boot

Randomly crashed this morning, now it is in a perpetual bootloop no matter what I do.
I have a Nexus S 4g
I did a factory reset and that didn't work.
I tried to use CWM USB mounting to copy over a new rom and that didn't work.
I updated CWM to the latest version and sideloaded CM10.1.2 and the latest gapps, and yet the bootloop persists. I'm positive that it installed both of these zips successfully.
My last resort was trying to return it to stock using a a 2.3.4 image and relocked and unrooted. Still won't boot up.
What can I do to fix this? I'm confused. Is it possible that my phone is entirely broken?

Hello,
I recently was running Cyanogenmod 10.1.3 (i think... it was the latest stable build) when I got a notification to update to 10.2.1. I did so through the cyanogen updater and it seemed to work fine but the phone would randomly reboot. Finally it did so but remained at the initial loading screen. I then booted into TWRP and flashed the 10.2.1 again. Same deal it just wouldn't boot it remained frozen. I then tried to reflash 10.1.3, the previous version I was running. The phone imediatley booted into fastboot with a flash failure indicator despite TWRP saying it had been sucessful. I have since tried to install the Mototola's OEM firmware, as well as do a restore from a backup I had made in TWRP. nothing works and I can't move past fastboot. Im interested to know why this is happening and how I can reflash properly without getting an error? Any replies will help. Thanks.

I recently upgraded from stock opo cyanogen to the cyanogen nightlies. now whenever i go back to TWRP to make a backup, it breaks my os and i get stuck in a bootloop. i have to factory reset my phone and set everything up again, and all of the backups are just bootloops. i already tried re-flashing TWRP and cyanogen, both through the TWRP manager and through the fastboot flash recovery command. if anyone has a solution it would be much appreciated.

it turns out it had nothing to do with twrp, it was a corrupted verion of gapps.
i just reflashed everything. (cm12, new gapps download, and supersu)

TWRP/CM12 bootloop

Hey guys,
Got a weird problem with my OPO on a last week. With no reason my phone got a bootloop after a regulair reboot. Nothing special was installed and i can see no reason for such behavior.
I tried to reflash it and so on, but that's there the weirdness starts.
I can install CM11S and CWM of Philz recovery, but any attempts to install TWRP (6.0/6.1/7.0) fails (vol. down + power just leads to reboot). Same for the latest CM12.1 builds. Tried installing those by using several toolkits and flashing via fastboot+cmd. No success. Bootloader was unlocked and seem to be still unlocked. Tried wiping system/data/cache via CWM/Philz - still no chance to get TWRP and CM12.1 work (results in a bootloop).
Anyone experienced such problem, or any ideas on how to solve the problem?
Any help appreciated.
Hey dont know much but it might be a modem firmware issue. Flash the latest cm12.1 modem file and then try installing cm12.1. Generally when i get stuck in bootloop i use the mac osx toolkit to boot the phone into twrp recovery via fastboot mode. But cknsidering you have already tried that then modem issue had given me bootloop once when i had upgraded from 11 to 12.1

Opo in bootloop with almost any stock rom

Hi, the problem is the one from the title, i want to start from the beginning: a couple of days ago, i was on facebook (on COS13, no twrp upgraded from COS12.1 with the incremental fom the stock recovery) then facebook crashed and then my phone rebooted and entered in a bootloop(and coldn't boot into stock recovery to wipe cache) I tried going back to stock with this guide: http://forum.xda-developers.com/oneplus-one/general/guide-return-opo-to-100-stock-t2826541
but still bootloop with no possibility to access stock recovery and twrp , then i tried to flash cm 11s with oneplus one toolkit but still bootloop(but i was able to boot in twrp), i also tried bacon root toolkit to flash cm 12.1 but guess what, still bootloop with no acces to any recovery, then i tried coloros, i flashed it with oneplus one toolbox: http://inthiaano.com/oneplus-one-color-os-installer/
but i get an "invalid nv partition" then it shuts off. Currently i'm on cm11s XNPH44S in bootloop (EDIT: I can access also stock recovery now)
