You can't flash anything but stock since you have a locked bootloader and xt926. please re-download the fastboot package for jelly bean, don't modify anything (you can't add any custom recovery) and post screenshots of any failures in rsdlite.
For flashing roms, you will have to use safestrap instead.

Are you sure this is XT926, not XT925? Please take a new screenshot of RSD Lite (before you flash) with the model information fully visible, as in this screenshot

Yeah, of course i have XT926. On the phone's back side is XT926. And i flashed already safestrap for XT926 when the phone worked, and use Droid Utility for XT926.
But i has some trouble with battery and want to flash once again. Last time before stuck in fastboot mode i tried to flash LTE modem for my local frequency from XT925(Telcel). Maybe this can explain why i can't flash files for XT926. Maybe i flash fastboot for XT925 and phone think now that i have XT925, but it's wrong. But as i see - i have correctly XT926 fastboot and can change two fastboot version with RSD Lite or Droid Utility:
- AP Fastboot 10.9B(*) (sha-9d3ad55, 2012-09-15 04:39:04)
- AP Fastboot 10.9B(*) (sha-1e9f557, 2012-11-03 09:05:20)
Maybe i need a newer fastboot for JB (maybe 4.2?)? Anyone have it? I can't flash latest stock rom from Motorola over official Motorola utility too. Has the same error with gpt_main.
Can you please unzip your Rom file. Inside that file will be a /bat file called "flash_fastboot.bat"
Run that with your phone in fastboot mode.
If that doesn't work download this fastboot.
http://forum.xda-developers.com/showthread.php?t=1953948
And put "mfastboot.exe" in your unzipped Rom directory, rename it to "fastboot.exe" (to replace the old version that's in there)
and try again.
Let me know how it goes.

Thanx, Brad!
In original firmware like VZW_XT926_4.1.1_9.8.1Q_39_VQW_CFC.xml.zip isn't file flash_fastboot.bat
Only flashboot.exe in Windows directory
So i replace Windows/fastboot.exe with mfastboot.exe from the link. And then flash once again with RSDLite.
Have the same result with gpt_main and downgraded security version.
And i can change two fastboot version, but can't flash anyway something with the both version.

I'm at work atm when I get home I'll post the script.
Are you using Windows?
If you are you need to move the fastboot.exe into the main directory and run this file from the same directory.

Yeah, i can enter into recovery mode without any problems.
And already press + and - to get a recovery menu.
Flash once again from desktop in recovery mode? Or place image files into sd-card? Or place an archive with ROM into sd-card? =)
Interesting, Can you please delete the line

Yeah, i understand what means this error. =)
I've downloaded the latest original version that can find - http://forum.xda-developers.com/showthread.php?t=2035131
As i know, this is not leaked version. Just original ROM-image from Mototola. Or?
And the original firmware from Motorola utility also have the same errors, but i don't know why....
[/COLOR]Wait, dont. but we need to find the update.zip for the xt926 razr hd.

Sorry I have hit a wall and dont know how else I can help you.
I have an XT925 so I cant even try to emulate your problem on my phone.
All i can say is to try a different version of the firmware?
Brad
One last attemp, download this file
"http://www.mydroidfiles.com/downloads/Satman80/delta-ota-Blur_Version.0.7.2-9.1.41.XT926.Verizon.en.US/delta-ota-Blur_Version.0.7.2-9.1.41.XT926.Verizon.en.US.zip"
then follow these steps,
"http://www.droidforums.net/forum/droid-razr/213197-offical-ota-zip-installation-guide-droid-razr-razr-maxx.html"
I need some help right here. I have flashed XT925 by mistake with stock veryzon 4.1.2 (XT926 firmware). I'm stuck in fastboot mode, my Bootloader is LOCKED, thats really weird because I was unlocked before I've tried to update via OTA (my phone was bricked after that OTA update fail). I tried to flash Retail Brasil JB and some other firmwares via fastboot and RSD Lite 6.1.5 with no results... this is what my fastboot screen shows up:
---------------------o---------------------------------
Device is LOCKED, Status Code: 0
Battery OK
Connect USB
Data Cable
downgraded security version
update gpt_main version failed
failed to hab check for gpt_backup: 0x35
failed to load GPT
CID Read failure
Invalid CID status 0x69
No SP partition found
Fastboot Reason: Invalid GPT
----------------------o---------------------------------
Hope you can help me :S
Are you sure you were unlocked? Seems to me you may be screwed.

Yes I'm sure, my status code was 3 not 0. What do you think I can do now? Is there any solution?
I remember seeing something about the gpt while searching for a solution for a different fastboot error. But like you I'm to lazy to search for it. Since it is your problem you probably should.

I don't know what to do, i tried a lot of possible solutions out there but I can't flash a single file to my phone. I've tried via fastboot and RSD 6.1.5 with no results, and I can't start recovery, just fastboot mode showing this screen:

I tried the newest Retail Br, Telcel Mexico and one from Veryzon for XT926 with no results.
I can't flash anything in fastboot or in RSD Lite, so maybe I will take it to Telcel, they will send it to Motorola and let's see what they say. Hope they don't notice that I unlocked the bootloader, although it says that is LOCKED and status = 0... let's see what happens
Any other ideas? Please, there must be something to do to fix this.

Make sure your phone is recognized in fastboot mode. While in fastboot mode type the following in the command prompt:
Code:
fastboot devices
You should see the devices serial number in the output.
If that works type the following in the command prompt while in fastboot mode:
Code:
fastboot oem fb_mode_clear
Then type the following:
Code:
fastboot reboot-bootloader
When it reboots back into the bootloader attempt to flash the stock firmware again.

If this is truly what you get when you try to flash then I can guarentee your phone is not connected correctly. It should say USB Connected where it currently says Connect USB.
---------- Post added at 04:31 PM ---------- Previous post was at 04:27 PM ----------
Make sure the correct drivers are installed. You can get them from HERE. Note that windows will remove your drivers at will. So even if you had them installed before, install them again.

It doesn't work. I've just tried that and I just can't flash a single Rom. The "fastboot oem fb_mode_clear" command doesn't work, I can't clear my fastboot screen on my device.
Sorry but my phone its connected properly, it says usb connected at the end of the screen. I reinstalled drivers too, but nothing changes, so maybe I'm screwed.

Power it off and charge it with a wall charger. I believe you cannot fastboot if the charge is below 50%.
Where does it fail? What does the utility say?

What are the errors you get when flashing the stock firmware for your carrier?
Hi all,
My moto g has a problem. It is stuck at the "warning bootloader unlocked screen"
I can boot into fastboot and flash stuff but when i try to boot it keeps displaying the "warning bootloader unlocked screen"
i have tryed several guides to flash orignal firmware but somehow it doesnt work.
Stange thing is is that i have CMW installed but when i flash TWRP or something it says its ok but then when i go to recovery CMW always comes back.
Had the same problem, white boot logo. Download from here http://forum.xda-developers.com/showthread.php?t=2646415 During the process it will say something about boot loader, but never mind. U should have a OTA update when it finished and will lose root, but its not a big problem

I went back to my carrier rom and It worked... after a few flashes on lollipop I couldn't get it to work even on stock lollipop...
hope it helps.
Try with mfastboot-v2. You can find the link with a quick search. mfastboot may help with flashing larger system images than normal fastboot. Just give it a try.
And if you are on lollipop & trying to downgrade to kitkat, skip flashing motoboot.img & gpt.bin

I had the same problem, and tried everithing said here, the only thing that worked, was mfastboot - v2, thank a lot for the sugestion.
Device not seen
I'm having the same problem – stuck on "Warning Booloader Unlocked" screen. Can't get to the bootloader screen. 'adb devices' and 'mfastboot devices' both show empty device lists, which means that I can't execute any adb or mfastboot commands...
Moto G XT1032 (Gen 1, 2013), attempted to install 5.1 firmware using steps at www . droidviews . com/restore-moto-g-to-stock-and-downgrade-firmware (sorry, not allowed to add live link). Bootloader unlocked, USB drivers installed, USB debugging enabled. All mfastboot steps executed without error, but I missed sparsechunk.0 – duh. Finished with a 'mfastboot reboot', and ever since, can't get past the bootloader warning, and no device seen by adb or mfastboot... Can't imagine it's a connectivity or PC driver issue, as was working fine minutes ago before the reboot.
All the solutions I've found depend on adb or mfastboot being able to see my device. Any tips to get the device seen again?
Thanks!

Problem solved – eventually got into bootloader screen on phone, after at least 20 tries... If you have same problem, keep on trying.
I flashed the cm14.1 and gapps, worked fine... then i flashed the Google assistant zip on my moto g xt1033. After that, it would not boot up to anything.. I tried getting it into recovery but simply no response... when i press the power button for longer, it just shows the white page and the unlock bootloader details... then turns off back again.... then i try to get it into recovery, same situation... Whattodo?
Before this, to install the animated nav bar, i got the SYSTEMUI pk, modified its rw permissions as given ( http://forum.xda-developers.com/moto-g/general/cm-14-1-pixel-nav-animation-button-moto-t3509255 ) here and followed the same thing,, the navbar showed up with no problems and i used it with no problems at all.....
Please suggest a solution....
(awkwardly enough, i am experiencing it a little warm even after i'ts been close to 30-40 mins after is shut itself off)

Yes i can access bootloader menu, sorry i took so late to reply because i was on a short trip lol..
SO, i just started it back by connecting it to PC via USb and it shows battery low(charging).
The bootloader is 41.1A(*)
Charge it for 4 hours and flash stock ROM using mfastboot from your PC.
Or flash TWRP from your PC, wipe all partitions, reboot and flash again CM+gapps.
In addition to what others have suggested, you can follow the instructions on the link posted below. Basically, you would unzip the file on your computer, boot into bootloader on your phone and run the 1-step bat file. After a few minutes, you will automatically be on 4.4.4. It is easy to re-root and flash a recovery (I used Philz, which offers you the ability to root in its reboot menu). Lastly, I updated the twrp to the latest and flashed a new ROM.

OK just to give you some information:
So this is what happening to me:
I flashed wrong files on my moto g. I think the files i flashed on my moto g were of Moto G3 or something. So my phone was stuck in bootloader rebooting.
I gave it to a shop and he got it back to kitkat with a flickering screen. Then i got the OTA and i was updating it to lollipop via OTA. It then went off., i dont know what happened but it would not even turn on by pressing power button. Then i connected it to a charger and notification light was blinking. Eventually, i got here and tried the procedure. So my mobile was recognised as qhsusb_bulk and then i followed the procedure further and then it was also recognised as qualcomm 9008 etc.
I flashed the blankflash and my bootloader screen showed up with a bootloader of 41.1A(*). Device is locked. Status code:0.
so when it comes in fastboot it shows:
Fastboot reason:Failed to initialize partition table
USB connected.
Now i tried to flash stock rom

Following the brick, i gave my phone to repair in a shop. I got it back on kitkat. So i eventually got the OTA. then i updated to the OTA and my phone went off half-way. Now it just reboots to the bootloader and has notice :
Fastboot reason: Failed to initialize partition table
Believe it or not, when i attempted to use mfastboot commands, mine didn't work either. I'm not sure what went wrong, as all of the commands succeeded. However, when I flashed the bat file I described above, my phone was back in action less than 5 minutes later. I don't recommend using the OTA to update from KitKat though. It's far safer and better to install Philz , obtain root, flash twrp and then flash an updated ROM of your choice.

I'm not sure. My bootloader is unlocked, but I used fastboot (threads in Moto G General which includes one's by Lost101 has the specific instructions) to load Philz. Once flashed, boot into recovery. Then when you boot back into system, the recovery will ask you if you want to root. Select yes and you are good to go. Lastly, flashify app in the Play Store can be used to quickly flash twrp. I guess another way to root would be to flash superuser (also in lost101's threads in the OP section) via recovery as well. There are easier and quicker steps to do what I outlined, but the main advice is to use the bat file to quickly restore your phone to GPE kitkat and afterwards avoid updating through Motorola.

Hi There,
I hope someone can advise and possibly provide help resolving this problem.
The Moto G4 XT1622 is not able to boot to OS, It only boots into bootloader, cannot go into recovery and cannot go into Factory Mode etc.
The error i get on the phone is....
"Start Up Failed"
Failed to verify hab image boot
Failed to verify boot image
ERROR: Failed to pass validation, backup to fastboot
Fastboot Reason: Fall-through from normal boot mode
I have tried to clear fb_mode and tried to unlock the bootloader without success.
I have tried various stock roms to get this fixed. The phone has never been unlocked and not been rooted.

It looks to me like you're trying to flash a firmware that is a downgrade from what was already on the device. It's probably blocking you because it won't allow a downgrade. Flash the version that the device already had on it or flash a newer version.

I throught that would be the case but i have tried the 4 roms available for this device and even the 7.0 OTA rom but nothing if able to flash. I cannot even unlock the bootloader.
Thanks

You may have to faatboot flash the individual .img(boot, system, cache, recovery, etc..) files by themselves.

Do you have USB debugging enabled?

Hi,
I dont think so as I cannot check due to the phone not able to get pass bootloader. I tried to use the fb_mode command but still goes back to bootloader saying filed to start.
Thanks..
