1+1 running on cm12. my problem is that the ringtone and notification volume decreases to the low end automatically on my phone after the cm12 update. any help appreciated, thanks in advance.

I have the same problem. I have done a factory reset and reinstalled CM12. The ring volume still drops randomly. I think it is a bug. If it isn't resolved soon I may go back to CM11.

used Priority Tab with Indefinitely option. This will stop changing the volume level.

I updated to build LRX22G and the problem went away. After the update the priority interruptions were already set to indefinitely, I never changed it so I don't know what it was set to before the update.

I had the same build and was still having the issue.
changing into priority mode was the workaround to fix it.
Try this app, it will increase ringtone volume to 100% before receiving call.

It seems after I took the ota my phone will no longer vibrate for calls or any other notifications. I seen an at&t user had the same issue and a factory reset fixed it. Didn't work for me. I was stock rooted. I've now unlocked the bootloader, twrp recovery and installed cleanrom. Still have the same issue. I use the volume button to change it to mute with vibrate and the phone does vibrate, but the squiggly lines don't show up around the crossed out speaker. In the sound settings Silent Mode is set to Vibrate. Any ideas?

That's pretty odd. I would try to ODIN back to stock and then open bootloader again and root ETC.

This also happened to me after I had installed and then uninstalled a sound profile editor.
To fix it I found that the following worked for me.
1) First pull down the menu from the home screen and set to SOUND.
2) Next go to Settings...Sound.
3) Scroll down and turn on Sound and Vibration
4) Pull down menu again and toggle to vibrate.
Now the correct icon will show in the status bar and you will get vibration notifications.
I totally found this by accident and it worked for me, hopefully will work for you too.
My audio has disappeared for notifications. I think its part of profiles screwing something up but I have been unable to reset things so I can get my notifications to work again.
When you try to set the notification sound none of the previews work. Setting a ringtone works, just not notification.
I am on stock CM11S for now.
Does anyone have a workaround? I thought this CM bug was fixed back in CM10

Could you get this working? I'm having the same issue, no notification sound on any app, I can't even get the sound preview on the settings app.
Are your notifications separate from ringtone?
Settings -> Sound -> Volumes -> Link ringtone.....

I have the same issue:
Symptoms:
No sound, just vibrate whenever there is a notification
Ringtone works perfectly
when going through the sound settings menu for notifications I get a list of all notifications. Clicking on any of these results in no sound whatsoever. (I used to get a preview of what that tone sounded like).
I've tried everything short of a factory reset
I'm running the latest OTA, but this issue was occuring before the latest OTA as well
I installed the infamous cm11 theme (https://play.google.com/store/apps/details?id=com.infamous.cm.theme) and then uninstalled it. I think my sound stopped working then but I'm not sure.
Could it be my notification sound files have been deleted? Where can I find them on my system? Browsing through I don't see anything under /notifications or /media/notifications
Some searching unearthed this bug (https://jira.cyanogenmod.org/browse/BACON-6) . It seems to describe my symptoms but I've never used profiles. I used quiet hours but have disabled it completely.
Please help! I really don't want to factory reset. And if I have to, how do I back everything up and then restore (whilst still having my sound fixed?)

This fixed it. For reference go to Settings -> Sound -> Volumes and uncheck the 'link ringtone and notification volumes' box. Max out the notification volume and you should be golden. You can recheck it again if you'd like.

The notification .ogg files hould be in /system/media/notifications folder.

I'm running RobbieL's sabermodded slim.
On occasion when I get an incoming notification alert the speaker starts crackling and hissing. I can usually get rid of the sound by choosing whichever app gave me the alert in the pull down notifications. In some instances I need to re-boot the phone to get rid of it. It has also occurred a couple of times when answering an incoming call.
Is this familiar to anyone? Is there a fix at rom, kernal, setting level?

So ringtone goes silent after some hours. Doesn't really matter if phone is in use or it's during night.
First I thought it was ROM issue, but this happens on CM12 original image.
The only thing that is "the same" that I flashed recently is firmware_cm12.
It was working flawlessly before on cm11 and slimsaber roms.
I've tried installing only CM12 without any app and it's the same this morning.
Notifications, like SMS, are still working and sound is played when I receive them. Only ringing is dead silent.
I checked many times that there's no wonky profile with quiet hours or anything else, even tho the issue is fixed by rebooting phone...
Please help!

I have this issue too. More interesting is that if i try to select another ringtone it is silent too on the ringtone selector page (where are lot of another ringtone). All ringtone is silent. Only the reboot help... Of course the ringtone volume is up
The last CM12 that i tested was the 0116, so it's looks like this bug is present in the latest too

Yes, I get the same behavior, just forgot to write that on xda
Same happening for CM12 on my Nexus 4 as well. Do you use any profile switcher utility?
I use Profile Scheduler which has normally worked just fine.

Each time I make a call, or receive one, the phone call volume is set just a tiny bit above zero, so I can hear nothing and I have to crank it up. I have to do this each time and I still did not get into the habit of doing it (partially cause I do not subconsciously want to do it forever), so the frist few seconds are always just awkward. It has nothing to do with the mixer_paths.xml, cause I have a "90" settings (by default) set there, so it must be something else.
I know of no setting that would make a default in-call volume globally for a phone. This happens in normal calls, facebook messenger calls.. I think I noticed this happening first time with facebook in-call volume.
Is there anything to be done?

I am on CM11 44s, I set the volume to 2 notches and it resets back to full for the next call. The setting is not getting saved or something.
This problem has cropped up a couple of days back, previously it was working fine.
Anyone having any idea?
I stopped having this problem, after I upgraded to Android 6.0.1. However, in call volume dropped even when it is maxed out, which is kind of worse situation actually.
