Which headset are you using?
I have the same problem with the Samsung Galaxy S4/Note 3 earphones (they have Android compatible volume buttons). They have large, rounded buttons:

If you are using the same set, this is expected behavior. It's not normal, just expected. The OnePlus One doesn't work properly with them. I first encountered it when I modified a Beats cable with iPhone inline remote to work with Android phones by taking the remote from the S4's headset and wiring it up to the Beats cable. At first I thought I wired it up incorrectly, but then I tested the cable on both a Galaxy S5 and a Note 3, on which it worked perfectly.
The volume buttons work fine; it's the play/pause button that's a bit temperamental. Most of the time, the phone doesn't read it as "play/pause", but instead "volume up".
The volume buttons:
The play/pause button:
So when you double press it to skip track, it steps up the volume twice instead. I've had some success by pressing volume down and play/pause at the same time, but you can't use it to skip track.
For the time being, use a ROM that supports "Use volume buttons to skip track".
Side note: the older Samsung headset that came with the Galaxy S3/Note 2 works properly on the OnePlus One. They have smaller, rectangular buttons:
Thanks @vantt1 for your reply...ya absolutely right..the older earphones without volume rocker is working fine but shall not CM provide some update regarding this error...
Also ROM updating wont make warranty lapse?

Single-button inline remotes and Apple headsets work fine disregarding the volume control. At the moment this seems more like a hardware-related issue rather than a software-related one, because it persists across different ROMs and kernels, much like the One's incapability of supporting CTIA headsets.
Software modifications shouldn't void the warranty, as long as it is not the cause of a hardware failure.
Just got the Samsung Level On. Same thing happens; volume buttons are fine, but play/pause = volume up. This really sucks, given the limited options that have an Android-compatible 3-button remote.
Right @vantt1

My rom is Android revolution HD 6.4.0
My bluetooth keyboard is a dell Y-RAQ-DEL2
Seems to work ok. Wish I had a bluetooth mouse to go with it. Pairing is simple. Just go to the bluetooth screen on android. Now turn the keyboard on and hold down the pairing bluetooth button on the back til the dell keyboard is detected on the android side. A key will be listed so just type it in and the pairing is finished.
Rom: VI 4.0
Bluetooth keyboard: Apple bluetooth keyboard
I did pair the mouse but it didn't seam to do anything
I've had luck using the keyboard for the Motorola Xoom/Atrix. You just need to put the keyboard into computer pair mode by turning it on while holding keys V+A+R, and you can pair it to admitting else with Bluetooth. I've used it with my sensation, Xoom and touchpad.

Hi,
I just bought this one:
http://www.amazon.co.uk/gp/product/B004EFZ2FA/ref=olp_product_details?ie=UTF8&me=&seller=
So far it's okay. The keypad is fairly squashed and is a bit awkward to use because it's laid out in a 'chocolate box style' rather than staggered, but at the same time it's not quite small enough to hold in your hands and reach all of the keys with your hands. The keys feel pretty cheap, but for £20, what do you expect, especially as it has a mouse too : )
However, having said that, now that ICS is here the mouse function works everywhere, not just in certain apps. This gives almost full control of the phone. The only thing missing initially (that I've noticed) was the ability to press 'Menu'. I've solved this by altering the keyboard layout file as per the instrustions here: http://chris.boyle.name/2010/08/android-keymaps. I chose to re-map the 'Alt Gr' to menu.
By trial and error I'm finding all sorts of Ctrl key combinations to do various things, such as open bookmarks in the web browser (Ctrl B), history (Ctrl H) and so on.
On the plus side it will be small enough to go in a suitcase to take on holiday along with my MHL cable so I can plug it into tvs in the hotel room. Yeah baby!!!
So I have searched and searched and tried everything I can find. No go.
I'm on the Evita HTC One XL, running cm10.1 latest nightly. I have tried the M2 version as well as a ton of other nightlies. Media volume and speaker volume are fine. However my in call volume out of the earpiece is very low. I struggle taking calls outside. I know it's not hardware because Sense ROMs have excellent output. I tried CarbonROM as well, which is AOSP and CM based, and have the same call volume issue.
I have looked at most of the volume boosters on the Play store, but they never affect in call volume for earpiece. Only headset, bluetooth, speaker.
Can anyone suggest any tweaks or mods to help me? I tried adding ro.config.vc_call_vol_steps=15, but all that does is make volume steps smaller, without increasing max volume. I see that for other devices there are gain settings, but I haven't been able to find anything for the Evita.
If I can provide any additional information let me know. Not sure a logcat will help in this instance.

I am finding the same thing as well. The volume seems to be very low in the ear piece.
however speakerphone is nice and loud.
Here is a workaround for the volume bug.
Bug - The call volume is really low and as of now we need to press 'volume down' and then 'volume up' to get the max volume in call.
Workaround - Use an automation software to do this work for you everytime you receive/dial a call.
I use 'Automagic Automation' for my needs. (I like it's user interface better than Tasker's). This will work with Tasker too.
Below is a screenshot of what happens. (The Flow)
Explanation - As soon as I pick/dial a call, my Flow first decreases the volume and then increases it with a gap of few milliseconds automatically without the need to press the hardware keys to do so. (I tested it only for the calls received on the phone, not when I dial a number )
Note - It only works this way and not if I directly increase the volume to max. The time gap is also important. Otherwise it fails.
Press 'Thanks' if I helped!

Ok so my title states it pretty well. I have a bunch of bluetooth devices I use, JBL Charge, Car Stereo, JayBird headphones, and every other android device I have had you can control the volume from the device. However with Googles new nexus 5, it seems that feature doesnt work? Now I have to use the bluetooth speaker to change the volume. Any ideas on a fix?
Same here, seems to be a bug. Use an equalizer app to lower the volume

Ok, any recommendations?
Works fine on my Sony MDR 10 rbt headset.. So it does not seem to be an overall n5 bt problem
Mine and my wife's n5 have this problem. Default bt audio output is too loud and is distorted. Sometimes the volume can be controlled by the volume buttons, most of the time not.
I have the default eq like this

The problem looks to be app compatibility with 4.4. Pandora doesn't allow any bluetooth volume control, but Slacker which was updated Nov 8th works fine.
I used the volume buttons on my Nexus 4 to turn down the BT audio all the time when I listened to Pandora. It's a system ride function so it shouldn't matter what app is playing audio

This is not a Nexus 5 issue. It is Kit Kat issue on all devices that use the default Google Bluedroid Bluetooth stack, meaning Nexus devices and Google Play versions (other devices running a modified stack may not be affected).
Supposedly, it is an extension to Bluetooth AVRCP 1.3 that enables Bluetooth audio devices to directly control Android's system volume. It seems some genius thought two independent volume levels (headphone volume and handset bluetooth level) was too confusing for folks. Clearly they did not think about (or test) that some Bluetooth audio devices with their own volume controls might default to a very high or low level, and control of only the handset's bluetooth level would not provide sufficient range of adjustment.
So once again we have Goofle googing up bluetooth in Android. Please share your disappointment in the following threads so that we might see a fix ASAP. Guessing they are facing law suits for hearing damage if it is not addressed quickly.

Like the title states,
I have recently purchased the V-moda crossfade LP from amazon (on sale), and realized that the 3 button controller doesn't work. Normally, the center button work as single tap (pause/play) and double (next song) and tripple (previous song). And i have tested this same cable on my brother's one plus, and it worked fine. On the Z3, it will just increase the volume.
That being said, other headphones i have such as xiao mi piston works fine (volume and centre button), all responsive. My Sony headphone XBA-C10 centre button works perfectly on the Z3 (not the volume buttons, which is normal).
I think there is something wrong with the Sony ROM that's causing the center button not working (only increase volume) because the same headphone (crossfade LP) center button works on the one plus one.
It depends how many pins (rings) headset have, and in which order. Maybe they are not compatible fully.

So does that mean if i change to another cable it will work right?
You don't need a full cable, an adapter should be fine, should easily find one online. My old Philipps earphones/headset came with an adapter to be compatible with more devices.

I've tried two adaptors that my brother has, and none of them works.....but rather screw up my phone and i had to reboot to get it back working again. Would you mind sharing what adaptor you use?

I used to use the one that came with my Philipps headset on my dad's iPhone. I have not used it on the Z3. It had a label/tag on it showing which phones the adapter was needed for. I gave it to my father who put the headphones and adapter pin somewhere so I can't test it.
I am pretty sure there are only 2 variations in the pin order, depending on which phone you use. Of course do correct me if I am wrong.

I have a galaxy s21. I just got a warranty exchange and I noticed my Bluetooth volume on my car stereo is not at loud as my old phone.
I have both phones in my possession and played the same song individually on each device and I noticed that the new phone's volume is lower than my old one. All the settings are the same on both phones.
Any clue what's going on? Thanks!

I've heard this happen occasionally. Samsung browser sometimes triggers it. Eventually it resets spontaneously. Oddly I haven't seen this behavior for months now. Same load and app versions. It maybe a Developer option setting.
Check all settings again including volume limiting and bt settings in Developer options. Try toggling those, try turning off Disable absolute Volume. Use the master volume control and cycle from zero to 100% a couple times.
Try this as well if the device has this option, may only apply to Wearables:

It can be quit the pain but it's almost surely a settings issue. There are apps that allow you to access the hidden settings that presumably also would give access to absolute bt volume. I haven't had to try that.
For best audio use the C port hard wire output.
Enable the UHQ upscaler in advanced sound settings.
