sd card

EVERYTIME i use my phone:
SD CARD UNEXPECTEDLY REMOVED
REMOVING SD CARD
PREPARING SD CARD
I can't find any reason why it is doing this.
We have saved everything off of the SD Micro Cards, and reformatted, yet same issue - has anyone found a solution yet?
Thanks,

Custom Rom or no? It possibly be this but cant put my finger on why it's doing this. Try formatting it on your computer, then on your phone. If all else fails then maybe factory reset your phone but remember that you'll lose EVERYTHING when you do this so know what your doing...

Hi, thank you for using XDA Assist. Have you formatted the card? Try re-formatting it to FAT32.

Hi there,
Have you tried another sd-card? Or reformat sd in recovery?
That's the first I should have tried, and in that order.
FYI:
[GUIDE] [INFO] All about SD CARDS - which one, why and how! Posts 1-3.
Recover DELETED files from you sd card, even on your PC.
Good luck
